Exercise tests and ventricular tachycardia.
All treadmill exercise tests done at the Palo Alto Veterans Administration Medical Center from 1973 to 1982 were reviewed to identify episodes of ventricular tachycardia (>/=3 consecutive ventricular ectopic complexes) or ventricular fibrillation occurring during or within 8 minutes of cessation of symptom-limited exercise. Of patients with a clinical diagnosis of coronary artery disease (900 tests), ventricular tachycardia occurred in 36 (4.0%) and ventricular fibrillation in 6 (0.7%). Of patients without known coronary disease (1,700 tests), ventricular tachycardia occurred in 12 (0.07%), and no patient had ventricular fibrillation. Most arrhythmias ceased spontaneously and only 5 patients required cardioversion. We conclude that exercise tests are safe, the incidence of ventricular tachycardia or ventricular fibrillation is low-these arrhythmias occurring largely in patients with known coronary disease-spontaneous return to sinus rhythm is common and no deaths were associated with 2,400 consecutive tests.

Detection of coronary artery disease in aortic stenosis by exercise gated nuclear angiography.
Because the clinical diagnosis of coronary artery disease (CAD) in the presence of aortic stenosis (AS) is difficult, the value of exercise gated nuclear angiography in detecting CAD in 33 patients with AS was assessed. Exercise left ventricular (LV) ejection fraction (EF) and wall motion analysis were evaluated after symptom-limited supine cycle ergometer exercise. Sixteen patients had severe AS (valve area 0.8 cm2 or less). Thirteen had significant associated CAD (50% or greater reduction in luminal diameter of 1 major coronary artery). Twenty patients had normal coronary arteriograms. All 10 patients with mild to moderate AS and normal coronary arteries had normal nuclear studies. Patients with CAD, regardless of the severity of AS, had a decrease in LVEF during exercise (12 of 13 patients) and regional wall motion abnormalities (11 of 13 patients). Abnormal exercise gated nuclear ventriculographic studies occurred in the absence of CAD in 10 of 20 patients, and all had severe AS (mean aortic valve area 0.8 cm2 or less, range 0.4 to 0.8). Ten had an abnormal LVEF response to exercise and 7 had exercise-induced abnormal wall motion. These findings suggest that the presence of an abnormal LVEF, whether in conjunction with an abnormal wall motion analysis, is indicative of further invasive evaluation; conversely, in those patients with a normal response of LVEF and normal wall motion during exertion, invasive studies may be safely deferred.
Jerking stiff-man syndrome.
A female patient had permanent axial muscular rigidity similar to the "stiff-man syndrome", together with axial myoclonus triggered by stretch reflexes and by supramaximal stimulation of the supraorbital nerve. The disorder responded to treatment with diazepam and baclofen. This disorder bore a marked similarity to the so-called "jerking stiff-man syndrome".

Induction of organ-specific antigens of the rabbit male accessory glands by injection of testosterone or gonadotrophin.
Highly specific antiserums for two antigens of rabbit male accessory glands, called FII and FV, were used to study the effect of hormonal treatment on their appearance during postnatal development. Single or double serial injection of testosterone, or human chorionic gonadotrophin, produces an increase in accessory gland and testis weight, and at the same time induces the synthesis of a detectable amount of specific antigens. This was proved by using groups of animals 4 and 6 1/2 weeks old. The antigens were consistently absent in accessory glands from normal and control animals of the same age. The histologic characteristics of maturation were also observed. Ultrastructural studies revealed an increase in number of microvilli supranuclear vaculoes with secretory content and well developed rough endoplasmic reticulum. These changes were more easily detected in young animals treated with chorionic gonadotrophin. The significance of these findings is discussed in relation to the presence of specific carriers (cytosols).
