The experimental and clinical studies were carried out to alleviate the bone marrow suppression by antineoplastic drugs. Faster recovery of granulopoiesis was observed by pretreating recipients with RBC hypertransfusion and/or OK-432 (Picibanil, biological products of beta-streptococci.) In experimental mice, higher granulocyte counts could be maintained with hypertransfusion in the peripheral blood, and the recovery of granulopoietic series and CFU-S of bone marrow cells were found to be more rapid after cyclophosphamide administration. OK-432 also resulted in higher peripheral granulocyte, whereas the total nucleated cell counts and CFU-S were decreased in the bone marrow, suggesting sparing bone marrow granulocyte reserve and its migration to the peripheral blood. The mechanism of higher granulocyte count after hypertransfusion was not clearly explained but it was considered that erythroid suppression caused colateral flow of multipotential stem cells to granulopoiesis. The effect of both combinations was unexpectedly less significant in the recovery of granulopoiesis, but it was thought that the optimal time interval should be sought between pretreatment and the administration of anti-neoplastic agents. The clinical use of hyper transfusion and OK-432 also proved the alleviation of granulocytopenia, and rapid granulocyte recovery at the time of consolidation therapy among children with AML.
Electrical stimulation of the posterior limb of the internal capsule was performed in six patients with central pain due to vascular lesions of the thalamus. Three patients suffered from thalamic hemorrhage and the other three from ischemic insult. These patients developed dysesthesia, hyperpathia and spontaneous burning pain several months after cerebral vascular diseases. These severe pain were not relieved by any trials of anticonvulsants, psychopharmacological and analgesic drugs. Stimulating electrode system manufactured by Medtronic, Co. was implanted stereo-taxically in the posterior limb of the internal capsule which was 25 mm lateral to the posterior commissure. Ramped bipolar stimulation with a square pulse 2-3 V, 0.2 msec, 50 Hz elicited sensory response such as warm or comfortable sensation. Satisfactory relief of pain was obtained in most cases by 30-minutes-long stimulation twice or three times a day for various period of time ranging from 10 to 14 days. Long-term follow-up ranging from three months to two years showed that three patients had a good result (incomplete pain relief by stimulator with no medication required), two had a fair (incomplete pain relief by stimulator with decreased doses of medication) and another one had a poor result in which the pain had not been relieved with the stimulation in one month. No remarkable side effects were seen. These results indicate that the internal capsule stimulation is more effective and lasts longer than the various ablative methods to relieve central pain.(ABSTRACT TRUNCATED AT 250 WORDS)

Eight fatalities due to cardiac rupture, of 40 consecutively autopsied myocardial infarctions, were clinicopathologically examined. Total duration of infarctions in 8 cases ranged from 9 hours to 14 days. The longer the period, the more the infarction extended, and aneurysmal dilatation of infarcted wall and tissue weakness were striking. The weakness was caused by the lack of replacement of necrotic muscle fibers by granulation, and was related to aneurysmal structure per se and also to its paradoxical pulsations. Postinfarction cardiac rupture occurring in relatively later phase was considered to be mainly due to weakness of infarcted wall, with less influence factors of mechanical stress such as physical exertion, hypertension, or intracardiac pressure. In the case where cardiac rupture took place in a relatively early stage of the episodes, infarction covered only a small range and aneurysmal change of the ventricle was slight, for which dynamic factors, represented by the degree of a localized strain elicited by myocardial contraction of non-infarcted site, seemed to play a significant role.
A case of mesenchymal hamartoma of the liver in an 18-month-old boy is reported. CT and US revealed a large, cystic tumour surrounded by a thick wall. In the lower portion of the tumour, a small solid component was recognized on CT. Angiography revealed marked arterial displacement, fine tumour vessels and a large radiolucent area. CT, US and angiography were helpful to make a diagnosis of mesenchymal hamartoma of the liver.
