Late focal pancreatic metastasis of renal cell carcinoma.

Metastatic carcinoma to the pancreas is uncommon. Pancreatic metastasis from a renal cell carcinoma is exceptional, but may occur many years after the initial diagnosis and treatment of the primary tumor. Presentation of our patient mimicked a head of the pancreas carcinoma so well that it was only after the resectional phase of a Whipple operation that the diagnosis of metastatic renal carcinoma was made 18 years after left nephrectomy. The patient is alive and well 18 months after surgery, having gained weight.

The results of surgical techniques in hepatic hydatidosis: treatment with drainage versus treatment without drainage--a 6-year experience.

Fifty-nine consecutive patients who underwent surgery for hepatic hydatid cysts between 1 March 1988 and 31 July 1994, were included in this study. The aim was to compare the results of surgical techniques with respect to post-operative complications, morbidity and recurrence of the disease. Patients were divided into two groups. The first group (1) of patients (n = 30) were treated surgically without drainage and the second group (2) of patients (n = 29) were treated surgically with tube drainage of the cystic cavity. In the study, there were 12 (20.33%) male and 47 (79.66%) female patients, with an age range of between 16 and 85 years. In five of the patients a communication between the cyst and the biliary system was documented. 11.8% of the cysts were localized extrahepatically. Post-operative morbidity was 10% in group 1 and 44.7% in group 2 (P < 0.05). Average post-operative hospital stay was 8.5 days in group 1 and 18.6 days in group 2 (P < 0.05). During the follow-up period, which lasted from 1 to 6 years, recurrence rates were not significantly differentiated in the two groups (P > 0.05).

Transient reverse ventilation-perfusion mismatch in acute pulmonary nitrofurantoin reaction.

A 67-yr-old woman with a history of myocardial infarct was admitted to emergency for marked dyspnea, nonproductive cough, nausea and fever. The thorax X-ray revealed a bilateral alveolar and interstitial infiltration pattern with basal accentuation. The cardiac examinations were normal. Technegas ventilation and Tc-99m-macroaggregated albumin (MAA) perfusion scans were performed to rule out pulmonary embolism. Bilateral multiple ventilation defects with normal perfusion was observed. The patient had been taking nitrofurantoin for four days for a bladder infection. Hypersensitivity to nitrofurantoin was suspected and the drug was discontinued. An antihistaminic and anxiolytic medication was started. The majority of the clinical symptoms disappeared within 24 hours. The control chest X-rays disclosed a marked improvement. Ventilation and perfusion scans obtained 48 hours after nitrofurantoin withdrawal were normal. The drug-related pulmonary reactions should be taken into account in patients on medication. Reversible ventilation defects can be the only lung-scintigraphic finding encountered in acute pulmonary nitrofurantoin reaction.

Estrogen dermatitis.

The aim of the present study was to evaluate the estrogen dermatitis of women who have chronic skin disorders with exacerbations or premenstrual dermatitis in a cyclic pattern. Twenty-three women exhibiting skin disorders of pruritus, urticaria, eczema, papulovesicular eruption, hirsutism-acne with hyperpigmentation (hirsutism and/or its related disorders such as acne) and 18 healthy control subjects were included in the study. Sensitivity to estrogen was described in 14 of 23 women. Of the 14 estrogen sensitive women, nine had a premenstrual flare of their skin lesions and five had a chronic dermatitis with exacerbations. In the evaluation of endocrine profile, mean serum testosterone and LH levels of the patient group were significantly higher than controls (2.814 +/- 0.839 vs. 1.561 +/- 0.645 nm/l, P < 0.001; 10.843 +/- 2.538 vs. 4.539 +/- 1.215 IU/l, P < 0.0001). The LH/FSH ratio of the patient group was also significantly higher than controls (1.765 +/- 0.329 vs. 0.810 +/- 0.0116, P < 0.0001). Mean serum progesterone level of the patient group was significantly lower than the control group (0.499 +/- 0.201 vs. 0.977 +/- 0.396 ng/ml, P < 0.001). Hyperandrogenism and anovulation were the two more common outcomes in the patient group. Skin lesions of estrogen sensitive women were all cured with the administration of tamoxifen 20 mg daily for 7 days premenstrually.

