Transcatheter chemo-embolization is a technique for achieving a marked antitumor effect by embolizing the hepatic artery with a gelatin sponge immediately after infusion of adriamycin 60 mg into the same artery. Chemo-embolization was performed in 212 cases of hepatocellular carcinoma and 23 cases of metastatic liver cancer, a total of 235 patients. Of this population, 19 patients with hepatocellular carcinoma underwent hepatectomy, and in 7 of them, complete necrosis of tumors having thick capsules and less than 4 cm in diameter was confirmed. The cumulative survival rate of patients with unresected hepatocellular carcinoma was 75.6% at 6 months, 52.6% at 1 year, and 20.3% at 2 years. The longest survival time was 3 years and a half, and this patient is now living. As regards metastatic liver cancer, complete necrosis could be achieved in 1 of 2 patients undergoing resection, whereas the cumulative survival rate of unresected cases was 55% at 6 months and 44% at 1 year: only two patients survived for more than 1 year. Technically, the use of a balloon catheter (7F) resulted in the virtual elimination of unsuccessful cases and the prevention of adverse effects due to migration of the gelatin sponge.
A case of diffuse tracheobronchial amyloidosis in a 70-year-old woman thought clinically at first to have lung carcinoma is reported. Bronchoscopic biopsy was necessary to make a diagnosis. Amyloid was deposited markedly in the subepithelial layer. After Congo-red staining amyloid showed the usual apple-green birefringence under polarizing microscope. Electron microscopic study disclosed typical amyloid fibrillar structure. Amyloid protein of this patient was confirmed to be AL protein by immunoperoxidase study which showed specific staining by the anti-A lambda antiserum. When potassium permanganate reaction was used amyloid protein of this patient resisted to the reaction. We reviewed 19 cases from the previously reported cases of primary amyloidosis of the lower respiratory tract, and gave some description of pathologic and clinical aspects of this disease.

A case with large primary mediastinal seminoma responded very well to chemotherapy. A 13-year-old boy was evaluated for complaints of edematous face and neck. Chest radiography and computerized tomography revealed a large anterior superior mediastinal mass. Needle biopsy demonstrated seminoma. After two-drug combination chemotherapy with vincristine and prednisolone and three-drug combination with vincristine, prednisolone and cyclophosphamide, there was a considerable regression of the mediastinal mass. The lower neck and mediastinum were irradiated at a dose of 3,750 rad after cessation of chemotherapy. The patient has been asymptomatic without evidence of recurrence for a follow-up period of more than four years.
Forty-one dust-and-ozone-exposed and 37 nonexposed workers, belonging to the Research and Development Division of a photo-copier manufacturing industry, were examined to assess the effect of the exposure to carbon, iron and resin dust and ozone in the air of the work environment by means of questionnaires on their physical condition, smoking habits and exposure history by interview, chest X-rays, testing of ventilatory functions, transcutaneous PO2 (tcPO2) test and H2O2-induced hemolysis test. The following results were obtained. Respirable dust concentrations in the air of the work place were 0.1-1.0 mg/m3, total dust concentrations 0.2-2.0 mg/m3, and ozone concentrations 0.004-0.06 ppm (0.008-0.12 mg/m3). According to the Japanese Classification of Radiographs of Pneumoconioses, the exposed workers showed a higher rate of profusion 0/1 and over, and category 1 and over (1/0 and over) than the nonexposed workers. Ventilatory function testing revealed no difference between exposed workers and nonexposed workers, but small airway narrowing was suspected in smoking workers in comparison with nonsmoking workers. Transcutaneous PO2 showed no difference between exposed and nonexposed workers, between smoking and nonsmoking workers, and between any of the paired six combinations out of the four groups of workers, i.e., nonsmoking and nonexposed, nonsmoking and exposed, smoking and nonexposed, and smoking and exposed. It was estimated by H2O2-induced hemolysis test that smoking and/or dust exposure, especially long-term exposure, gave rise to aggravation of fragility of the erythrocyte membrane by lipid peroxidation with ozone or active oxygen produced by the reaction of dust and alveolar macrophages. The questionnaire was considered indispensable to ascertain the workers' exposure history. No worker in this work environment showed clinical signs indicating a need for medical care or home response. There results may indicate that in resolving the pathogenesis of pneumoconiosis further studies will be required on the prevalence rate of profusion 0/1 and over (or category 1 and over) under low concentrations of dust exposure and on the intrapulmonary reactions induced by low doses of dust exposure.
Immuno-precipitation Inhibition Technique (IPIT) method of determining disopyramide levels in human plasma was reevaluated in respect to factors affecting assay procedure, specificity and recovery. With this method only 20 microliters of plasma is required to obtain a rapid, sensitive and specific determination. Due to little intra-assay and inter-assay variation, the IPIT method is suitable for clinical determinations for disopyramide. Furthermore pharmacokinetic parameters calculated from the disopyramide concentrations coincided with the reported data determined by other methods. It was demonstrated, however, that precise temperature control and sample mixture are essential for accurate plasma disopyramide determination using this method.