In order to clarify the structural background for compartmental syndrome in the lower limb, biomechanical study was carried out on cadavers and pathological investigation was done on amputated limbs in clinical cases. Experimental ligation of arteries was made on rats and histochemical analysis of the ischemic muscles was also performed. Furthermore, the pressure inside the leg compartments of normal controls was measured. Based on these observations, the following conclusions were drawn. Some structural specialties of compartments are responsible for the clinical fact that anterior compartment syndrome is most frequently seen in the lower limb. Circular fibers existing at the wall of the anterior compartment is probably related to frequent occurrence of a high pressure in the compartment. In the muscle with ischemic changes, regenerating muscle fibers are found just under the fascia. This fact suggests that the muscle fibers have possibilities of regaining their function. Ischemic change of nerves is more severe in the peroneal than in the tibial nerves. This fact may be due to the anatomical specialties of compartments rather than circulatory characteristics of nerves themselves.

In 5 patients, angiographically occult vascular malformations led to intracerebral or intracerebellar hemorrhages. Angiographic findings were not suggestive of vascular abnormalities, but signs of a mass were present in 4 patients. Computed tomography (CT) scanning suggested the presence of cryptic vascular lesions accompanied by intraparenchymal hemorrhage in 2 patients. In all patients surgical treatment was successful. Histological examination confirmed these lesions to be venous malformations in the cerebellum in 2 patients and in the left frontal region in 1, and arteriovenous malformations in the cerebellum and right putaminal region in 1 patient each.
Banding studies were done on tissues from tumors excised from 22 Japanese patients with non-Hodgkin's lymphomas. All tumors were found to be associated with aneuploidy. The chromosome abnormalities were diverse, with each chromosome type being involved in these abnormalities. Terminal deletions, derivative chromosomes as a result of unbalanced or nonreciprocal translocations, and markers of totally or partially unknown origin accounted for the majority of the structural abnormalities. Balanced reciprocal translocations were seen only occasionally. The 14q+, 6q-, partial trisomy of 1q, 11q+, 18q+, and 19q+ abnormalities were seen in more than two patients. The incidence of a missing sex chromosome was significantly higher than that of autosomes, but no particular other karyotypic abnormality seemed to be associated with the event. All six patients whose chromosomes could be totally characterized were in complete remission. Patients with one or more markers of unknown either totally or partially origin, had a median survival of only 8 months (p less than 0.01). Five of the former six patients showed a nodular histology. Fifteen of the latter 16 patients had a diffuse histology, with 13 of the 15 having diffuse histiocytic lymphoma. The median survival of 9.5 months for the 14 with only abnormal metaphases in the lymphatic tissues (AA-group) was shorter than the 26+ months for the seven patients with both normal and abnormal metaphases (AN-group). Thus certain aspects of chromosomal changes appear to correlate with histology and/or prognosis in non-Hodgkin's lymphomas.
A case of Japanese Burkitt's lymphoma (BL) with a t(2; 8) (p11; q24), a variant translocation in BL, is reported. The patient was a 45-year-old woman in whom a subcutaneous right axillary tumor first occurred. Remission was not achieved despite extensive chemotherapy. Of the four nonendemic BL, two endemic BL, and one nonendemic Burkitt-type acute lymphocytic leukemia (ALL-L3) cases with a t(2; 8) reported so far, including the present case, four (two nonendemic and two endemic) were positive for Epstein-Barr virus-determined nuclear antigen (EBNA) and two revealed extremely high antibody titers against Epstein-Barr virus (EBV), the remaining one not having been tested for EBV. Thus, a possible close relationship between the t(2; 8) and EBV infection has to be considered. The t(2; 8) in nonendemic BL seems to occur more often in adults than in children.
