Tropical diseases in immigrants and internationally adopted children.

Immigrants, particularly those from the developing world, make up a significant number of tropical disease cases seen in a temperate climate. Internationally adopted children are another group in which tropical and other infectious diseases must be considered. The differential diagnoses of various syndromes in these populations in relation to relatively more common tropical diseases are reviewed.

Serum sickness with furazolidone.

Two cases of serum sickness from furazolidone (Furoxona), prescribed for giardiasis in Latin America, are described. No previous case of serum sickness related to this drug has been reported in the United States. Tartrazine (yellow dye number 5), a component of furazolidone tablets (Furoxona) manufactured in Latin America countries but no longer included in the drug (Furoxone) in the United States is suggested as a possible cause of serum sickness.

Adult

The presence of an estradiol binding component in cytosol from immature rat epididymides.

Cytosol prepared from epididymides of sexually immature (21-23-day-old) rats contains a macromolecular binding component for estradiol-17 beta. This binding moiety sediments as an 8-S species on 5-20% sucrose gradients containing 0.01 M KCl. Under conditions of high ionic strength (0.4 M KCl) the 8-S peak of estradiol binding is shifted to the 4-S region, suggesting dissociation of receptor aggregates. Time-course studies indicated that binding equilibrium was essentially achieved after 2 hours incubation at 0 degrees C. Although unlabeled estrone and estriol are capable of inhibiting [3H]estradiol binding to epididymal cytosol, they are less effective than unlabeled estradiol. Unlabeled 5 alpha-dihydrotestosterone (5 alpha-DHT) at a 100-fold molar excess did not cause a statistically significant inhibition of [3H]estradiol binding. Unlabeled estrogens, but not unlabeled 5 alpha-DHT or cortisol (at the concentrations used), were capable of displacint [3H]estradiol from its binding sites. The dissociation of [3H]estradiol from the binding component is very slow, with half-time of dissociation being greater than 16 hours. The epididymal estrogen binder is saturable at low concentrations of ligand. The dissociation constant was of the order of 10(-11)M and the concentration of binding sites was approximately 10(-14) mol/mg protein. This estrogen binder has the characteristics which are usually attributed to steroid receptors and is clearly different from the testicular androgen-binding protein and the epididymal androgen receptor.

Antibiotic metabolites from a marine pseudomonad.

An antibiotic-producing pseudomonad was isolated from a seawater sample from a La Jolla, Calif., tidepool. The pseudomonad produces two novel antibacterial compounds, 2-n-pentyl-4-quinolinol and 2-n-heptyl-4-quinolinol. It also synthesizes indole-3-carboxaldehyde, 6-bromoindole-3-carboxaldehyde, and the known antibiotic p-hydroxybenzaldehyde. Each of these compounds was identified by analysis of spectral data, and the structures were confirmed by synthesis or comparison with authentic samples.

Anti-Bacterial Agents

Employing task arrangements and verbal contingencies to promote verbalizations between retarded children.

This study investigated the effects of arranging task events for interdependence, to increase the probability of social responding. During task interdependence, the subjects, participating in dyads and a four-person group, obtained task materials (a puzzle piece) from their partner before completing their task (appropriately placing the puzzle piece). The verbal contingency required a verbal request to precede a subject's receiving a task material from his partner. The verbal contingency yoked with task interdependence made task completion contingent on the appropriate verbalization. The findings from two experiments suggested that task interdependence was sufficient to increase partner-directed verbalizations for three of the four subjects. When the verbal contingency was added, all subjects increased their requests and other verbalizations to partner. Applied to a four-person group, the verbal contingency yoked with varying levels of task interdependence correspondingly affected the pattern and level of group communications. The greater the task interdependence, i.e., the more members each subject depended on to complete his task, the more complex the social network of verbal contacts, and the higher the level of both requests and other verbalizations for the group.

Giardiasis.

The recent near-epidemic incidence of Giardia lamblia infection in visitors to the Soviet Union illustrates the importance of this intestinal flagellate as a cause of diarrhea in travelers worldwide. Clinical states range from the asymptomatic cyst-passing stage, to the chronic or subacute stage mimicking gallbladder or ulcer disease, to the transient or, rarer, persistent acute stage with steatorrhea and substantial weight loss. Symptoms may be related to IgA deficiency. Secondary lactose intolerance may follow eradication of the parasite. Diagnosis is usually based on repeated stool examinations or examination of duodenal contents. Quinacrine hydrochloride is the most effective treatment, but metronidazole and furazolidone are also useful. Contaminated water is the most likely source of infection.
