Clinical forms of severe tardive dyskinesia.

The authors describe 19 patients with severe tardive dyskinesia, 11 of whom had a diagnosis of affective or schizoaffective disorder rather than schizophrenia. Most patients had been receiving long-term neuroleptic treatment with few interruptions and had received only one or two different neuroleptics. Frequent eye blinking was the most prevalent prodromal sign of tardive dyskinesia (in seven patients). Four subtypes of tardive dyskinesia could be distinguished: choreoathetosis, tardive dystonia, blepharospasm, and tardive akathisia. Optimal pharmacotherapy most often consisted of combinations of neuroleptics, lithium carbonate, benzodiazepines, and antiparkinsonian drugs. However, after an average of 62 months, only five patients had markedly improved.

Comparison of severe and mild tardive dyskinesia: implications for etiology.

Nineteen patients with severe tardive dyskinesia were compared with 45 patients with mild tardive dyskinesia on demographic and treatment variables. Age, sex, duration and number of psychiatric hospitalizations, and amount and duration of neuroleptic treatment did not discriminate between mild and severe tardive dyskinesia. Severe tardive dyskinesia patients were treated with significantly (p less than .01) fewer neuroleptics and had significantly (p less than .05) shorter neuroleptic-free periods. Onset was more rapid in severe tardive dyskinesia patients than in mild tardive dyskinesia patients. Severe tardive dyskinesia is more likely t result from greater patient vulnerability than from heavier neuroleptic exposure. Unipolar depressed patients who are older and on higher neuroleptic dosages may be more susceptible to severe tardive dyskinesia.

Measurements of n-p correlations in the reaction of relativistic neon with uranium.

We report a preliminary measurement of coincident neutron-proton pairs emitted at 45 degrees in the interaction of 400, 530, and 650 MeV/A neon beams incident on uranium. Charged particles were identified by time of flight and momentum, as determined in a magnetic spectrometer. Neutral particles were detected using a thick plastic scintillator, and their time of flight was measured between an entrance scintillator, triggered by a charged particle, and the neutron detector. The scatter plots and contour plots of neutron momentum vs. proton momentum appear to show a slight correlation ridge above an uncorrelated background. The projections of this plane on the n-p momentum difference axis are essentially flat, showing a one standard deviation enhancement for each of the three beams energies. At each beam energy, the calculated momentum correlation function for the neutron-proton pairs is enhanced near zero neutron-proton momentum difference by approximately one standard deviation over the expected value for no correlation. This enhancement is expected to occur as a consequence of the attractive final state interaction between the neutron and proton (i.e., virtual or "singlet" deuterons). The implications of these measurements are discussed.

Tricyclic use in the cognitively impaired elderly.

The complex interrelationships among tricyclic antidepressant (TCA) effects, depression, and dementia in elderly patients are reviewed. Although results are mixed, some studies have shown TCA efficacy in elderly patients with organic disorders and associated depressive symptoms, such as forced weeping in pseudobulbar states. Available data, although not clear-cut, indicate that memory deficits or overt confusional states directly attributable to TCAs are relatively rare in patients without preexisting organicity. Studies in normal nonelderly subjects suggest an interaction wherein cognitive improvement due to relief of depression competes at higher doses with drug-induced memory impairment. In older depressed patients, significant relationships have not been found between degree of response to TCA and either baseline cognitive impairment or cognitive impairment during drug trials. Results from an ongoing study comparing imipramine and trimipramine in older depressives suggest that clinical improvement outweighs the anticholinergic effect, resulting in improved memory function.

[Prevention of gastroduodenal complications in coronary surgery. Usefulness of preoperative fiberoscopy].

Fiberoscopic examination of the oesophagus, stomach and duodenum was carried out in 60 coronary disease patients prior to aorto-coronary bypass. Lesions of the upper digestive tract were detected in one-third of the patients; half of them were asymptomatic. The management of these cases depended upon the urgency of the cardiovascular operation. In 17 cases, a 1-month treatment of the digestive lesions with antacids and cimetidine was instituted immediately after fiberoscopy, and revascularization was performed 8 days later. In 3 cases, the cardiovascular operation was postponed; after 1 month of treatment only one patient had active lesions on control fiberoscopy and had to undergo digestive surgery prior to aorto-coronary bypass. No gastroduodenal complications of cardiovascular surgery was observed in this series.

The future of health sciences.

The past advances in the health sciences will be overshadowed by the achievements to be expected by the year 200. A list shows the dates when certain targets will be reached according to the educated guesses of scientists in the respective fields. The need for a new ethics to handle the changes that will follow upon the discoveries is stressed.

[Use of dopamine in sequelae of extracorporeal circulation. Apropos of 50 patients].

Dopamine was used after open heart surgery in a group of 50 patients. The therapeutic indications were: diffuse coronary insufficiency in 27 cases, myocardial infarction in 12 cases and early septicaemic shock in 6 cases. In the other 5 cases, dopamine was prescribed for short lasting circulatory insufficiency. The dosage varied between 3-15 gammas/kg/min. The variations of different parameters were studied over 12 hours. Improvement of cardiac efficiency was an early feature (less than 3 h) and was observed for all dosages (above 3 gammas/kg/min). It was accompanied by a marked diuresis. The chromotropic effects were non-existent at these dosages. At medium-term dopamine was effective in 40 patients (80 p. 100). Dopamine is effective in over 3 out of 4 cases after cardiopulmonary bypass. Its effect is immediate, and is observed in dosages less than 10 gammas/kg/min.

[Application of echocardiography to the diagnosis of tricuspid valve diseases. Apropos of 103 cases].

In a series of 103 consecutive surgical patients, the normal and pathological echocardiographic appearances of the tricuspid valve were analysed and compared with the anatomical findings. It was possible to define the nature of the lesions by echocardiography, which was superior to angiography in this field. The right ventricular index (right ventricular dimension body surface area) correlated well with the mean pulmonary artery pressure (r = 0.75; p less than 0.001). The index may be useful in the long term follow-up of post-operative patients.
