Arteriovenous malformation of the vein of Galen.

Vein of Galen aneurysm may be caused by a spectrum of abnormal arteriovenous connections ranging from extensive malformations involving the tentorial dura and upper brain stem vessels to the rarer condition of a simple end to end fistulous connection. The diagnosis of the latter condition of a simple arteriovenous fistula is important because it may be effectively managed by surgical interruption of the feeding fistulous vessels. Two cases with the direct fistulous connections are presented.

Intracranial ectopic pituitary adenoma. Case report.

The authors report a unique case of ectopic intracranial pituitary adenoma, associated clinically with generalized seizures and aggressive behavior. The lesion presumably arose from cells in the pars tuberalis and did not involve the sella turcica.

Jugular venography for evaluation of abnormalities of the skull base.

Jugular venography done to evaluate abnormalities at the base of the skull demonstrated three distinctly different patterns depending on whether there is occlusion, invasion, or growth with in the internal jugular vein. Improper technique results in a lack of intracranial dural sinus filling which may masquerade as venous occlusion. This problem is avoided by adequate neck compression along with proper volume and rate of delivery of contrast. Radiographically, an abnormal jugular vein at the base of the skull will show a concave defect in true occlusion, constriction or invasion of the vein by tumor, or tumor growth within the vein.

Midline sagittal sonographic evaluation of the midbrain.

A technique has been developed for the A-mode display of the upper brain stem using certain bony landmarks to guide transducer placement. Normal findings were substantiated at the time of penumoencephalography and angiography in order to determine the anatomic origin of the ultrasound reflections. Findings in patients with mass lesions distorting the upper brain stem will also be reported.

Tolosa-Hunt syndrome.

A 58-year-old woman with painful ophthalmoplegia of the left eye associated with thrombosis of the left superior ophthalmic vein and normal angiography is presented. Symptoms cleared on systemic prednisone therapy within 72 hours. The clinical picture associated with the venographic findings and response to steroids are characteristic of the Tolosa-Hunt syndrome.

Tumours of the third ventricle in children.

Experience with 25 cases of tumour of the third ventricle in children is reviewed. The majority of the lesions were infiltrating astrocytomas of the anterior and mid-portions of the third ventricle. These tumours were generally inoperable. However, other tumours within the third ventricle were benign, encapsulated, and resectable. Surgical exposure was carried out through one of three routes: posterior fossa, transcallosal, or subfrontal. The radiological criteria by which the operative approach is determined are discussed.
