[Activation of deficient right cerebral blood flow by language after global aphasia].

A 39-year old patient abruptly presented with global aphasia. Six years later a mild deficit of articulation and sentence comprehension persisted. A cerebral blood flow study showed that a) during verbal activations, the blood flow increased in the right hemisphere and decreased around the lesion in the left hemisphere; b) the activation was globally higher than in the control group. These data suggest that the right hemisphere plays a significant role in the recovery from aphasia in patients with severe left hemispheric lesions.

Isolation from mouse fibroblasts of a cDNA encoding a new form of the fibroblast growth factor receptor (flg).

Structural definition of the receptors for neurotropic and angiogenic modulators such as fibroblast growth factors and related polypeptides will yield insight into the mechanisms that control early development, embryogenesis, organogenesis, wound repair and neovessel formation. We isolated 3 murine cDNAs encoding different binding domains of these receptors (flg). Comparison of these ectoplasmic portions showed that two of the forms corresponded to previously described murine molecules whereas the third one had a different ectoplasmic portion generated by specific changes in two regions. Interestingly, expression of this third form seems to be restricted in its tissue distribution. Such modifications could influence the ligand specificity of the different receptors and/or their binding affinity.

Evaluation of somatic cell variants deficient in glycosylphosphatidyl-inositol anchoring as candidates for genetic correction.

Many cell surface glycoproteins are anchored in the lipid bilayer by a glycosylphosphatidyl-inositol (GPI) structure. Recently, a number of cell lines which are deficient in the biosynthesis and/or addition of this anchor have been described. In this report, we summarize the current knowledge on these lines and discuss their potential use to isolate the genes involved in the GPI anchor biosynthetic pathway with a specific emphasis on L cell fibroblasts.

Perfusion mapping with Tc-HMPAO in cerebral haematomas.

The cerebral distribution of HMPAO was mapped tomographically by SPECT in 20 patients with an intra cerebral haematoma. The images were assessed visually and by an analysis of an asymmetry index (AI) for various cerebral areas. Visual inspection identified the lesion in each of the 20 cases. Reduced tracer activity and abnormal AI were consistent findings. Remote effects (diaschisis) were also observed in the adjacent (19 cases) and frontal (14 cases) cortex, and in the controlateral cerebellum (16 cases). Sequential studies at 60, 150 and 300 minutes in 10 patients showed remarkably stable patterns. HMPAO maps were compared with cerebral blood flow (CBF) measured with 133 Xenon (133Xe) in eight cases. The two methods showed similar results in the region of a lesion, but remote effects were more obvious with the 133Xe in two patients.

Upbeat and downbeat nystagmus occurring successively in a patient with posterior medullary haemorrhage.

In a patient with posterior medullary haemorrhage, first upbeat and later downbeat nystagmus occurred in the primary position. The lesion was limited to the posterior and medial part of the medulla. Clinical and electro-oculographic examination first showed upbeat nystagmus in the primary position and upgaze, with downbeat nystagmus in downgaze. Two and a half months later, there was downbeat nystagmus in the primary position and downgaze and upbeat nystagmus in upgaze.

[Amnesic syndrome caused by limited infarction in the right anterior thalamus].

A case of global and persistent amnesia due to a right polar and anterior thalamic infarction is reported. There was a moderate impairment of attention and categorization ability. The amnesia was primarily anterograde, with partial disturbance of short-term memory and severe deficit of long-term memory for verbal and visuo-spatial materials. A severe increase of pro-active interference was present. Retrograde memory was mildly affected. Two years later, a second infarction in the right thalamo-sub-thalamic territory occurred, increasing the deficits of attention, short-term memory and recall.

Management of focal intracranial infections: is medical treatment better than surgery?

Three groups of patients with single hemispheric brain abscesses or subdural empyemas, from 1 to 5 cm large, with similar initial prognosis, have been treated either by medical treatment alone (20), aspiration (21), or excision (15). Differences in survival were not found, but medical treatment alone was better for long term sequelae. Surgical procedures (either aspiration or excision) were better for both isolation of the organism and the hospital stay before discharge. In spite of good results, it is unwise to conclude too strongly in favour of no surgical treatment as this study was not randomised.

