Femoral shortening in correction of congenital knee flexion deformity with popliteal webbing.

Severe knee flexion deformity with popliteal webbing or pterygium is considered to be uncorrectable. The soft tissues and, in particular, the main nerves and vessels are short relative to the bone. Femoral shortening was used in correction of such a deformity in a child with arthrogryposis. The operative procedure is described. Femoral shortening should be considered as an aid to correction of any severe knee flexion deformity.

The spectrum of beta-thalassaemia mutations in the Lebanon.

We screened 110 DNA samples from carriers of beta-thalassaemia, using the ARMS-PCR technique with primers for common Mediterranean mutations. Unidentified samples were subjected to a heteroduplex analysis with Universal Heteroduplex Generators covering the beta-globin gene, followed by DNA sequencing. In total, 16 different mutations were detected, the most frequent being IVSI-110 (40%), followed by other common Mediterranean mutations (IVSI-1, IVSII-1, IVSI-6). Other mutations detected were of Lebanese, Turkish, Iranian, Kurdish, Bulgarian and Asian Indian origin. The most heterogeneous religious group seems to be the Sunni Muslims, with 13 mutations, while only 2 mutations were detected among the Christian Maronites. Results from this study are compared with those from other Mediterranean and neighbouring countries.

Dominant functions of right versus the left hemisphere.

The aim of this work was to study the function of the left and right cerebral hemispheres concerning linguistic, musical and cognitive abilities, through the evaluation of the deterioration of these domains following lesions of either hemisphere. The study was carried out on 80 subjects (60 males, 20 females) with a mean age of 48 years. Subjects were categorized into four groups, 20 in each. Group I had left brain lesion with dysphasia; group II, left brain lesion without dysphasia; group III, right brain lesion without dysphasia; group IV, control. Patient's evaluation included a dysphasia test, psychometric battery, tests for musical abilities, brain CT scanning and EEG. Group I showed deterioration in the linguistic abilities, while groups II and III had intact abilities. Deterioration in musical abilities was noticed in group III, with normal scores in groups I and II. There was deterioration of the intellectual abilities in all patients' groups, to varying degrees. The significance of the results is discussed.

The use of the Ma-Griffith technique for percutaneous repair of fresh ruptured tendo Achillis.

All patients treated for a rupture of the Achilles tendon by the technique of percutaneous repair were identified from a five year period up to 1987. A review of the patients and results of this technique was undertaken using a clinical grading system. Review was conducted between 18 and 81 months following the injury. Of 38 patients followed up, 71% achieved good results and only 8% poor results. The re-rupture rate for this series was 8%. Sural nerve involvement (13%) was found to be a troublesome complication. During the period under study, a modification to the technique was introduced which abolished this complication. We present our results and describe the modification to the technique. The importance of the use of nonabsorbable sutures is emphasized.
