Elution of fibronectin proteolytic fragments from a hydroxyapatite chromatography column. A simple procedure for the purification of fibronectin domains.

Human plasma fibronectin is composed of at least five distinct domains which we refer to as Hep-1/Fib-1, Gel, Cell, Hep-2 and Fib-2 depending on their affinity for heparin (Hep), gelatin (Gel), the cell surface (Cell) or fibrin (Fib). These domains are aligned from the NH2 to the COOH terminus in the above order and can be separated from each other by mild proteolytic digestion. We have studied the elution of fibronectin thermolysin digest from a hydroxyapatite column using a linear gradient (0.5-190 mM) of sodium phosphate buffer. The five major fibronectin domains were eluted from the hydroxyapatite chromatography column in the following order: Gel, Fib-2, Cell, Hep-1/Fib-1 and Hep-2. They were identified on the basis of their molecular mass, affinity to different macromolecules and reaction with domain-specific monoclonal antibodies. All domains except the Cell and Hep-2 domains eluted as single homogeneous peaks. The Cell domain eluted as two different peaks and the Hep-2 domain eluted as four different peaks. This is the first time that heterogeneity of these two domains has been observed. Since chromatography of a fibronectin thermolysin digest on a hydroxyapatite column provides a good separation of the five major fibronectin domains, we have elaborated a procedure in which each fibronectin domain is purified by no more than two steps; hydroxyapatite and molecular exclusion chromatography. Fractionation of fibronectin proteolytic digest on a hydroxyapatite chromatography column should be of great value in the comparative analysis of fibronectin from different sources and in the study of fibronectin heterogeneity. Its use in combination with molecular exclusion chromatography offers a simple and high-yield method for the purification of large amounts of fibronectin domains.

Dependence of the shape of the plasma fibronectin molecule on solvent composition. Ionic strength and glycerol content.

Fibronectin has been purified to apparent homogeneity according to measurements of molecular weight and diffusion constant from light scattering and sedimentation in the analytical ultracentrifuge. This gives two estimates of molecular weight close to 500,000. (The difference with the gel electrophoretic value of 440,000 may indicate the presence of some material of molecular weight higher than that of the monomer.) The (corrected) diffusion constant is found to decrease with ionic strength, much more rapidly in 30% glycerol than in water. A corresponding change occurs when the glycerol content is varied from 0 to 30% at moderate ionic strength, but at very low ionic strength the diffusion constant does not depend on glycerol content. It is concluded that fibronectin can occur in two extreme conformations: the open form, in 30% glycerol at moderate ionic strength, should correspond to the extended shapes visualized by others by electron microscopy of samples prepared from solutions containing glycerol, while the closed form occurs under more physiological conditions. The Stokes radii of these forms, 14.5 and 9.6 nm, respectively, have been compared with calculated Stokes radii of chains of beads of overall length 140 nm having varying stiffness, simulated by a Monte Carlo procedure. These have a Stokes radius of 14.5 nm when made relatively extended to where the distance between chain ends averages 90 nm, as observed in the electron micrographs. When these chains are made very flexible, the Stokes radius reaches a limit of about 10 nm, apparently a lower limit to which a randomly coiled molecule of this size and volume may collapse in a disordered fashion. These results and electron micrographs obtained at low ionic strength suggest a tangled model of the compact form, rather than the ordered model recently proposed by others. Finally, some speculation is offered in regard to a possible physiological role of a conformation change of fibronectin.

Progressive teleangiomatous osteolysis. Case report.

Progressive teleangiomatous osteolysis (P.T.O.) is a very rare disease, particularly when one recalls that only five cases have been recorded at the Rizzoli Institute (Campanacci, 1981). The aetiopathogenesis is unknown and according to Campanacci it is not clear whether it is attributable to an angiomatous condition or to a post-traumatic dystrophy. A typical case is presented, localised in the thoracic cage and complicated by severe haemothorax. The patient was observed over a period of 17 years and made a complete recovery.

Does hyperbaric oxygen (HBO) delivery rescue retinal photoreceptors in retinitis pigmentosa?

As previously reported in the literature, hyperbaric oxygen delivery seems to modify the natural course of retinitis pigmentosa. In order to evaluate these first encouraging data, 48 affected subjects were separately studied in two subgroups (cases and controls). All patients underwent yearly an ophthalmological examination completed by a maximum amplitude electroretinogram, conducted according to our 'differential derivation' system, a new recording technique specifically designed to enhance the signal-to-noise ratio. Oxygen delivery was provided regularly for 90 min daily (2.2 Absolute Atmosphere) in three cycles according to a standard protocol. In the cases, electroretinographic mean values were as follows: at T0 (basal) 4.68 +/- 3.81 microV; after one year (T1) 8.46 +/- 5.71 microV; at two years (T2) 10.7 +/- 7.6 microV; at the end of the study (T3) 14.4 +/- 11.7 microV. In the controls, electroretinographic mean values were as follows: at T0 4.92 +/- 3.05 microV; at T1 5.04 +/- 3.07 microV; at T2 3.46 +/- 2.77 microV: at T3 2.97 +/- 3.61 microV. Amplitudes showed a remarkable (p<0.001) increase in the cases, while a slightly significant (p<0.02) decrease was evident at the end of the study in the controls. In our opinion, retinal oxygen availability may be critical in retinal degeneration and hyperbaric oxygen delivery, inducing hyperoxia, seems to be able to bring about the rescue of the retinal photoreceptors helping them in their metabolic requirements. Unfortunately, our study demonstrates an increase in electroretinographic responses only, which may not necessarily also mean an evident change in visual acuity.

