Cross-cultural nursing. A report of faculty collaboration through regional networking.

Healthcare environments increasingly are becoming culturally diverse. A project to incorporate multicultural issues in curricula, identify cross-cultural student learning opportunities, and increase faculty skills in teaching transcultural concepts is described. Through regional networking, faculty members identified barriers to teaching cross-cultural concepts and strategies to address those barriers. Recommendations call for proactive approaches for sharing resources to strengthen cultural dimensions in schools of nursing, particularly in rural areas that lack cultural diversity.

Portfolio applications in a school of nursing.

The portfolio is an effective way to document and evaluate academic and career development. The authors discuss various applications of the process of portfolio implementation via computer in a graduate school of nursing. Portfolio guidelines, format, and a program outcome grid are presented. The portfolio also provides product information that can be used by faculty members for program evaluation.

A preschool severely disabled boy and his powered wheelchair: a case study.

A detailed account of the progress of a preschool child learning to steer a powered wheelchair via a mouth-operated joystick is described. Some problems were encountered during learning, but the positive effect on emotional, intellectual and behavioural development was noted.

Computer microtechnology for a severely disabled preschool child.

The developments and considerations essential to providing microtechnological aids for a preschool severely disabled child confined to a bed in a hospital ward are described. This single case report illustrates the needs of a quadriplegic boy dependent on a ventilator via a tracheostomy. Provision of a computer, a variety of specially designed switches and software together with a self-driven powered wheelchair maximized expression of his developmental needs. The decision-making process of providing the equipment is described as well as the problems encountered. His development in language and cognitive skills is discussed in the context of his change in experience and competence as he learned to control the technological aids.

Preparation of antibody free spermatozoa by in vitro immunodepletion using immunobeads.

In the present study ejaculates from six infertile men with antisperm antibodies were processed on a six step discontinuous percoll gradient. This treatment yielded a sperm fraction with a different percentage of sperm with antisperm antibodies. The resultant sperm preparation was incubated with immunobeads. Sperm were recovered following incubation by swim-up. Sperm recovered at the end of processing showed reduced populations of antisperm antibodies. Even though sufficient numbers of sperm were obtained to consider assisted reproduction, reduced motility was recorded in four of the ejaculates when compared with routine processing. Following trial preparations of semen samples this technique may play a role in sperm selection for assisted conception.

Uptake and release of alcohol by homograft tissues in tympanoplasty.

Alcohol is known to produce severe cochlear damage and it is possible that deafness as a complication of tympanoplasty can be caused by alcohol which remains in homograft materials after inadequate washing. An investigation was made in which the quantity of alcohol taken up by ossicles, cartilage, temporalis fascia, dura and ear-drum was estimated. The rate of release of alcohol from such materials into saline during washing was also measured. The amount absorbed was found to depend upon the nature of the material, its weight and its surface area. The rate of release of alcohol from these materials differed significantly for each material.

Continuing professional education and interacting variables affecting behavioral change in practice: instrument development and administration.

BACKGROUND: Instruments were developed to measure variables identified by Cervero that evaluate the complex processes and effectiveness of continuing professional education. METHOD: Content validity was established with a panel of experts. Four studies were conducted to determine reliability. Data were merged to conduct factor analysis. RESULTS: Reliability was confirmed across studies using Cronbach's alpha. Stability was confirmed in a test-retest study. Factor analysis confirmed instrument items measure the concepts of Cervero's Model. CONCLUSION: The reliable and valid instruments may be useful for continuing education and staff development nurses to measure the effectiveness of education on behavioral change in practice.

Pharmacoeconomics of lipid lowering therapy in Ireland.

Increases in expenditure on medicines above the level of increases in health care are generally, a feature of all Western health systems. From the early 1990's, the average annual growth rate (AAGR) in pharmaceutical expenditure exceeded the AAGR in health among all the European member states 1. In Ireland, the expenditure on drugs, as a percentage of health care spending, was 7.1% in 1987 compared with 9.2% in 1997. The state expenditure on medicines increased from 165.8 million Pounds in 1993 to 278 million Pounds in 1998 representing an average increase of 11% each year. All the available evidence indicates that the expenditure on medicines will continue to show real growth, and take an increasing share of the total health care budget. Analysis shows that the main reasons driving such growth include those of "product mix"--the prescribing of newer, more expensive medicines, in addition to the 'volume effect' comprising growth in the number of tablets per prescription. These two factors account for 80% of the observed increase in drug cost 2. Six therapeutic classes accounted for 16 of the top 20 most expensive drugs prescribed under the GMS in 1998 3. These areas can be classified as follows: peptic ulcer disease, asthma, hypertension/cardiac failure, antidepressants, anti inflammatory and lipid lowering drugs. In this article we discuss the clinical evidence base, and the pharmacoeconomic implications of lipid lowering therapy in this country.

Population based study of the cost of inpatient care for HIV-lnfected individuals in an Irish teaching hospital.

A microcosting evaluation of the cost of inpatient care of 69 HIV-infected patients in an Irish teaching hospital was conducted from the hospital perspective. The median cost per admission was 6220 Euros and median cost per inpatient day was 557 Euros. HIV related admissions were associated with a greater cost per admission than non-HIV related admissions. There was an inverse relationship between inpatient costs and CD4 count. Ward costs accounted for the majority of inpatient expenditure. This is the first detailed study of the cost of inpatient care for HIV-infected individuals in Ireland. This study provides the data necessary to estimate the burden of HIV disease in Ireland and to evaluate the cost-effectiveness of antiretroviral and other strategies to manage HIV infection in the Irish context.

Pain clinic attenders: an audit.

An audit of the first 135 referrals to a new Multidisciplinary Pain Clinic was conducted. Demographic data, pain experience and pain effects were recorded. Chronic Non-Malignant Pain was the diagnosis found most frequently. In that group, the mean duration of pain was seven years, the mean number of surgical operations was 1.95 and the mean number of medications being consumed at the time of presentation 3.5. Impaired occupational, social and recreational functioning was noted. Twice as many women were affected as men and all educational and socio-economic groups were represented. Patient profiles were similar to data published on chronic pain patients attending the Mayo Clinic, Minnesota, USA.

What cost chronic pain?

Chronic non-malignant pain costs the American economy $40 billion a year. An attempt to quantify the total cost to the Irish economy of 95 patients attending a Multi-Disciplinary Pain Clinic was made. The cost to the Health Services was estimated and added to the amount of Social Welfare payments received and the lost earnings of each patient. The results indicated that the 95 patients cost 1.9 million pounds at the time of referral. A subgroup of 22 patients who were younger and unemployed, accounted for 1.5 million pounds. Allocation of resources to treat these patients is necessary.

New and investigational treatment options for ischemic stroke.

We conducted a MEDLINE search of new treatment strategies and experimental agents for treating acute ischemic stroke published from 1989-1996. Clinical trials involving thrombolytics, glutamine release inhibition, N-methyl-D-aspartate receptor antagonism, opioid antagonism, calcium channel blockade, free radical scavenging, membrane stabilization, intercellular adhesion molecule-1 antagonism, ganglioside administration, and growth factor administration were included. Basic research articles were selected based on progress of the therapeutic class toward clinical trials. Approval of tissue plasminogen activator indicates progress in new treatments for acute ischemic stroke. Experimental therapies with potential may become available soon. Recognizing signs and symptoms of stroke is crucial to ensure prompt administration of these agents. The time to diagnosis determines the therapeutic approach for acute ischemic stroke.
