[The clinical effects of a new antiviral 9-(2-hydroxyethoxymethyl) guanine (aciclovir) against herpes virus infections].

The clinical effects of a new anti-viral 9-(2-hydroxymethoxymethyl) guanine (Aciclovir) against Herpes virus infections have been investigated. The patients had malignant tumours or auto-immune disease complicated by shingles and chicken pox due to Vaicella zoster virus (VZV) (43 cases), Herpes simplex virus (HSV) (10 cases) and 9 cases which were clinically diagnosed as Herpes, though the virus was not confirmed as the causative agent. As a general principle the dosage of Aciclovir was 5 mg/kg, t. i. d. for 5 days by slow intravenous infusion. The clinically effective rate against VZV was 93%, being excellent in 42% and against HSV it was 80%, being excellent in 40% and when the results of the cases of unknown origin were included it was excellent in 40% and the cumulative effective rate was 88%. Concerning the efficacy in reduction of pain, swelling, disappearance of vesicles and new scab formation, the effect was most noticeable after the third day of treatment. Treatment given early in the disease is likely to provide better results. Concerning side effects, one of 62 patients had proteinuria and the other had a drug rash and an abnormal liver function test. It is likely that the combination of treatment and the primary disease had some influence, but the cause/effect relationship to Aciclovir treatment is not clear.

Angiographic and computed tomographic appearance of secondary carcinoid of the liver.

Liver angiograms of 27 patients with hepatic carcinoid metastases were analyzed for distribution, size, degree of vascularity, and patterns of tumor staining. Computed tomography (CT) scans were evaluated before and after bolus contrast administration in 13 of the cases. The diagnostic capability of the two procedures was compared. Angiography detected metastases, some less than 1 cm in size in all 13 patients; in 5 of 13 cases (38.5%) CT did not detect any hepatic metastases. In the eight patients in whom CT demonstrated metastases, the patterns of lesions and usefulness of contrast varied. Angiography is essential for evaluation of patients with suspected carcinoid metastases to the liver.

CT of soft-tissue tumors.

Computed tomography was performed on 52 patients with soft-tissue tumors, 36 malignant and 16 benign. Major advantages with CT include the possibility of differentiating between lipoma and other tumors and the ability to demonstrate the transverse location of a lesion (intra- or extracompartmental). It was not possible to relate attenuation values to histologic type or grade of malignancy. CT was found superior to conventional radiography in demonstrating additional bony destruction of the pelvis and spine. The risk of overestimating the size of high grade malignant tumors because of accompanying edema is discussed. CT should precede angiography in the investigation of soft-tissue tumors, and angiography may primarily be reserved for those lesions where vascular relationships are not adequately demonstrated by CT.

Lymphographic classification of malignant lymphoma.

In an attempt to raise the diagnostic accuracy of lymphography in malignant lymphoma, especially in early stages, the disease were radiographically classified into 3 groups such as atypical, typical and advanced group mainly based on the lymphographic findings. The findings showed characteristic features corresponding to the stages of the disease. Atypical group showed the following findings: small filling defects, increased number of lymph node and visualization of the marginal sinus. The findings were regarded as those of early stages of malignant lymphoma.
