Social support of patients attending a geriatric day hospital.

A longitudinal survey of the social support of patients attending one geriatric day hospital confirmed an impression that support was not being adjusted to meet the changing degrees of dependency among the patients. An experimental study, in which the social support available to a small group of consecutive new admissions was reduced, showed that their dependency scores were not significantly different from those of a control group which continued to receive support. The significance of this finding in reducing the cost of day hospital care is discussed.

Acute monoarticular presentation of osteoarthrosis of the knee.

A study of 387 patients aged 65 years and over presenting with an acute hot painful swollen joint as the major or only presentation of osteoarthrosis of the knee is discussed. Short-term phenylbutazone therapy is evaluated. Eighty-nine per cent of patients obtained good to complete relief of acute symptoms within 14 days.

The effects of discontinuing long-term diuretic therapy in the elderly.

The effects of discontinuing long-term diuretic therapy were investigated by means of a double-blind randomized controlled trial. A total of 141 elderly patients in the long-stay wards of six hospitals were found to be taking maintenance diuretics, and for 33 of these the drugs were judged to be mandatory. Of the remaining patients, 52 continued to receive diuretics while 54 were given matching placebo tablets. Eight in the latter group required diuretic therapy to be resumed during the following 12 weeks. The main change observed in patients who completed the trial was a slight increase of ankle oedema in the placebo group. Plasma potassium levels below 3.5 mEq/1 were found in some patients taking diuretics but not when the drugs had been withdrawn for 12 weeks. Blood pressure rose slightly in the placebo group and plasma urea rose slightly in the diuretic group. It was concluded that old people receiving long-term diuretic therapy without obvious current indication should have the drugs withdrawn under careful supervision so that those needing them could be identified.

Evaluation of patients' assessment of day hospital care.

The method of linear analogue self-assessment (LASA) was used to quantify the views concerning day care which were held by patients attending a geriatric day hospital. The results suggest that day hospitals are an acceptable form of care for the elderly.

A double-blind comparison of chlormethiazole and dichloralphenazone: a sedative/hypnotic in geriatric medicine.

In a double-blind crossover trial, chlormethiazole was compared with dichloralphenazone and placebo in 38 elderly patients requiring night-time sedation. Chlormethiazole was significantly superior to dichloralphenazone and placebo for the parameters 'heavy sleep' and'time of onset of sleep'. The incidence of side-effects did not significantly differ whilst on placebo. Because of the high incidence of "placebo reactors', a sub-group who slept badly whilst on placebo were further analysed. The data indicated a definitive trend suggesting that the difference found in the overall material was valid for the sub-group of 17 subjects. Problems of placebo reactors in clinical trials are discussed and the view supported that 'placebo reactors' should be excluded by a preliminary screening test before admission to double-blind controlled trials evalvuating hypnotic drugs.
