Transcription factors as targets of the anti-inflammatory treatment. A cell culture study with extracts from some Mediterranean diet plants.

During the inflammatory response at least 2 transcription factors, NF-kappaB and AP-1, are involved in the altered profile of gene expression. We used human hepatoma (HepG2) and human umbilical vein endothelial cells (HUVEC) as a model system: NF-kappaB and AP-1 were activated by the proinflammatory cytokine IL-1 in the absence or presence of 21 selected plant extracts and the effect was evaluated by the electrophoretic mobility shift assay (EMSA). In both types of cells activation of NF-kappaB by IL-1 was significantly inhibited by extracts from Scandix australis and Artemisia alba, whereas extracts from Amaranthus sp., Eryngium campestre, Thymus pulegioides and Reichardia picroides elicited cell-type dependent response. The IL-1-induced AP-1 activation was diminished by extracts from Scandix australis, Amaranthus sp. and Artemisia alba more potently in HUVEC, while extracts from Urospermum picroides and Scandix pecten-veneris in HepG2 cells. Inhibitory activities of plant extracts towards cytokine activated NF-kappaB and AP-1 depend to some extent on the order of addition of IL-1 and plant extract to the cell culture, but the mechanism of action of extract components is not clear: although plant polyphenols may participate they are unlikely to be the only mediators, and MAP kinases were found generally not involved in down-regulation of transcription factors activity by plant extracts.

Thymoma--the usefulness of some prognostic factors for diagnosis and surgical treatment.

AIMS: The aim of the study was to identify prognostic factors which could help evaluate both the treatment offered to patients with thymoma and late results. METHODS: Forty patients were treated for mediastinal thymoma. The patients were staged clinico-pathologically (according to Masaoka) on the basis of the retrospective analysis of their operation protocols as follows: seven (17.5%)-stage I, 19 (22. 5%)-stage II, 17 (42.5%)-stage III, seven (17.5%)-stage IV. Analysis of DNA contents in cell nuclei of 23 thymomas was performed by the flow cytofluorometric method. RESULTS: From the whole group of patients, 65% survived for 5 years, 55% survived for 10 years and 43% survived for 15 years. We noted significant differences in survival time between stage I and stage IV (P<0.0012); stage II and stage IV (P<0.0006), as well as between stage III and stage IV (P<0. 005). Significantly worse prognosis was observed in the case of cortical thymomas as compared with medullary or mixed types (P<0. 0001 P<0.002). Analysis of DNA content showed signficantly higher probability of survival for the patients who had DI=1.0 (diploid), as compared with DNA >1.0 (aneuploid) (P<0.006). Of the 11 patients with diploid tumours, 91% survived for 5 years, but of the 12 aneuploid, only 23% survived. CONCLUSION: The most important positive prognostic factors influencing survival rate in patients with thymoma are: lower stage, medullary type (according to Muller-Hermelink classification), possibility of performing complete resection, diploidal nature of the tumour. Multivariate analysis of survival revealed clinico-pathological stage (according to Masaoka) and histological type (according to Salyer) as significantly independent prognostic factors.

[Catamenial pneumothorax during menstruation].

Recurring pneumothorax associated with menstruation was named catamenial pneumothorax. Proposed causes for catamenial pneumothorax include allows air to enter the peritoneal cavity via the genital tract, prostaglandins, diaphragmatic fenestrations and systemic endometriosis. In this article, we review the literature experience with this entity and critically assess treatment options based on pathophysiology mechanism.

[Lung function after recovery from empyema treated by closed chest tube drainage].

In 32 patients 8 years after recovery from unilateral empyema treated by chest tube drainage the pulmonary function was studied. The most frequently occurring abnormalities were diminished dynamic lung compliance, diffusion capacity and perfusion evaluated by scintigraphy. Restrictive patterns were observed only in 2 patients. The greatest abnormalities were observed in patients with extensive empyema, in patients with delay in drainage and with residual radiological changes after recovery.

[Spontaneous esophageal perforation--Boerhaave's syndrome].

Spontaneous perforation of the esophagus (Boerhaave's syndrome) is an emergency that requires early diagnosis if death or serious prolonged illness is to be averted. The literature is reviewed and the causes of delay in diagnosis are analyzed.

Effect of pargyline on brain N-tele-methylhistamine in portocaval-shunted rats: relation to amine neurotransmitters.

