[3 years of "new developments" in pharmacotherapy. An update which is also a proposal for participative experimentation].

The analysis of the contents of key national and international drug bulletins provides a comprehensive overview of what has been produced worldwide in the field of pharmacology and therapeutics over the last few years after the last publication in R d 1). The work presented, tries to avoid the risk of being a purely descriptive listing of true and false "new" information, and may be seen as an "experiment of information transfer": the available data are presented and summarised from various complementary points of view, to suggest also a methodology of permanent education, for which instruments are also proposed. Further, the text is tentatively proposed as an experiment of communication between nurses and pharmacists, to stimulate their dialogue and interaction both with respect to the use of drug literature and to the evaluation of daily needs of information in routine hospital practice.

[Primary aneurysms of carotid bifurcation: surgical management].

From 1980 to 2005, eight patients, four male and four female, age range between 33 to 86 years (average 55.2) with the diagnosis of primary aneurysm of the carotid bifurcation, underwent surgical management. Patients with postendarterectomy pseudo-aneurysms (also rotulated as "secondary") were excluded from the study, due to different pathogenic, clinical and therapeutic features. Four aneurysms were degenerative by nature, occurring in the older patients of the series. Some other etiologies include arterial fibrodysplasia, brucellosis, Behçet's disease and penetrating wound of the neck. All patients underwent the surgical resection of the aneurysm, followed by reestablishment of the carotid continuity in 6 cases; two patient had a definite ligation of the internal and external carotid artery, respectively. There was no operative mortality nor significant morbidity. From 1 to 25 years after the operation, three patients died for different reasons, two were lost for follow up and the remaining three are found alive and well. The main features of clinical presentation, etiology, diagnosis and surgical management of this entity are presented and discussed and compared with similar experiences reported in the international literature.

[A comparison of the life habits of abstemious patients and drinkers].

The life habits of 358 males abstemious (ABS) and 248 male risky or with harmful alcohol consumption (BRD) are compared; selected from the patients attending to a clinic of familiar medicine, of the Instituto Mexicano del Seguro Social (IMSS) and to General Hospital of the Secretaria de Salubridad y Asistencia (SSA). BRD are different from ABS because of a sedentary life, tobacco consumption, higher frequency of other drug use, poor nutrition, over weighted and poor physical condition. The difference between ABS and BRD is not only due to the alcohol consumption and related problems, but two groups of individuals with different ways of life. The present paper shows the first Mexican results of an International Multicentric Investigation coordinated by World Health Organization (WHO).

[Nizatidine].

Nizatidine is a new H2-receptor antagonist, as potent as ranitidine. It does not interfere with hepatic metabolism and it lacks anti-androgenic effects. An evening dose of 300 mg suppresses nocturnal acid secretion without diurnal carryover. Published clinical trials are limited. Over 3800 patients who have been treated with nizatidine, 300 mg for up to eight weeks or 150 mg at night for a year, did not appear to have significantly unexpected or unwanted side effects. Rate of healing for both duodenal and gastric ulcers and reduction in the rate of relapse are comparable to those of ranitidine.

Drug Evaluation↗

[Drugs and pregnancy].

The need for further information on drug utilization patterns during pregnancy in different countries was assessed by reviewing literature by hand and computer searches. The resulting profile, from 13 identified studies, was that pregnant women used an average of 4.7 drugs. The most commonly ingested medications were vitamins and iron preparations (almost all women), analgesics, antiemetics and antacids. However, important variables (such as date of surveillance; country; size of involved population; habits; physiopathological and demographic characteristics), differently taken into account in each selected study, made it impossible to construct a comprehensive, detailed, up-to-date picture about drug utilization during pregnancy. The evaluation confirmed the need for and value of systematic permanent surveillance of drug utilization in pregnancy, so as to avoid the use of data obtained in widely differing contexts, times, and methods, in a field where knowledge is often based on scanty information.

[Diagnosis and classification in psychiatric disorders].

This paper provides a general overview of the evolution of the diagnostic concepts in Psychiatry, of the research methods for future improvement, of the goals reached and of the deterrants that have to be surpassed. Classification, standardization and systematization of mental disorders has not been an easy task. For this reason, during a long period of time a wide diversity in the classification of the mental illness that ran along with the thinking trend of each and everyone of the different authors has been observed. This resulted in a division of the specialty and in the contamination of its ideologies and also, it has damaged the reputation of the diagnosis. The development of psychopharmacology and of the epidemiological systems made necessary the implementation of a strict diagnostic system that could make possible the planning of treatments, the establishment of prognosis and the design of preventive measurements. England was the cradle of this change due to the fact that the situation of the specialty availed itself of the influence of the prestigious and noteworthy German professors exiled in this country. Since 1972, some of the material that has been published illustrates the new tendency of nosology. Since then, there has been a great proliferation of works in this field. The culminating point of these investigations is represented by the Diagnosis and Statistical Manual of the American Psychiatric Association and by the International Classification of Illnesses (ICD-10). From this point of view, diagnosis is just a provisional hypothesis of a work that can be identified because of its pathological expressions and which is also of help in the communication between specialists.(ABSTRACT TRUNCATED AT 250 WORDS)

[The factors associated with hospital maternal death in the state of Morelos, Mexico].

The objective of this case-control study was to assess some risk factors related to hospital maternal mortality in the state of Morelos, Mexico. The cases were all maternal deaths which occurred in Morelos between 1989 and 1991 (n = 35), and the controls were women who presented the same complications and were taken care for at the same hospitals than the cases, but survived pregnancy and delivery (n = 35). The results showed that living with a partner is a protective factor (OR = 0.08), as well as using family planning methods (OR = 0.15). Not receiving care at the first place where it was requested was a risk factor (OR = 6.6). Maternal deaths had the same number of prenatal care visits than controls, but their first visit occurred, in average, one month later. The implications of the small sample size, characteristic of the studies on maternal mortality, are discussed at length. The results pinpoint the need to strengthen high-risk pregnancies screening programs through early prenatal care, and promote intersectorial strategies to provide care to pregnant women.
