Treatment of chronic active hepatitis (CAH): a retrospective review of 130 patients.

The authors review the course of disease in 130 patients (87 HBsAg-positive and 43 HBsAg-negative) with chronic active hepatitis (CAH) observed in the last six years. Patients were treated with prednisone (36 HBsAg-positive an 8 HBsAg-negative), azathioprine (7 HBsAg-positive and 12 HBsAg-negative) or a combination of prednisone and azathioprine (23 HBsAg-positive and 14 HBsAg-negative), or remained untreated (21 HBsAg-positive and 9 HBsAg-negative). Among HBsAg-positive patients improvement was observed in 54% of the 66 patients treated, and in none of the 21 untreated patients (P less than 0.001). Treatment did not modify the course of the disease in HBsAg-negative patients with CAH; this is probably related either to the small number of patients in each treatment group, or to geographical differences. Furthermore treatment was not effective in patients older than 40 years, whether HBsAg-positive or HBsAg-negative. Three out of 87 HBsAg-positive patients, and two out of the 43 HBsAg-negative patients had died by the end of the observation period irrespective of treatment. Full remission with clearance of HBsAg was observed only in one patient after 4 years of treatment with steroids.

Platelet changes after placement of aortic prostheses in dogs. II. Impaired surface-induced arterial thrombosis.

An experimental preparation was developed to assess changes in surface-induced arterial thrombosis in nine dogs with woven Dacron aortic prostheses. The preparation consisted in measuring the occlusion time of an A-V shunt with controlled flow rate (200 mg/min). This measurement depended on formation of an occlusive, platelet-rich thrombus on a diaphragm of polyester mesh. Two to 6 months after placement of thoracoabdominal aortic prostheses, all animals demonstrated striking prolongation of A-V shung occlusion time. This occurred in the presence and absence of heparin anticoagulation. No change in A-V shunt occlusion time was noted in dogs who had sham operations. These findings complement out studies showing biochemical and functional changes in circulating platelets in dogs with aortic prostheses, and together they are consistent with mild platelet damage sufficient to impair surface-induced thrombosis. They also support our contention than, in dogs, platelets interact reversibly with prosthetic surfaces.

Decreased adenosine deaminase activity in peripheral lymphocytes from tumor patients with metastasis.

We determined the levels of adenosine deaminase (ADA) activity in peripheral lymphocytes of 22 tumor patients: 54% had metastasis; 59% were undergoing chemotherapy at the time of the study. The mean ADA value of all patients was significantly lower than that of 15 healthy subjects of a similar age. When the patients were allocated by treatment and metastasis, the decrease of ADA activity appeared to be associated to the presence of metastasis.

[Action of free fatty acids on platelet aggregation induced with thrombin].

The sinergic effect on thrombin-induced platelet aggregation by Free Fatty Acids (FFA) was studied "in vitro". Our results showed that "sub-critical" concentrations of long carbon chains of saturated and unsaturated FFA potentiated the platelet aggregation induced by "critical" concentration of thrombin. However, we suggest that: a) exogenous long carbon chains of FFA could enhance the platelet membrane lipids and increase the ciclic Endoperoxides, Prostaglandins and Thromboxan A2 synthesis; b) exogenous long carbon chains of FFA could induce the platelets more "sensitive" to aggregating effect of "critical" concentrations of thrombin.

[Potentiating effect of some fatty acids on platelet aggregation induced with ADP, collagen and 1-epinephrine].

Since there is evidence of the correlation between the platelet membrane phospholipids and the aggregating properties of several agents, we studied in vitro the effect of Free Fatty Acids (FFA) on ADP, collagen and epinephrine-induced platelet aggregation. Our results showed that "sub-critical" concentrations of long carbon chains of saturated and unsaturated FFA potentiated the platelet aggregation induced by "critical" concentrations of ADP, collagen and epinephrine. However, we suggest that exogenous long carbon chains of FFA could enhance the platelet membrane lipids and could make the platelets more "responsive" or "sensitive" to lowest aggregating stimuli.

[Adenosine deaminase activity in peripheral lymphocytes during acute viral hepatitis (preliminary data)].

Adenosine deaminase (ADA) activity was determined in peripheral lymphocytes of 6 patients with HBsAg positive acute viral hepatitis (AVH), 8 patients with HBsAg negative AVH and 12 normal controls. The average of ADA activity in these three groups was 25, 25 and 21 nmoles/min/mg prot. respectively without significance for statistical analysis. However high ADA values (40, 32 and 39) were observed in the peripheral lymphocytes of three patients with AVH.

Postnatal behaviour of guanase activity in rat tissues.

The behaviour of guanase activity and the 8-azaguanine/guanine deamination ratio of rat tissues during postnatal development were studied. In the liver and brain, the enzymatic activity, present from birth, gradually increases up to the 4th week. In the kidney, guanase activity appears after the 1st week and reaches the level found in adult tissues after the 6th week. The value of the deamination ratio of tissues at birth is higher than that of adult animal tissues.
