Primary intracranial arachnoid cyst in the elderly.

Sixteen cases of symptomatic arachnoid cyst in elderly patients (> 65 years of age) including of our own case are considered and their clinical and therapeutic features compared with those of the larger series comprising younger patients. From this study, no clinical differences emerged between arachnoid cysts in elderly patients and those of their younger counterparts. Surgical treatment was confirmed as being necessary for a good outcome in this age group when the cysts became symptomatic.

Metastasis of breast carcinoma to intracranial meningioma. Case report.

A case of metastasis from breast carcinoma found within a cerebral meningioma is reported. Histological diagnosis of the cerebral tumor prompted radiological investigation that visualized the asymptomatic malignant systemic tumor and excluded other metastases elsewhere. The pathogenetic aspects of this exceptional occurrence are discussed in the light of the relevant literature.

Solitary cerebral metastasis from ovarian carcinoma: report of 4 cases.

Four cases of solitary cerebral metastasis from ovarian carcinoma are reported. Although systemic metastases from this type of tumor are frequent, they rarely affect the nervous system. From the 4 cases presented and a review of the literature, it appears that a better outcome may be obtained by a combined treatment of the cerebral metastasis, comprising surgery, radiotherapy and chemotherapy.

Solitary plasmacytoma of the skull: report of two cases.

We report two cases of solitary plasmacytoma of the skull, a very unusual presentation and location of this tumor. There seems to be no difference in prognosis between the tumor originating from the skull (osseous form) and from the dura mater (non-osseous form), differently from the data of other parts of the body. The risk of secondary multiple myeloma appears to be low but the mean follow-up is too short for any conclusion on this point.

Solitary cerebral metastasis as first symptom of lung cancer.

The authors report 35 cases of solitary brain metastasis from lung cancer manifesting clinically before the primary tumor. Surgical removal was total in 27 cases and partial in 8. All patients received postoperative radiotherapy. The primary tumor was diagnosed during the hospital stay in 12 cases, within two months of discharge in 18 cases, at necropsy in 5 cases. Median survival of patients was 16 months and the main cause of death was progression of the primary cancer (72% of cases). The clinical and pathological characteristics of solitary brain metastasis from lung cancer are reported in the light of the present 35 cases and those reported in the literature.

[Intracranial actinomycosis].

Actinomycosis is a bacterial infection mainly affecting the cervico-oro-facial area, abdomen and lungs, sometimes spreading to the central nervous system. Extremely rare is the primary cerebral location of the disease.

Medulloblastoma in late adults: report of two cases and critical review of the literature.

The authors describe two cases of medulloblastoma occurring in two men aged 71 and 67 years respectively. Medulloblastoma in late adults (older than 65 years) is an exceptional occurrence, and in this age group is generally of the classic type. Our first case was exceptional for the long survival of the patient compared with those so far reported for the same age group.

Prolonged stabilization of multiple and single brain metastases from breast cancer with tamoxifen. Report of three cases.

Cancer frequently metastasizes to the brain, and such lesions, whether multiple or solitary, have a poor prognosis, despite all efforts to treat them. There have been recent sporadic reports of brain metastases from breast cancer responding for some years to antiestrogens (particularly tamoxifen) or bromocriptine. We report three cases of brain metastasis from cancer--two multiple and one a solitary lesion. The long survival of the patients--two for 5 years and one for 6 years, with more than an acceptable quality of life - should prompt therapeutic trials to test tamoxifen and designed to assess its effects on a sizable number of patients.

Post-Paget osteosarcomas of the skull. Remarks on five cases.

We report our experience with 5 cases of post-Paget osteosarcoma of the skull, a rare lesion of the neurocranium. Four patients were treated by surgery and radiotherapy and one by surgery alone. Two patients received chemotherapy. Histologically, the tumor was found to be an osteosarcoma, fibroblastic in 2 cases, mixed in 2, and osteoblastic in 1. Combined treatment (surgery, radiotherapy and chemotherapy) positively influenced survival (median survival, 6 months). The prognosis for post-Paget osteosarcomas of the skull seems to be worse than for primary sarcomas, probably due to their intense vascularization. This facilitates the spread of tumor cells to other organs (as observed in our cases) and reduces the reduced effectiveness of chemotherapy. Although the latter lengthens survival and reduces the incidence of metastases, it is not as efficacious as in primary sarcomas.