HPLC determination of histamine, serotonin, dopamine, and noradrenaline in the brain tissue of rats with portocaval anastomoses (PCA) has revealed a selective increase in histamine concentration. In the posterior hypothalamus, the steady-state level of the amine metabolites showed an inverse pattern; N-tele-methylhistamine (t-MeHA), as estimated by gas chromatography-mass spectrometry, was not changed significantly by portocaval shunting, whereas 5-hydroxyindoleacetic acid (5-HIAA) and homovanillic acid were more than doubled. Interestingly, the net increase in t-MeHA concentration in response to pargyline (80 mg/kg i.p.) was almost the same for PCA and sham-operated rats. This implies that the great enhancement of the histamine level in this area might be a consequence of the persistent stimulation of its synthesis and the unchanged activity of histaminergic neurons. In the rest of the brain, on the other hand, the steady-state level of t-MeHA was higher after PCA (3.8-fold), as were the levels of 5-HIAA and homovanillic acid. Surprisingly, t-MeHA remained unchanged after monoamine oxidase blockade. Of the pargyline-induced alterations in the concentrations of indoles and catechols, the most pronounced were those in the serotonin level; serotonin was elevated more than twofold in hypothalamus and more than 12-fold in the rest of the brain, with a concomitant 80% decrease in 5-HIAA. The dopamine and, to a much smaller extent, noradrenaline levels were also increased, and the levels of homovanillic acid and 3,4-dihydroxyphenylacetic acid fell below the detection limit. The study suggests that at least two different mechanisms operate in the brains of PCA rats to counteract the excessive synthesis of neuromediators, e.g., increased deposition and increased metabolism.

Liver regeneration in rats after complete and partial occlusion of the portal blood influx.

The role of portal blood influx in liver regeneration was studied in rats. Partial hepatectomy with removal of 45% of the liver mass was performed after end-to-side portacaval shunt (PCS) leading to complete diversion of portal blood from the liver, or after side-to-side PCS causing partial portal blood deprivation. Liver resection was limited to 45% to avoid the high mortality rate in rats with vascular anastomoses and 70% hepatectomy, but it did not change the pattern of liver regeneration. The total RNA and DNA content, the rate of DNA synthesis and the number of hepatocyte mitoses in regenerating liver were measured in comparison to sham-operated controls. Complete occlusion of the portal blood influx did not block hepatoproliferative response, but caused a significant decrease and delay of regeneration. Partial preservation of portal flow in rats with side-to-side PCS markedly improved liver regeneration in comparison to end-to-side PCS, but the process was slower than in the control group.

The effect of "end to side" portacaval anastomosis on regeneration ability of white rat liver.

The study evaluated the effect of portacaval anastomosis on the regeneration ability of the liver after partial hepatectomy in rat. The regeneration of liver was evaluated using the following tests: deoxyribonucleic acid synthesis, the activity of tyrosine aminotransferase (TAT) and the concentration of DNA and RNA in subcellular fractions of liver obtained by differential centrifugation. The results indicate the harmful effect of the portacaval shunting on the liver proliferation, which was near 50% decreased and the peak of the regeneration was delayed for about 12 hours. The most significant changes were observed in the nuclear and cytoplasmic fraction. In addition, high mortality ratio was found in the animals with portacaval anastomosis followed by partial hepatectomy.

[Experimental studies of the pathomechanism of portal encephalopathy. I. Changes in monoamine oxidase (MAO) activity in the cerebral cortex and cerebellum of rats after portacaval shunt].

Rats with portacaval shunt (PCS) were used as a model of hepatic encephalopathy and compared to sham operated controls. Neurochemical studies in rats have demonstrated increased activity of MAO in brain cortex and cerebellum at 20, 40, and 60 days after surgery. The greatest increase of MAO activity was seen 60 days after PCS.

[Experimental studies of the pathomechanism of portal encephalopathy. II. Monoamine oxidase (MAO) activity in the rat brain after porta- caval shunt using 2 different methods].

The authors present effect of the end to side portacaval shunt (E-S PCS) and side to side portacaval shunt (S-S PCS) on monoamine oxidase (MAO) activity in brain cortex and cerebellum of rats. There were significant differences between two experimental models of PCS. Our results show greater increase in MAO activity after end to side portacaval shunt.
