Diffuse, progressive hyperpigmentation: an unusual skin manifestation of mycosis fungoides.

Pigmentary changes in mycosis fungoides usually occur in association with poikiloderma atrophicans vasculare or following therapy and regression of lesions. Several cases of hypopigmented mycosis fungoides have also been reported. We present the case report of a patient who developed pruritic, diffuse macular hyperpigmentation of the skin. Biopsy specimens from hyperpigmented skin revealed histologic and ultrastructural features typical of mycosis fungoides. Giant melanin granules were found in the tumor cells, as well as in keratinocytes and Langerhans cells. As far as we know, this is the first report of cutaneous hyperpigmentation as a single presenting sign of mycosis fungoides.

Etretinate-induced skeletal muscle damage.

Three patients who received etretinate, two for psoriasis vulgaris and one for exfoliative dermatitis, developed clinical and electromyographic features of muscle damage during treatment. In one patient histological and ultrastructural findings indicated segmental muscle necrosis. Withdrawal of the drug led to clinical recovery and normalization of muscle enzyme levels and electromyogram. To the best of our knowledge, this is the first report to show etretinate-induced reversible skeletal muscle damage.

Etretinate treatment of the nevoid basal cell carcinoma syndrome. Therapeutic and chemopreventive effect.

A patient suffering from the nevoid basal cell carcinoma syndrome, in whom various surgical and conservative treatment modalities had been tried previously, was placed on oral etretinate therapy. A therapeutic effect of the etretinate on existing basal cell carcinomas and a prophylactic effect in inhibiting new tumor formation were well demonstrated during the 13-month period of treatment.

Two years of follow-up of oral ketoconazole therapy in 60 cases of pityriasis versicolor.

In a series of 60 patients (42 males, 18 females) with widespread lesions of pityriasis versicolor, 200 mg of oral ketoconazole was administered daily for a period of 24 days. Routine laboratory examinations, including microscopic and Wood's light examinations, were performed prior to initiation of treatment and twice during the treatment period as well as 3 and 6 months, and 1 and 2 years after completion of treatment. In 95% of the patients the clinical cure was maintained after 3 months but after 6 months it was still present in only 80% and after one year in only 57%. At the end of two years 60% showed relapse of the disorder. In 75% hypopigmented macules persisted after 6 months but without microscopic evidence of the fungus. No side effects were reported. It is evident that oral ketoconazole is effective in maintaining a clinical cure mainly during the first 3 months after treatment. The question as to whether side effects might appear with a more prolonged course of the drug should be given serious consideration before continuation of treatment is contemplated.

Cutaneous reactions to nifedipine.

Nifedipine, a calcium-channel-blocking agent, is widely used as an antianginal and antihypertensive drug. Cutaneous reactions to nifedipine are extremely rare. A review of the literature revealed only three additional cutaneous eruptions which include vasculitis, erythematous edema of the legs and erythromelalgia. We present the first case of fixed drug eruption due to nifedipine.

Rapid response of transient acantholytic dermatosis to selenium sulfide treatment for pityriasis versicolor.

The observation of a 51-year-old male patient with transient acantholytic dermatosis (TAD) is briefly reported. The discrete eruption, composed of erythematous escoriated papules and papulovesicles, moderately itching, was located on the back and chest, more permanent during the summer season. The biopsy revealed suprabasal acantholysis and Darier-like elements. Lesions of pityriasis versicolor, confirmed by microscopic examination and fluorescence in Wood's light, were intermingled with TAD in the same patient. A topical treatment with selenium sulfide brought TAD lesions to the disappearance in 1 week. Comments are made in relation with the possibility of Malassezia furfur to induce acantholytic phenomena.

Sweet's syndrome and subacute thyroiditis.

A 63-year-old woman developed simultaneous occurrence of acute febrile neutrophilic dermatosis and subacute thyroiditis, thus suggesting that both diseases had a possible identical viral etiology. As far as we are aware, this is the first report of the association of Sweet's syndrome and subacute thyroiditis.

The heterogeneity of tumours associated with epidermodysplasia verruciformis.

A patient with epidermodysplasia verruciformis also had lesions of seborrheic keratosis, irritated seborrheic keratosis, and intraepidermal and invasive squamous cell carcinoma as well as infiltrating squamous cell carcinoma associated with eccrine poroma. Electron microscope studies revealed intranuclear virus particles. Immunoperoxidase studies using rabbit anti-bovine papilloma virus serum showed a positive reaction.

Pruritic urticarial papules and plaques of pregnancy. A review of 21 cases.

Twenty-one patients with pruritic urticarial papules and plaques of pregnancy (PUPPP) were reviewed. Direct immunofluorescence (DIF) examination was negative in all patients. DIF in patients with PUPPP is a useful and reliable laboratory aid in distinguishing this peculiar dermatosis from herpes gestationis.

Generalized hair follicle hamartoma: the third case report in association with myasthenia gravis.

A 29-year-old woman, referred because of the development of diffuse papules and plaques on the face and progressive hair loss, was found to have generalized hair follicle hamartoma, a very rare condition previously described in only two female patients. All three patients also suffered from myasthenia gravis. The histological appearance of both the involved and uninvolved skin was similar to trichoepithelioma.