[Correlation of thalamic aphasia and cerebral blood flow].

Relations between linguistic deficits and cerebral blood flow (CBF) were studied in 20 cases of thalamic aphasia due to hemorrhage. Language analysis was based on BDAE, verbal intelligence quotient and verbal subtest of the memory quotient (Wechsler). CBF analysis (and of asymmetry index: AI) was done with 133 Xenon by SPECT technique in tomographic slices and in 15 areas of interest, i.e. cortical and deep areas. Relationships were analyzed by multiple correlations procedure and stepwise regression. Significant correlations were observed between linguistic results and AI of cortical but also deep areas (lenticular). Dynamic anomalies (fluency) were correlated with the IA and/or CBF of the frontal cortex. Verbal comprehension, naming and paraphasia were related to the AI of deep structures (insula and lenticular nucleus) and the AI of posterior cortex (temporo-occipital). Several correlations were found significant between results on verbal IQ of the WAIS and IA of the insula and lenticular nucleus.

[Prolonged remission in subacute sclerosing panencephalitis: 2 cases].

Long survival in subacute sclerosing panencephalitis (SSPE), including total disappearance of clinical signs, is rare. Two cases are reported. They concern a girl and a boy who, at age 13 and 15, developed SSPE and are still in remission 6 and 5 years later. After a typical onset and course over periods of 12 and 18 months, clinical improvement was observed and periodic EEG complexes disappeared. However, the electrophoretic oligoclonal pattern of CSF proteins and the elevated measles titers persisted (in one case specific CSF IgM were still increased 6 years after the onset). MRI showed asymmetrical areas of high-intensity signal in both white and gray matter, predominant in the temporal, parietal and occipital regions. The age at which SSPE begins and the interval between measles and SSPE onset are not prognostic factors. On the other hand, in reported cases with lasting remission SSPE did not progress beyond Jabbour's stage II. The second typical feature of these long-term improvements is disappearance of EEG periodic complexes and emergence of a normal basic background activity. No other prognostic factor has been reported.

[Clinical course after surgery of aneurysms of the anterior communicating artery].

The evolution and correlations between initial state and outcome (one year) are analysed in 81 patients with anterior cerebral artery aneurysm. The age and initial evaluations are seldomly correlated with late evaluations, Jennett and Holbook scales, frontal syndrome, amnesia. The evaluations at one month are very significantly correlated to the late evaluations.

[Rhythmical myoclonus and tremor at rest disclosing mesencephalic metastasis].

We report a case in which rhythmical myoclonus and tremor at rest revealed a thalamo-subthalamic metastasis from a bronchial carcinoma. Tremor of the upper limbs and face (4 Hz) disappeared with sustained posture and action. A cogwheel phenomenon, hypotonia and disorders of automatic and voluntary movements were also present. Surface electromyographic recordings showed a rhythmical, synchronous activity of the biceps brachialis and triceps muscles at rest. Pathology disclosed lesions of the red nucleus and neighbouring area and severe compression of the substantia nigra which were likely to be the cause of the signs and symptoms.

In vitro attachment of glycosyl-inositolphospholipid anchor structures to mouse Thy-1 antigen and human decay-accelerating factor.

Glycosyl-inositolphospholipid (GPL) anchoring structures are incorporated into GPL-anchored proteins immediately posttranslationally in the rough endoplasmic reticulum, but the biochemical and cellular constituents involved in this "glypiation" process are unknown. To establish whether glypiation could be achieved in vitro, mRNAs generated by transcription of cDNAs encoding two GPL-anchored proteins, murine Thy-1 antigen and human decay-accelerating factor (DAF), and a conventionally anchored control protein, polymeric-immunoglobulin receptor (IgR), were translated in a rabbit reticulocyte lysate. Upon addition of dog pancreatic rough microsomes, nascent polypeptides generated from the three mRNAs translocated into vesicles. Dispersal of the vesicles with Triton X-114 detergent and incubation of the hydrophobic phase with phosphatidylinositol-specific phospholipases C and D, enzymes specific for GPL-anchor structures, released Thy-1 and DAF but not IgR protein into the aqueous phase. The selective incorporation of phospholipase-sensitive anchoring moieties into Thy-1 and DAF but not IgR translation products during in vitro translocation indicates that rough microsomes are able to support and regulate glypiation.