We examined the capabilities of scintigraphic phase imaging in detecting the site of the onset of ventricular activation and the pattern of ventricular activation sequence. ECG-gated equilibrium cardiac blood pool scintigraphy was obtained in the left anterior oblique (LAO) and right anterior oblique (RAO) projections. The standard 12-lead electrocardiograms (ECG) were recorded in 29 cases with various conditions. These included seven normal subjects as controls, eight patients (pts) with complete right bundle branch block (CRBBB), one with CRBBB and left axis deviation (LAD), two with complete left bundle branch block (CLBBB), one with the Wolff-Parkinson-White (WPW) syndrome (B-type), six with right ventricular apical endocardial pacemakers, one with a right ventricular anterior wall myocardial pacemaker, and three with left ventricular apical myocardial pacemakers. Phase image analysis was performed using the first harmonic of the Fourier transform to fit a cosine curve to the time-activity curve of each pixel in the cardiac blood pool study. The results were as follows: In pts with WPW syndrome (B-type) and artificial pacemakers, the site of the earliest phase angle corresponded to the site of the onset of ventricular activation as predicted by ECG and chest radiographs, respectively. However, in normal subjects and in pts with CRBBB, the site of the earliest phase angle was observed at the basal (upper) interventricular septum, which was different from the site of the onset of ventricular activation previously reported by Sodi-Pallares et al and Durrer et al. This discrepancy may have been caused by the paradoxical motion of the basal (upper) interventricular septum in those cases. A similar discrepancy was also observed in pts with CLBBB. Although the site of onset of ventricular activation was predicted to be near the insertion of the anterior papillary muscle of the right ventricle by ECG and electrophysiology, the pixels showing early phase angle were distributed widely along the interventricular septum in those pts with CLBBB. This error in the phase image may have been caused by the extensive paradoxical motion of the interventricular septum. Thus, caution should be exercised in estimating the site of the onset of ventricular activation by phase images. The main direction of phase changes corresponded well to the ventricular activation sequence estimated using ECG in all subjects.(ABSTRACT TRUNCATED AT 400 WORDS)

Leukemia cells from patients with acute myeloid leukemia in relapse were treated with various inducers of differentiation of human myeloid leukemia cell lines. Leukemia cells in primary culture from most, but not all, patients underwent morphological, cytochemical and biochemical changes after treatment with inducers of differentiation such as 12-O-tetradecanoylphorbol-13-acetate (TPA), retinoic acid, actinomycin D, aclarubicin, and alkyl lysophospholipid. The most effective inducer varied from specimen to specimen. Leukemia cells from patients in relapse were compared with those from untreated patients. The responsiveness to TPA of leukemia cells from patients in relapse was similar to that of leukemia cells from untreated patients. However, retinoic acid or actinomycin D resistance was more frequently observed in leukemia cells from patients in relapse than in those from patients before initial therapy. This is the first report to indicate that leukemic cells from relapsed patients who are resistant to cytotoxic chemotherapeutic drugs can be induced to differentiate into mature cells by appropriate inducers. However, the responsiveness to inducers of leukemia cells from patients in relapse is not the same as that of leukemia cells before therapy.

Stacking equilibrium quotients were evaluated for three purine deoxyribo-dimers, d-ApA, d-m6Apm6A and d-GpG, using procedures developed in earlier studies, and compared with those for the corresponding ribo-dimers. It was shown that the deoxyribo-dimers were apparently more strongly stacked than the corresponding ribo-dimers at 25 degrees C. To explain the observed difference in stability (as measured by the apparent stacking quotient) in the two series of dimers a new extended two-state model is advanced, that the differences in the apparent stacking quotients are due to a limited availability of the number of the sugar-ring puckering modes in the stacked ribo-dimers as compared with the case in the stacked deoxyribo-counterparts. The intrinsic stacking quotients, which are interrelated with the apparent stacking quotients by the microscopic sugar puckering equilibrium constants, were also evaluated for the purine deoxy- and ribo-dimers, and were found to be the same in any pair of these dimers.

A case is reported of a traumatic middle meningeal arteriovenous fistula, which was formed on the contralateral side to the head injury without a skull fracture and was no longer demonstrable on the repeated angiogram after 35 days without any surgical intervention. Possible mechanisms of fistula formation and its spontaneous closure are discussed.
Efficacy of acute intravenous verapamil, 10 mg, and chronic oral verapamil, 320 mg, daily were studied electrophysiologically in 15 patients with paroxysmal supraventricular tachycardia (PSVT). Plasma verapamil concentrations were measured concurrently. Both intravenous and oral verapamil significantly increased the AV node conduction time, the cycle length producing a Wenckebach period, and the refractory period of the AV node. These changes were reflected in changes in plasma verapamil concentration. The echo zone and the supraventricular tachycardia (SVT) zone markedly narrowed after administration of both intravenous and chronic oral verapamil. Verapamil's efficacy was found to be related to the type of SVT. For instance, verapamil was more effective in SVT due to AV nodal re-entry than in SVT due to concealed accessory pathway. Fourteen patients were followed from 3 to 31 months and all except one were well controlled. In conclusion, verapamil was effective in prophylaxis of paroxysmal SVT.