Cytogenetic, morphologic, and clinical data of 33 acute nonlymphocytic leukemia (ANLL) patients with t(8q - ;21q+) and 19 patients with acute promyelocytic leukemia (APL) were collected from seven laboratories in Japan. The latter class included 18 patients with t(15q + ;17q-) and one with a normal karyotype. The t(8q - ;21q+) and t(15q + ;17q-) translocations were each shown to be associated with a specific type of ANLL, namely, AML-M2 and APL-M3, respectively. No patient with APL had the M3 variant. The t(8q - ;21q+) translocation seems to be more common as an abnormality in ANLL in Japan as compared to findings in other countries. The high incidence of t(15q + ;17q-) among Japanese patients with APL indicated in this study, however, still awaits confirmation.
Motilin, [Met]enkephalin, [Leu]enkephalin, somatostatin, taurine, gamma-aminobutyric acid (GABA), and glycine were tested for their effects on Deiters neurons of the lateral vestibular nucleus in rabbits. Iontophoresis was carried out with multibarrelled micropipettes. All four peptides and three amino acids produced depression of neuron firing. No facilitatory responses were observed. The depressant action of each peptide when iontophoresed alone was dose-dependent and was rapid in onset and recovery. Their characteristic actions suggest the possibility of their independent roles as strong inhibitors, although the experimental paradigm does not allow conclusions about the individual potency of each peptide. When GABA was administered together with motilin, [Met]enkephalin, or somatostatin, the effects of the peptide and GABA were additive, producing depression greater than that with application of either substance alone. When GABA was applied in conjunction with [Leu]enkephalin, more complex interactions were observed. At low iontophoretic currents, [Leu]enkephalin antagonized the action of GABA, producing a depression less than that of GABA alone and of considerably slower onset, suggesting an additional modulatory effect. These observations support the conclusion that all substances tested are chemical mediators in the lateral vestibular nucleus and [Leu]enkephalin may be a neuromodulator as well. Because recent immunocytochemical studies indicate that Purkinje cells in the cerebellar cortex are chemically heterogeneous and exhibit immunoreactivity for motilin, taurine, the enkephalins, and somatostatin, as well as for the GABA-synthesizing enzyme glutamic acid decarboxylase, it is suggested that the Purkinje cell projections to vestibular and cerebellar nuclei are multimodal in their chemical coding. The uniformly depressant action of the peptides and amino acids reported here is consistent with earlier observations that Purkinje cells exert an inhibitory influence on the vestibular and central cerebellar nuclei.
We have carried out basic investigations of an intravenous minimum dose bolus (MinDB) injection method for cerebral computed angiotomography by comparing blood iodine concentrations with high resolution computed tomography (CT) images of cerebral blood vessels. With the GECT/T 8800 scanner, a MinDB injection of 1 ml/kg of 60% Conray at a rate of 2 ml/s made it possible to increase the blood iodine concentration in the carotid artery to more than 15 mg/ml for about 20 s. Computed tomography performed during this period enabled us to obtain clear images of the circle of Willis, including other main cerebral arteries, deep veins, and cortical and lenticulostriate arteries. Cerebral computed angiotomography using this method should be useful for noninvasive screening or detection of cerebrovascular lesions themselves, such as cerebral aneurysms, cerebrovascular obstructions, arteriovenous malformations, and moyamoya disease, and should also be useful for determining the anatomical relationship between the cerebral blood vessels and other parenchymal or space occupying lesions.
A chromosomal classification of 93 children with acute lymphoblastic leukaemia (ALL) is presented. The chromosomal categories were normal, hyperdiploid, pseudodiploid and hypodiploid: the chromosomally abnormal cases were classified according to the presence of an abnormal clone. The longest follow-up was 9 years. Infants and older children were over-represented in the pseudodiploid category: this association was statistically significant. Patients in the hyperdiploid and hypodiploid categories had the longest first remissions and overall survival and those in the pseudodiploid category the shortest. These effects were statistically significant even when the effect of age and leucocyte count were taken into account. Thus chromosomal findings at diagnosis in ALL can be used as an independent prognostic factor.