Experimental hydatid disease of the liver.

Hydatid disease caused by Echinococcus granulosis is an endemic problem in many parts of the world and the liver is the most frequently involved organ. A suitable experimental model for hydatid disease of the liver, resembling naturally infected livers, is established in this study by injection of protoscolices via the mesenteric vein of the mice. This model can be used in assessment of the efficacy of various agents in treatment of hydatid disease of the liver.

Spiral nerve cuff electrode for recordings of respiratory output.

The feasibility of using the spiral nerve cuff electrode design for recordings of respiratory output from the hypoglossal (HG) and phrenic nerves is demonstrated in anesthetized, paralyzed, and artificially ventilated cats. Raw neural discharges of the HG nerve were analyzed in terms of signal-to-noise ratios and frequency spectra. The rectified and integrated moving average activity of the HG nerve had a peak value of 1.74 +/- 0.21 microV and a baseline value of 0.72 +/- 0.11 microV at elevated respiratory drive induced by increases in CO2 or oxygen deprivation when recorded with 10-mm-long cuffs. The frequency content of the HG electroneurogram extended from several hundred hertz to 6 kHz. Spiral nerve cuff recordings without desheathing of the nerve provided large enough signal-to-noise ratios that allowed them to be used as a measure of respiratory output and had much wider frequency bandwidths than the hook electrode preparations. A major advantage of the cuff electrode over the hook electrode was its mechanical stability, which significantly improved the reproducibility of the recordings both in terms of signal amplitudes and frequency contents.

Variable kidney position mimicking renal artery branch stenosis in a Tc-99m-DTPA captopril scintigraphy.

Tc-99m-DTPA captopril scintigraphy was performed in a patient with suspected renovascular hypertension. Markedly impaired right renal function (Glomerular filtration rate (GFR) values for the right kidney = 14 ml/min, left kidney = 79 ml/min) was detected in the initial captopril study. Only lower pole activity of the right kidney was observed during the whole study. Since prior ultrasonographic examinations have shown bilateral normal kidney parenchyma, branch stenosis of the right upper pole was suspected. Besides significant function improvement in the following baseline study (GFR values for the right kidney = 59 ml/min, left kidney = 79 ml/min), the right kidney, this time normally shaped, was visibly higher positioned. Because of the possibility of mobile kidney and/or branch stenosis, the patient underwent selective renal angiography. A long pediculed right kidney without renal artery stenosis was found. The final diagnosis was essential hypertension. Kidney position anomalies could influence the reliability of the captopril scintigraphy, particularly when a theoretical kidney depth formula is employed for the attenuation correction.

The effects of nasolacrimal canal blockage on topical medications for glaucoma.

Twenty patients with primary open angle glaucoma who have been treated with identical antiglaucoma eye drops in both eyes were examined. Silicone punctal plugs were used to occlude the inferior punctum of one eye, in order to block the nasolacrimal canal. The intraocular pressures and effects of the medical therapy before and after punctal occlusion were compared. Punctal occlusion significantly decreased the intraocular pressure with an average of 2.00 +/- 0.43 mmHg in the plugged eyes (p < 0.001). The intraocular pressure in the unplugged control eyes did not change significantly (p > 0.05) after punctal occlusion of the fellow treated eye.

Intracystic application of mebendazole solution in the treatment of liver hydatid disease. Preliminary report of two cases.

Mebendazole, a derivate of benzimidazole, is commonly used for the treatment of hydatid disease in patients at high operative risk and/or to prevent secondary hydatidosis. In this study, 2 cases of liver hydatid disease, in which mebendazole was used both orally and for intracystic application are presented. These cases have been treated by percutaneous drainage and videolaparoscopic methods. Mebendazole solution at a concentration of 2.4 micrograms/ml was injected into the cyst. For a period of 6 months 50 mg/kg/day oral treatment was given. At the end of a 6- to 9-month period, the total recovery was observed. This is the first report of mebendazole pair-treatment in the literature.