Osteosarcomas of the skull. Clinical remarks on 19 cases.

BACKGROUND: Osteosarcomas are typically long bone tumors and rarely affect the skull, with most articles reporting single cases. As elsewhere in the body, these lesions may be classified as primary or secondary, chiefly post-Paget and post-radiation therapy. METHODS: The authors reviewed the osteosarcomas of the skull diagnosed at the Division of Neurosurgery of "La Sapienza" University of Rome. The patients were placed into two groups, according to the treatment received (prechemotherapeutic era and chemotherapeutic era). In the last five patients, a diagnostic-therapeutic protocol was adopted. RESULTS: The most effective investigations are plain radiographs, computed tomography (CT) with bone windows, and magnetic resonance imaging (MRI), with the latter two allowing assessment of the extraosseous extent of the disease. Chemotherapy has changed the prognosis dramatically, achieving cure in some cases (especially in de novo osteosarcomas). Before the age of chemotherapy, the median survival length was 16 months, but since its introduction, five of nine patients in this study are alive 2 years after diagnosis. The onset of metastases, chiefly to the lung, does not necessarily imply a poor prognosis. The authors propose a schedule for the diagnosis and treatment of new cases of osteosarcoma of the skull. CONCLUSIONS: Plain radiographs, CT targeted on the bone, and MRI are the most important diagnostic tools. Aggressive chemotherapy together with surgery (eventually including local radiation therapy in nontotal macroscopic surgical removal of the lesions) can drastically modify the prognosis of de novo and post-radiation therapy osteosarcomas.

Osteosarcoma of the skull in a child: case report and review of the literature.

Osteosarcoma does not often affect the bones of the skull, occurring preferentially in the appendicular skeleton. The patient's age at onset seems to be later when the tumor is in the skull than in other sites. CT and MRI are at present the best means of establishing the extent of the tumor. Surgical removal of the lesion combined with polychemotherapy is the basis of treatment. We report a case of osteosarcoma of the skull in a child and review relevant publications.

Solitary brain metastases from carcinoma of the bladder.

We report 6 cases of solitary brain metastasis from bladder carcinoma. Although systemic metastases from this cancer are frequent, central nervous system metastases are rare. Analysis of our material and of published cases shows that the tumor is resistant to therapy and the prognosis is consequently very poor.

Meningiomas in elderly patients. Clinico-therapeutic considerations.

This study examines the immediate and long-term results of surgical treatment for intracranial meningioma in patients aged 65 and over. Average patient age was 69 years. The most frequent localizations were the convexity (29.8%) and the sphenoid wing (20.1%). Preoperative risk factors included hypertension (16.1%), cardiopathies (16.1%), diabetes (12.9%) pneumopathies (12.9%) and peripheral vascular diseases (9.6%). All patients were assessed according to the Karnofsky Performance Status (KPS). Operative mortality was 18.5% (23 cases). At long term follow up (minimum 4 months, maximum 12 years, average 5 years) 31.5% of patients were cured, 32.3% had improved and 4% had worsened. The risk factors that mainly influenced results included poor preoperative clinical condition as expressed by low KPS, while the most frequent medical postoperative complications that increased the rate of operative mortality were brain edema, infections and lung embolism.

Late solitary cerebral metastasis from renal carcinoma.

The authors describe 2 patients with solitary cerebral metastasis occurring 13 years after treatment for a renal tumor in one and 17 in the other. Both patients are alive 30 and 36 months after surgery respectively. The pathogenetic and therapeutic features typical of this type of solitary metastasis with late-onset are discussed.

Extradural haematoma of posterior cranial fossa.

Two cases of extradural haematoma of posterior fossa with an acute course are reported. After trauma, the two patients did not show any symptom for a time of 16 and 18 hours respectively. Both of them showed a status of coma at the moment of hospitalization. The haematoma was diagnosed by CT scan. Both of them were immediately operated in order to evacuate the haematoma. Afterwards, one of the patients presented cerebellar symptoms while the other did not show any neurologic deficiency. The authors have analysed cases of this rare form of post-traumatic pathology referred in the literature since 1941, examining them from a clinical, therapeutic and prognostic point of view. The best diagnostic and therapeutic procedures to follow on this subject are then stressed.
