Disseminated rhinosporidiosis.

This paper reports a case of rhinosporidiosis which presented with widespread subcutaneous nodular lesions which failed to respond to surgical excision or to intravenous amphotericin B. Lesions in sites away from the nasal cavities are usually believed to be brought about by direct infection or by fingerborne autoinoculation. In this case, haematogenous dissemination was shown by the presence of sporangia within the lumen of a blood vessel. Systemic rhinosporidiosis is an extremely rare condition.

Liver biopsies from psoriatics treated with methotrexate.

To study the effects of Methotrexate (MTX) on the liver, light microscopic examination of liver biopsies from 18 consecutive patients with moderate to severe psoriasis receiving MTX seen between 1981-1985, were carried out. All patients received MTX (mean dosage of 1808 gm, mean duration of 88 months) prior to biopsy. There were 12 males, 6 females, with mean age of 55.1 yrs (range 39-77 yrs). All patients were HBsAg negative, non diabetic, and with no significant drug history or alcohol intake. Twenty liver biopsies were examined. Attempts to correlate severity of histological changes to dosage and duration of MTX therapy; and to the levels of raised SGPT at biopsy, failed to show any definite relationship. A variety of histological changes were seen in liver biopsies of MTX treated psoriatics, with 2 patients showing progression of histological changes following repeat biopsy.

Ergometric heart rate, blood pressure and work capacity (PWC170) in type I diabetics with diabetes-specific microangiopathy.

118 male and 68 female type I diabetics and 25 male and 23 female nondiabetic healthy controls were compared during submaximal bicycle ergometer tests using four work stages (duration: 6 min each) of 50 W, 75 W, 100 W and a submaximal stage producing a heart rate of 170 min-1. We found that male type I diabetics with and without retinopathy who had significantly higher than normal heart rates both at the start and during the ergometer test also had above normal ergometric blood pressures. Female type I diabetics, in contrast, did not differ significantly from the controls in terms of heart rate before and during the ergometer tests. The PWC170 of all male type I diabetics was, in contrast to that of the female patients, lower than that of the controls. In diabetics with diabetic nephropathy there was no correlation between exercised acceleration of the heart rate and the degree of nephropathy despite their higher heart rates at the beginning and during the ergometer test, but a correlation was found between the ergometrically increased blood pressure and the severity of the diabetic nephropathy. In male type I diabetics cardiocirculatory adaptation to muscular work was reduced, and this reduction became more marked as the degree of diabetic microangiopathy increased. Compared with the controls, the increase in systolic blood pressure evoked by activity on the ergometer in male and female type I diabetics was disproportionate to the actual load and correlated with the degree of diabetic microangiopathy.

Analyses of nicotine and cotinine in tissues by capillary gas chromatography and gas chromatography-mass spectrometry.

Selective extraction and chromatographic techniques have been developed to measure low nanogram quantities of nicotine and cotinine in tissues. Analyses were performed by capillary column gas chromatography with a specific nitrogen-phosphorus detector and by gas chromatography-mass spectrometry. With close structural analogues for internal standards, high quantitative accuracy and precision were demonstrated for the range 5-1000 ng per g of tissue. The sensitivity limit was 2-3 ng/g for both compounds. The advantage of these techniques compared to previously published methods is increased selectivity; the other methods were developed for analysis of biological fluids and are not readily adaptable to more complex biological matrices such as tissue homogenates. With the newly developed techniques, we were able to perform a pharmacokinetic study of nicotine and cotinine in mouse liver following a single intraperitoneal injection of nicotine.

Serum insulin-like growth factor-II and alpha-fetoprotein as tumor markers of hepatocellular carcinoma.

To evaluate the diagnostic application of serum insulin-like growth factor-II (IGF-II) and alpha-fetoprotein (AFP) levels in hepatocellular carcinoma (HCC), IGF-II and AFP were determined in 100 cirrhotic patients with HCC, 100 sex- and age-matched patients with cirrhosis alone and 50 healthy controls. The results indicated that IGF-II and AFP levels in patients with HCC were higher than in those with cirrhosis alone (p = 0.0001). There is an inverse correlation between IGF-II and (log)AFP (r = -0.410, p = 0.0001) in patients with HCC. Multivariate analysis indicated that IGF-II and AFP were closely associated, in a dose-related fashion, with the presence of HCC. Receiver operating characteristic curves were used to determine the optimal cutoff values of IGF-II (4.5 mg/g prealbumin) and AFP (100 ng/ml), respectively. Both IGF-II and AFP show a high specificity and positive likelihood ratio. The sensitivity was 42.0% for IGF-II and 73.0% for AFP. Determination of both markers in parallel significantly increased the diagnostic accuracy (96.5%) and sensitivity (97.9%), with a high specificity (95.1%) and positive likelihood ratio (19.9). In conclusion, IGF-II and AFP may be used as complementary tumor markers to discriminate HCC from cirrhosis.

Association between seropositivity of antibodies against hepatitis a virus and Helicobacter pylori.

Helicobacter pylori and hepatitis A virus (HAV) are documented to share common transmission routes including fecal-oral. This study examined the association between seropositivity of antibodies against H. pylori (anti-HP) and HAV (anti-HAV) via a community-based survey of 40 randomly selected kindergartens in 10 urban and 10 rural areas. Serum samples from 2,047 healthy preschool children and 104 teachers were screened for anti-HP by enzyme-linked immunosorbent assay, and for anti-HAV by microparticle enzyme immunoassay. In children, a low prevalence of anti-HAV (0.44%) was found, in contrast to a high prevalence in their teachers (78.8%); anti-HP seroprevalence was 6.4% for children and 30.8% for teachers. Anti-HAV and anti-HP seropositivities were significantly associated in teachers after adjustment for age, sex, and residential area through multiple logistic regression analysis (multivariate-adjusted odds ratio = 7.3; 95% confidence interval [CI] = 1.4-36.8, P < 0.001). Our findings suggest that HAV and H. pylori may have shared transmission routes in central Taiwan 15 years or more ago, but not any recently.
