[Balint syndrome as a result of a biparietal infarct. Neuropsychological analysis].

A case of Balint syndrome with bilateral parietal infarction is presented. Analysis shows the tight intrication of visual inattention and psychic gaze palsy. The deficit of visuo-manual adjustment is characterized by a fixed divergence between the target and the point where the patient places his hand, either in ballistic movements (classical optic ataxia), either in pursuit movements; for this reason, a simple disconnexion should not explain the classic optic ataxia. The discretion of aphasia allows the individualization of an anterograde and partially retrograde amnesia.

[Solitary intraspinal dorsal neurinoma. A case report].

Most spiral neurinomas are intradural extramedullary tumours. Intramedullary neurinomas are rare: only 20 cases have been published. The authors report a new case of solitary intramedullary dorsal neurinoma and review the main diagnostic and therapeutic problems raised by this type of tumour.

Retroclival cysts. Report of two cases.

Two cases of retroclival arachnoid cyst are presented. The clinical symptoms, putting aside the hydrocephalus, are dominated by pyramidal signs, oculomotor disorders and ataxia. In both cases, a posterior suprasellar extension was found as in the previous descriptions in the literature. This shows the close pathological relations of these cysts to the more classic suprasellar cysts.

[Postoperative abscesses and empyemas. Apropos of 13 cases].

13 cases of severe intracranial postoperative infections, abscesses and/or empyema, are reported. These lesions, whose clinical diagnosis is often difficult, have benefited from C.T. scan. The diffusion of the infectious process in operatory focus, the frequency of Staphylococcal origin and the importance of high doses antibiotherapy must be underlined. This one is, of course, an indispensable complement to reintervention, but it may be utilized alone with success in selected cases, or insure the definitive cure when reintervention is partial.

[Septo-optic dysplasia disclosed in adults].

A case of septo-optic dysplasia, presenting with focal epileptic fits in a 29 year old man, is described. Gas encephalography and CT scan demonstrated the main features of the malformation: a single ventricle overhanging a narrowed third ventricle, abnormal mass in the basal forebrain corresponding to an embryonic cyst, optic atrophy. Angiography revealed deep venous anomalies, and, in particular, absence or hypoplasia of the sinus recti.

[Disruptions of hemispheric blood flow and deep cerebral hematomas].

Cerebral blood flow measurements with single photon emission tomography and 133Xe inhalation was performed in 18 patients with hematomas, 14 thalamo-capsular, 4 capsulo-lenticular. In thalamo-capsular cases and in standard conditions, ipsilateral hemispheric C.B.F. and contralateral cerebellar C.B.F. were decreased; a concomitant depression of frontal and temporal C.B.F. was often noted. A similar change was found in capsulo-lenticular cases, the frontal C.B.F. being however less depressed. An attenuation of interhemispheric asymmetry was often found after acetazolamide intravenous injection. A significant correlation was demonstrated between interhemispheric asymmetry index (AI) and frontal and temporal AI.

[Dorsal disc herniation. 13 cases].

Thirteen patients with thoracic disc herniation have been operated on between 1960 and 1984. Nine presented with a progressive cord compression syndrome affecting sensory function more than motor. Four patients presented with acute cord compression. During this period, the authors have abandoned medullary angiography and myelography. The diagnosis is now based on computerised myelotomography. Three cases were treated by laminectomy with two postoperative deteriorations. Two cases were treated by a transthoracic approach which seemed to be too risky. Eight cases were treated through a postero-lateral approach, six being through a transverse arthropediculectomy using a microscope for better dissection of the vessels and Harrington rods to avoid secondary kyphosis. Seven of 9 cord syndromes were improved. Four cases with predominantly radicular syndromes were all improved.

Decreased morbidity from acute bacterial spinal epidural abscesses using computed tomography and nonsurgical treatment in selected patients.

We describe 5 patients with spinal epidural abscesses in whom computed tomographic scanning confirmed diagnosis without the use of myelography. One patient required urgent surgery because of rapidly deteriorating neurological status, but the other 4 were treated nonsurgically. The medical treatment of these patients and 9 others described in the literature consisted of antibiotics administered parenterally for a minimum of 8 weeks, followed by oral antibiotic therapy. Early diagnosis with computed tomographic scanning and a benign neurological state at the onset of treatment were associated with good results.

Production and characterization of monoclonal anti-Thy-1 antibodies that stimulate lymphokine production by cytolytic T cell clones.