[Dolichoectasic cerebral arteries and aneurysm of the abdominal aorta. Double vascular rupture, favourable course].

The authors report a case of dolichoectasic intracranial arteries associated with an aneurysm of the abdominal aorta. The patient presented with a double hemorrhagic vascular rupture, the abdominal aortic aneurysm being operated on as an emergency, with a favorable outcome. The cerebral and aortic angiograms showed extension of the pathological process to the large caliber arteries. Microcirculatory investigations at the nail bed and conjunctiva also showed distal lesions. The vascular abnormality underlying these multiple ectasic lesions would seem to be different to that of classical arteriosclerosis. Anatomopathological studies reported in the literature describe fibrous degenerative lesions of the media differing from those observed in fibromuscular dysplasia.

[Treatment of severe postoperative infections in head surgery. 20 cases].

The authors report the management and follow-up of 20 patients with severe postoperative intracranial infections, abscesses and/or empyemas. Clinical neurological diagnosis was difficult in patients with deficits resulting from the first operation but over half the cases had an inflammatory reaction in the operated zone. CT scan showed that the infection was not limited to a single site (e.g. the subdural area) but was more diffuse in the majority of cases. Antibiotics were given alone in 10 patients and in association with empyema aspiration or reoperation in the other 10 patients. There were two fatalities but the other patients made a generally good functional recovery.

[Secondary and late development of hematoma of the brain stem].

The functional and social outcome in 15 patients with brainstem hematomas surviving on the 15th day is reported. At the end of the first year post stroke, the main neurological deficits were sensory, postural and paresis of the 6th and 7th cranial nerves. This resulted from the preferential location of the lesions in the lateral and posterior pons. On a functional point of view, 8 patients were independent in daily living. Seven of them were regularly engaged in more elaborated activities, at home and outside: 1 had recovered his previous professional activities. Those functions evolved more slowly during the 2nd year and later on. A significant correlation was found between the initial neurological status and the functional outcome at 1 year, and also between the volume of the hematoma on C.T. scan and this outcome. In this series, the age of the patients was not correlated to the other variables.

[Cerebral blood flow deficit and thalamic infarcts. Topographic and neuropsychological confrontations].

Cerebral blood flow (C.B.F.) measurements with continuous xenon 133 inhalation and single photon emission tomography were performed in 11 patients with uni- or bilateral thalamic infarctions. Anterior infarction was associated with changes in parietal and anterior frontal C.B.F., thalamo-subthalamic infarctions with a more diffuse and definite reduction of C.B.F., thalamo-geniculate and superior juxta-thalamic lesions to a reduction of temporal and parietal C.B.F. A relative anatomo-functional concordance was found between the localization of the infarction and the cortical and deep situation of the C.B.F. reduction. Relations between the site of this reduction and the quality and degree of neuropsychological involvement were less clear. A verbal activation was realized in a patient with thalamo-subthalamic infarction which failed to evoke the C.B.F. elevation observed in normal subjects.

[Tomographical use of Tc hexamethylpropylene amine-oxime. First experience (78 studies)].

A new radiotracer of cerebral perfusion, 99mTc-labelled hexamethylpropylene amine-oxime, has been tried in 78 subjects: 6 controls and 72 patients. Qualitatively, the distribution of this tracer in healthy subjects was very much the same as that obtained with a reference method using 133xenon inhalation. Quantitatively, there was no correlation between the real blood flow rate and the normalized cerebral uptake rate. On the other hand, the asymmetry indices obtained in controls (but also in 16 patients) correlated very closely with those obtained with 133xenon. Our first results in acute ischaemic diseases as well as in the evaluation of vasospasm or Alzheimer-like presenile dementia point to wide fields of application for the new compound. Unlike its predecessors, it is always available and will probably be used, without any logistic investment, with the standard equipment of all Nuclear medicine departments.
