[Stereotaxic biopsy for primary lymphomas of the central nervous system].

Primary lymphomas of the CNS are rare. However the incidence of these lesions has increased recently. These tumors are often situated in the thalamus and basal ganglia, but can be multifocal. The treatment for primary CNS lymphoma is radiotherapy alone or in association with chemotherapy. Because the associated morbidity is very low stereotactic biopsy is an ideal method for determining the histologic diagnosis in patients with suspected CNS lymphoma. The authors present a study of 49 patients with primary CNS lymphomas in which a stereotactic biopsy was performed.

Arterial graft reconstruction in an aneurysm of the middle cerebral artery.

A case of a large aneurysm of the middle cerebral artery that was treated by resection of the aneurysm sac and reconstruction of the vessel is presented. The middle cerebral artery was reconstructed using a graft from the superficial temporal artery (STA). Cerebral vessel reconstructions are usually carried out with veins as grafts. This paper shows the possibility of using the STA as a graft. When a vessel reconstruction is contemplated, the skin flap should be planned in order to save the STA.

Cerebral Revascularization↗

Esthesioneuroblastoma with intracranial extension.

The authors present five cases of olfactory neuroblastoma with intracranial extension operated on in the Department of Neurosurgery in collaboration with otorhinolaryngologists. This tumor is most frequently reported as growing inside the nasal cavities, and it can extend to the paranasal sinuses. Cases presenting initially as intracranial tumors have been infrequently reported. At present, histological diagnosis of this tumor is aided by the use of electron microscopy and immunocytochemistry. Total resection combined with radiation therapy remains the most satisfactory treatment.

[Surgery of intracranial metastases of bronchial origin].

Forty-six out of a series of 76 patients with intracranial metastases from lung cancer underwent a surgical operation for complete macroscopic resection. 44 cases presented with a single metastasis. In 2 cases, there were 2 cerebral metastases. Most of the lesions were situated in the frontal lobe (41%). In more than one half of cases, the primary lung cancer was squamous carcinoma. The mean survival was 9 months. It was shorter in the case of anaplastic cancers (6 months) and adenocarcinomas (8 months) than in the squamous carcinomas (12 months). The five patients still alive 2 years after surgery all belong to this histological group. The results suggest that surgical treatment of single cerebral metastases from lung cancer, in the absence of extracranial dissemination, allows a considerable improvement in the quality and duration of the survival.

[Compression of the cauda equina by osteoarthritic pseudo-spondylolisthesis, with overlying signs of deficit. The possible role of a venous mechanism].

The authors report the case of a patient suffering from paraparesis where a venous pathology seemed to be responsible, and the literature is reviewed. A cauda-equina compression by L4/L5 arthrosic pseudo-spondylolisthesis caused troubles of the spinal cord venous drainage, seen at the myelography. There was a neurological deficit above the L4/L5 compression with a psoas and quadriceps deficit. After a L4/L5 laminectomy the neurological signs improved rapidly.