In an effort to derive monoclonal antibodies (mAb) which can induce production of macrophage-activating factor (MAF) by cloned murine cytolytic T lymphocyte (CTL) lines, we have fused spleen cells from a rat immunized with a CTL clone with the nonsecreting mouse myeloma X63-Ag8.653. Three mAb (designated I-22, III-5 and V-8) were found to stimulate MAF production by the immunizing CTL clone and (with a single exception) two other unrelated CTL clones. However, none of these mAb inhibited the cytolytic activity of the clones. Immunoprecipitation studies indicated that the three mAb reacted primarily with a 25-30-kDa protein which could not be distinguished from that precipitated by either a reference anti-Thy-1.2 mAb or a polyclonal rabbit anti-Thy-1 antiserum. Moreover, competition binding experiments demonstrated that the three mAb competed with each other and with the reference anti-Thy-1.2 mAb. Flow cytofluorometric analysis of the strain distribution of the molecules defined by the mAb revealed that two of the antibodies (I-22 and III-5) were directed against nonpolymorphic determinants of Thy-1, whereas V-8 mAb reacted only with Thy-1.2+ lymphocytes. One of the mAb (III-5) was also able to stimulate proliferation and interleukin 2 secretion by normal splenic T cells. Since mAb directed against a number of other surface structures on CTL clones did not stimulate MAF production, it thus appears that Thy-1 (or molecules associated with Thy-1) may play a functional role in T lymphocyte triggering.

Long term sequelae of hemispheric abscesses as a function of the treatment.

The authors compared the clinical, electroencephalographic and CT evolution of 31 cases of brain hemispheric abscesses. The vital prognosis was largely improved by the introduction of the CT scan, as compared with previous series. The functional evolution was better in the group of patients receiving high dosage (15 cases) antibiotics and/or puncture (4 cases) in the case of a large lesion, than in patients whose abscesses were removed (12 cases). The CT scan confirmed this difference, with frequent atrophy after excision.

Spinal intradural arachnoid cysts.

Based on the study of 8 cases of spinal intradural arachnoid cysts, the authors underline that the diagnosis is sometimes difficult because of the limitations of the paraclinical examination. They discuss aetiopathological problems.

Cervical neurological complications of rheumatoid arthritis. Surgical treatment techniques and indications.

A critical study of 11 cases with neurological complications resulting from damage to the cervical spine during rheumatoid arthritis prompts the authors to reconsider their therapeutic strategy. After recalling the various neurological complications and their mechanisms, they propose an extension of the range of indications for surgical treatment. The limits of this extension are defined according to neurological and radiological criteria and the risks inherent in each method.

Developments in the treatment and prognosis of multiple cerebral abscesses.

Multiple cerebral abscesses have been recognized with increasing frequency because of the use of computed tomography and have been the subject of various therapeutic proposals. Their prognosis has been considerably improved thanks to high dose antibiotic therapy. However, diagnostic and therapeutic problems persist.

Cell surface glycoproteins involved in the stimulation of interleukin 1-dependent interleukin 2 production by a subline of EL4 thymoma cells. II. Structure, biosynthesis, and maturation.

In the present study, we examined the biosynthesis and the maturation of two distinct membrane glycoproteins detected by two monoclonal antibodies (RL388 and RL119), which were selected on the basis of their ability to stimulate the production of interleukin 2 by a subline of the murine EL4 thymoma. RL388 detected a disulfide-linked heterodimer complex (Mr = 130,000) composed of a glycosylated heavy (Mr = 86,000) and a nonglycosylated light (Mr = 39,000) subunit. The unglycosylated precursor of the heavy chain was a polypeptide of Mr = 57,500, which was converted upon maturation into a Mr = 73,000 core-glycosylated intermediate, and then into the Mr = 86,000 surface-expressed molecule. Partial endo-H digestion of the core-glycosylated form suggested the presence of four N-linked glycan units. The antibody reacted with a protein determinant expressed on the mature form as well as the unglycosylated precursor of the heavy chain. Moreover, both subunits assembled rapidly during biosynthesis, and the glycosylation of the heavy chain was not required for this association. Taken together, these data suggest that the antigen detected by RL388 may be the murine homologue of the human 4F2 antigen. The antigen identified by RL119 was a surface glycoprotein of Mr = 55,000 with three to five N-linked glycan units. The unglycosylated precursor polypeptide was of Mr = 29,000. The fully core-glycosylated form of Mr = 41,000, which was detected after inhibition of glucosidase I with 1-deoxynojirimycin, was converted into a Mr = 39,000 intermediate, and upon further trimming, into a Mr = 36,000 endo-H-sensitive form. The latter could be detected for chase periods of over several hours, thus suggesting a low rate of intracellular processing. The wide cellular distribution of the molecules identified by RL388 and RL119 and their preferential expression on the surface of growing cells suggests that they may be associated with cell activation events.
