[Basic and clinical evaluations of cefmetazole in postoperative wound infections].

The time course of the concentration of cefmetazole (CMZ) in the serum and in skin and intestinal tissues was determined after a single intravenous injection of 2 g of the drug. CMZ moved into them well. Furthermore, 41 patients with postoperative wound infection (superficial in 29 and deep in 12) were treated with CMZ 2-4 g daily. Bacteriological examination of the lesions with simultaneously carried out. As a result, 101 strains of bacteria were isolated and identified. Mixed infection was found in 27 cases (65.9%). Fifteen strains (14.9%) of E. coli, 15 (14.9%) of B. fragilis, 7 (6.9%) of Klebsiella sp. and 7 (6.9%) of Proteus sp., were the main bacteria isolated. Eight cases (19.5%) had mixed infection of E. coli and B. fragilis. The committee (3 members) evaluated CMZ to be effective in 75.6% (31 of 41 cases) and bacteria disappeared in 60.5% (23 of 38 cases). The side effects observed were pyrosis and feeling of gastric malaise in 1 case. The results suggest that CMZ is useful, which exerts an excellent effect on postoperative wound infections.

Incorporation of [14C]-leucine into complement in experimental infection of rats in malnourished stages.

The complement biosynthesis was examined in the first complement response, that is at the stage of elevated complement level 3 days after Staphylococcus aureus infection, since this elevation might play a role in host defence before antibody response. [14C]-leucine was used as a labeled amino acid to examine the de novo synthesis of the complement. In the first complement response, the malnourished rats red with 0.5% protein, both in infection and noninfection groups, showed a significantly higher specificity in newly synthesized complement activity than did the rats in the well-nourished group fed with 18% protein. The infected rats showed much higher rates of de novo synthesis of complement activity than noninfected rats and this effect was more predominant in the malnourished group.

Isolation and immunochemical localization of collagenase in mouse peritoneal macrophages.

Collagenase was isolated from the culture medium of thioglycollate-stimulated mouse peritoneal exudate macrophages. The macrophage collagenase activity was inhibited by goat anti-mouse bone collagenase antibody, indicating that macrophage collagenase immunologically cross-reacts with mouse bone collagenase. The enzyme was localized in mouse peritoneal macrophages by indirect immunofluorescent antibody technique. Distinct granular fluorescence was observed intracellularly in most thioglycollate-stimulated macrophages, whereas slight or no fluorescence was observed in non-stimulated control macrophages.

Experimental myocarditis induced in mice by infection with influenza A2 virus.

A mouse model was established for the study of acute myocarditis that occurs during influenza infection. Challenge with more than 10 LD50 of mouse-adapted influenza A2 virus (H2N2) induced myocarditis macroscopically discernible as white, irregularly shaped lesions which were shown by histological examination to consist of necrotic myofibers surrounded by infiltrating mononuclear inflammatory cells. After challenge with 10 LD50 of the virus, macroscopic myocarditis was found to advance in a progressive manner up to the 7th day, while the virus titer in the heart reached its peak on the 2nd day and began to decrease on the 5th day of infection. However, development of myocarditis was significantly suppressed in mice which were irradiated with 400 R of X-rays before infection. In addition, myocarditis did not develop in congenitally athymic nude mice. These data indicate that myocarditis was not brought about by viral action directly, but that it was mediated by some function of the host against viral invasion, which was abolished by X-irradiation. The data also suggest that T cells played a key role in the development of myocarditis.

Level of complement activity and components C1, C4, C2, and C3 in complement response to bacterial challenge in malnourished rats.

In experimentally induced malnutrition in rats, there was no significant difference between the measured level of complement activity of the classical pathway (50% hemolytic complement [CH50]) and that of the alternative pathway (ACH50), although the levels of complement components C1, C4, C2, and C3 were depressed significantly. The complement activity showed a temporary elevation with a peak at 2 or 3 days after bacterial challenge with Staphylococcus aureus in rats, and we call this the complement response. After 3 days, CH50 and C3 in the malnourished rats and ACH50, CH50, and C3 in the well-nourished rats showed a significant increase, and C1, C4, and C2 in both groups tended to elevate. On the basis of these observations, the significance of the elevation of C3 in the complement response to bacterial infection showed a strong influence by enhancing the activation of both the classical and the alternative pathways, since C3 is known to be the junction of both complement pathways. In this way, C3 responded to an earlier stage than did the other components and may contribute to maintaining the body defense system against infection.

Biochemical and biophysical behaviors of plasma albumin. II. Comparison of plasma albumin from normal subjects and nephrotic patients.

Biochemical and biophysical analyses were made to study the properties of serum albumin from nephrotic patients of two types, remissive stage and relapsed state, and the results were compared with those in normal subjects. Two different ion exchange columns were prepared to isolate and purify each albumin sample and the materials eluted through the two columns were identified. The nephrotic elution materials, especially in the state of relapse, were of some difference from those of normal subjects. alpha-Helix contents estimated by circular dichroism spectra were found to be closer to normal albumin in remissive state than in the state of relapse. The binding of fluorescent probe, 1-anilino-8-naphthalene sulfonate, to albumin was examined by calculating the binding parameter, the dissociation constant and number of binding sites. The binding parameters were closer to those of normal albumin in remissive stage than the state of relapse.

Metabolic detoxication of bis-(2-hydroxy-3, 5-dichlorophenyl)-sulfoxide in man.

The metabolic detoxication of bis(2-hydroxy-3, 5-dichlorophenyl)sulfoxide (BTS) in man was investigated. Bis(2-hydroxy-3, 5-dichlorophenyl)sulfide (BT) was identified in beta-glucuronidase treated urine following the administration of BTS by thin-layer chromatography, gas chromatography, ultraviolet spectrum and quantitative analysis. No other metabolites were detectable. BT-glucuronide was also identified in urine. It was assumed that BTS was reduced to BT and successively conjugated with glucuronide in man, and excreted as BT-glucuronide in the urine.

The effect of vitamin C deficiency on complement systems and complement components.

The changes in the complement systems and complement hemolytic activities through classical and alternative pathways, and the individual components, C1, C4, C2 and C3 were demonstrated in the course of vitamin C deficiency in guinea pigs. During the course of vitamin C deficiency during the first week after the start of the experiment, all components except C1 slightly decreased, gradually increasing in the following weeks. This indicates that this period is important in the formation of an immune defence system in the host. At the time the symptoms of vitamin C deficiency appeared, C1, C2 and CH50 started to decrease. C3 increased when vitamin C deficiency became severe and showed a completely different pattern from those of the other components. The lowered C1 will be due to a collagen-like region in the characteristic of Clq subcomponent, since insufficient vitamin C state produces impaired collagen formation. The activity of the alternative pathway did not produce any change in this course, even in the severe stage. This indicates that the increase in C3 will contribute to maintaining the level of the alternative pathway and maintaining the body defence system in the vitamin D-deficient state, and that the complement system will be supported through the alternative pathway.

[Clinical study on ceftezole in oral surgery. Clinical effect of ceftezole used jointly with gamma-globulin (Gamma-Venin) (author's transl)].

Ceftezole, an antibiotic of cephalosporin C derivative was applied to treatment in 39 patients with odontogenic inflammation or postoperative infections. The drug was administered intravenously (1-5 g/day) for the period of 5-10 days. Twenty of them were administered jointly with gamma-globulin (Gamma-Venin). Therefore, we compared clinically between the group of ceftezole with Gamma-Venin and the other group without it. But no difference was noticed statistically between these groups. No side effect was observed with throughout all the cases.