Non invasive ventilation in the treatment of FUdr-induced lesional pulmonary oedema.

2-deoxy-5-fluorouridine, also known as floxuridine (FUdr) is a fluoropyridine antimetabolite, used in the treatment of metastatic renal cell carcinoma. We report the first case of lesional pulmonary oedema developed after receiving Fudr, recently treated in our unit. The patient refused endotracheal intubation, and was successfully treated associating noninvasive ventilation (NIV) with full-face mask to steroid treatment. The authors conclude that mechanical ventilation via face-mask can be an effective, comfortable, dignified method of support for patients with end-stage disease and acute respiratory failure.

[Atheroembolism: a form of systemic vascular disease].

Atheroembolism, a systemic vascular disease. Cholesterol crystal embolization (CCE) is an infrequent but serious disorder that is often an unrecognized medical problem. CCE may occur spontaneously from eroded atherosclerotic plaques or most frequently following procedures such as angiography, angioplasty, cardiac catheterization, anticoagulant therapy and aortic surgery. CCE predominantly affects elderly males with a frequent history of hypertension, atherosclerotic vascular diseases and renal insufficiency. CCE may result in protean clinical manifestations and may produce a spectrum of functional impairment. Confusion over the disease's natural history arises because the difficulty of establishing an antemortem diagnosis, and because the laboratory findings are non-diagnostic and non-specific. The mortality was 81% and the causes of death was most often due to multiorgan failure especially renal involvement. The definitive diagnosis depends upon finding the presence of intravascular cholesterol crystal in biopsy or autopsy specimens. The skin, muscle and kidney were the three most common sites for obtaining a premortem diagnostic biopsy.

[Does the analysis of DNA in myocytes have meaning?].

It seems that polyploidisation of the heart muscle nuclei is a physiological phenomenon that increases in pathological conditions such as muscolar dystrophy, myocardial hypertrophy. We studied the nuclear DNA content of myocites, by means of cytofluorimetric determinations in different sites of the left free ventricular wall in five hearts affected by dilatative myocardiopathy. In all the different sites examined we revealed diploid and tetraploid nuclear content. Variable was the presence of 8N poliploid peaks: specifically 40% in the external third, 47% in the medium third and 73% in the inner third. 16N polyploid peaks were revealed only twice, and exactly in one medium third and in one inner third. These data, even if quite preliminary, suggest the presence of an increasing gradient of ploydisation from the external toward the inner part of the left ventricular wall.

[Quantitative analysis of DNA using flow cytometry and immunocytochemical findings in 16 cases of cardiac myxomas].

Recently some Authors focused nosological and biological aspects of the cardiac mixomas. Their neoplastic nature and origin from undifferentiated mesencymal cell with multipotential capability have been confirmed. Morphological and immunocytochemical aspects alone are not able to screen "sporadic" from "non-sporadic" mixomas. The last are associated with local recurrence and distant embolization. "Non-sporadic" mixomas are also part of a heritable syndrome involving skin, endocrine tumors and lentiginosis. It seems that this kind of mixomas display a DNA tetrapoid pattern at DNA flow cytometry. In the series of the Authors, consisting of 16 operative mixomas, all but one showed a DNA diploid pattern. Only in one case the pattern was tetraploid.

[Histological, ultrastructural and clinical study of a malignant adrenal neoplasm producing minor mineralocorticoids].

The authors describe a case of an adrenal tumor of unusual dimensions with a benign histological appearance. After a short period diffuse metastases developed. The metastases should the features a poorly differentiated carcinoma. The size of the tumor appears to be a useful element of judgement in differential diagnosis between adenoma and carcinoma of the adrenal glands.

[Lobular carcinoma of the breast with neuroendocrine differentiation. Study of a case].

A breast tumor with morphologic features of neuroendocrine differentiation and containing large amounts of estrogen receptor protein was associated with areas of typical infiltrating lobular carcinoma. The neuroendocrine areas were argyrophilic, NSE positive and ultrastructurally did not show dense granules. Results of flow cytometric analysis revealed aneuploid nuclear DNA.

Serial determination of platinum, protein content and free sulfhydryl levels in plasma of patients treated with cisplatin or carboplatin.

We report here the results of serial determinations of free-sulfhydryl, protein content and platinum levels in plasma of cancer patients following either cisplatin (cis-DDP, 3 patients, 60 mg/m2) or carboplatin (JM-8, 3 patients, 300 mg/m2) i.v. administration. After treatment with cis-DDP, significant platinum binding to plasma components with MW greater than 25 kD was observed; the ratio free-sulfhydryls/protein content decreased during the first two hours, returning to normal values at 24 hours after injection. In contrast, no evidence of platinum binding to plasma components with MW greater than 25 kD was noted after JM-8 i.v. administration, and the ratio free sulfhydryls/protein content did not change significantly after treatment. In vitro experiments show that at a molar ratio cis-DDP:JM-8 1:10, the two compounds bind to low MW thiols with the same kinetics. These data seems to suggest different interactions at the plasma level of these drugs, which may be correlated with their different toxicity.
