Intraocular lens (I.O.L) implant in traumatic cataract.

I.O.L. either primary or secondary has proved to be useful after traumatic cataract extraction. Problem of binocular single vision will be better solved and the traumatic eye becoming divergent and amblyopic can thus be avoided.

Cytochrome P-450-mediated O-demethylation of two ellipticine derivatives. Differential effect of the murine Ah locus phenotype.

The O-demethylation of two antitumor drugs (9-methoxyellipticine and a 1-polyalkylamino-substituted analog) was studied by incubation with liver microsomes from rats and mice. The former drug underwent a cytochrome P1-450-independent biotransformation in mice, as shown by an indiscriminate response from individuals genetically responsive or nonresponsive to induction by 3-methylcholanthrene. On the other hand, the second drug was O-demethylated only by genetically responsive mice after pretreatment by 3-methylcholanthrene. It was also O-demethylated predominantly in rats pretreated with either 3-methylcholanthrene or Aroclor 1254.

[Short-term tracheobronchial clearance in apparently pure Gougerot-Sjögren syndrome].

Bronchopulmonary involvement in the Sjögren syndrome can lead to distal obstructive airway disease. This syndrome induces a decrease in secretions which become rare and thick, and consequently a slowing down of mucociliary activity. This activity, which can be estimated by tracheobronchial clearance studies, was investigated in seven non-smoking women (mean age = 56.7 yr) with the Sjögren syndrome but without patent distal bronchial impairment. After oral inhalation of radioactive particles labelled with 111In during spontaneous breathing (MMAD = 3 micrograms; sigma g = 1.4; energies gamma 173 and 247 keV, radioactive half-life = 2.8 days), the incorporated radioactivity was measured by a gamma-camera at the end of inhalation and then every 5 min throughout 90 min and 2, 4, 5, 6 and 24h thereafter. Biological decrease in radioactivity was exponential in form. Considering, as may authors do, the clearance of the tracheobronchial compartment to be completed within 24h, we calculated an initial biological period corresponding to the beginning of this clearance. It was equal to 101 +/- 27 min in 12 normal subjects. It was normal in three of our patients (T = 90, 91 and 101 min) and longer for the four others (T = 178, 203, 240 and 304 min). We hypothesize that three of these four patients with slow clearance and normal penetration index (tracheobronchial deposition/pulmonary deposition) had bronchiolar involvement, creating anomalies in the mucociliary escalator which were undetectable by clinical, radiological or functional examinations. These results argue in favour of strict pulmonary surveillance in patients with altered short-term clearances to enable early detection of bronchiolar disease.

Human papillomavirus infection of the cervix: the atypical condyloma.

We report on 162 cases of human papillomavirus (HPV) infection of the cervix seen in a two-year period in which the cell sample showed such marked atypia that errors of interpretation could easily have been made. These atypical condylomata are difficult to diagnose cytologically as well as histologically because they mimic dysplasia or carcinoma in situ and, on smears, even invasive squamous carcinoma. HPV particles associated with fibrillar material were found within nuclei of these lesions; their nature was further proved by the immunoperoxidase test. This new form of HPV infection of the cervix showed a 9.1% rate of progression to more advanced cervical lesions. The cytologic finding of atypical condylomata is an indication for colposcopy, confirmative biopsy and appropriate treatment.

Origin of the koilocyte in condylomata of the human cervix: ultrastructural study.

Twelve biopsies from condylomatous lesions of the human cervix were examined by transmission electron microscopy in order to study koilocytotic cells. The ultrastructure of koilocytes, revealed mainly cytoplasmic changes in the perinuclear area. These changes first appeared in cells of the intermediate layers and extended to the surface of the epithelium. The cells involved showed small perinuclear areas, with a fine granular material, which appeared as a lysis of the cytoplasmic matrix, forming microvacuolelike structures that gradually enlarged and fused together, ending with the formation of a large clear zone (koliocytosis). Binucleation and polylobulation were constant features. Intranuclear virus particles were often associated with koilocytes. Tubules and large bundles or single filamentous structures were also observed in koilocytes detected in smears and biopsies. This study provides further evidence that the koilocytes is the result of a process characterized by cellular changes due to cytopathogenic effect of the human papillomavirus.

Epidemiologic analysis of Trichomonas vaginalis infection in inflammatory smears.

Infections of the lower reproductive tract are common in Indian women of reproductive age. Hospital-based cytologic screening was undertaken on 63,265 women. The smears were examined for the presence of specific infections, such as Candida, herpes simplex virus, human papillomavirus, Trichomonas vaginalis (TV) and Chlamydia. This paper highlights the prevalence of TV infection in inflammatory cervical smears. Among the various infections detected, the rate of TV infection was the highest (5.1%). On further analysis the rate of TV infection showed an increasing trend up to the age of 49 years; an inverse association was observed with the educational status of the women. The prevalence was high in women with clinical signs (vaginitis, 6.9%) and low in those with a prolapsed uterus (1.2%) as compared to a normal cervix. These observations reveal the need for providing proper counseling and education on sexual behavior and genital hygiene besides treatment to control and prevent these infections.
