Total footplate extraction in stapedectomy.

The concept of total footplate removal in stapedectomy surgery is recommended as the preferred technique providing that the surgery is atraumatic. Only that part of the footplate which can be removed easily should be removed; in 4,761 consecutive stapedectomies a total footplate removal could be accomplished in 74%. Total and partial footplate removal necessitates a connective tissue graft to seal the oval window and must be coordinated with a suitable prosthesis which is efficient, functional and self-centering. The successful results between total and partial footplate removal are not statistically significant when employing the criteria of air-bone gap closure to within 10 dB, but when comparing the complete air-bone gap closure rate there is a statistically significant difference with total footplate removal providing a more favorable hearing result.

Juvenile otosclerosis. A 20-year study.

An in-depth study of 610 patients who developed clinical otosclerosis before the age of 18 is reported; this is 15.1% of the total number of stapedectomy cases performed within the period 1961 to 1981. The study illustrates differences noted in those patients who had surgery performed before age 18 and those whose surgery was performed after age 18. These differences will be analyzed with respect to sex, family history, bilaterality, preoperative cochlear reserve, stapes footplate pathology, type of footplate surgery, and postoperative hearing results, as well as the follow-up for delayed complications and delayed hearing losses. Statistical data supports the hypothesis that there is no contraindication to performing stapedectomy surgery in children. This study demonstrates that the success rate is significantly higher in the under-age-18 group and correlates closely with the less severe footplate pathology encountered in children.

The Robinson stapes prosthesis: a 15-year study.

A report on the stapedectomy procedure using the Robinson stainless steel stapes prosthesis over a 15-year period is presented in a study of 3,200 cases. Short-term results, long-term results, failures, and delayed complications are statistically analyzed. This implant is presently the most frequently used prefabricated stapes prosthesis in the country, and each year more otologic surgeons are adopting this surgical technique.

Australian snowboard injury data base study. A four-year prospective study.

Information on the rate and spectrum of snowboarding injuries is limited. This 4-year prospective study at 3 major Australian ski resorts assesses incidence and patterns of snowboarding injuries, particularly in relation to skill level and footwear. Ski injury data were collected for the same period. In a predominantly male study population (men:women, 3:1), 276 snowboarding injuries were reported; 58% occurred in novices. Fifty-seven percent of injuries were in the lower limbs, 30% in the upper limbs. The most common injuries were sprains (53%), fractures (24%), and contusions (12%). Comparing skiers' versus snowboarders' injuries, snowboarders had 2.4 times as many fractures, particularly to the upper limbs (21% versus 35% of upper limb injuries), fewer knee injuries (23% versus 44% of lower limb injuries), but more ankle injuries (23% versus 6% of lower limb injuries). Ankle injuries were more common with soft-shell boots, worn most by intermediate and advanced riders. Knee injuries and distal tibial fractures were more common with hard-shell boots, worn most by novices. Overall, novices had more upper limb fractures and knee injuries; intermediate and advanced riders had more ankle injuries. Falls were the principal mode of injury. To prevent injury, beginners should use "hybrid" or soft-shell boots and take lessons.

Linking mortality data with hospital morbidity data--a pilot study.

Record linkage is becoming an increasingly important technique in moni toring health outcomes. In this study we wought to examine the feasibility of linking mortality data held by the Registry of Birth, Deaths and Marriages with the Patient Master Index (PMI) at St. Vincent's Hospital, Melbourne, in order to document deaths soon after hospital discharge. Linkage was carried out using demographic details. Registry data required extensive editing and formatting prior to being matched with the PMI. 86.2% of cases identified by the computer algorithm were actual cases. Mortality data are an important source of information on health outcome but a process is needed to make them more usable. We have described one such method and suggested where the process could have been streamlined. The use of unique identifiers would have facilitated the matching process, curtailing several processing steps.

The effects of subacute and subchronic oral exposure to cis-1,2-dichloroethylene in Sprague-Dawley rats.

Cis-1,2-dichloroethylene was administered daily by corn oil gavage to male and female Sprague-Dawley rats at the following dose levels: 1.0, 3.0, 10.0 and 22.0 mmol/kg/day for 14 days. Doses gavaged during the 90-day subchronic study were 0.33, 1.00, 3.00 and 9.00 mmol/kg/day. There were no compound-related deaths or histopathological changes demonstrated. Significant increases in relative liver weights were seen after 14- and 90-days of treatment in both sexes. This study demonstrates some indication of toxicity at subacute and subchronic exposure levels as low as 0.33 mmol/kg/day. Implications of liver abnormalities were demonstrated at an exposure level of 1 mmol/kg/day while kidney abnormalities (relative weights) were demonstrated at an exposure level of 0.33 mmol/kg/day.

The effects of subchronic chlorate exposure in Sprague-Dawley rats.

Male and female Sprague-Dawley rats were exposed to drinking water containing 3.0, 12.0 or 48.0 mM sodium chlorate. The mean drinking water consumption varied between exposure groups from 100-200 ml/kg/day. Female exposure groups consistently drank more water (23-42%) than male exposure groups thereby receiving more chlorate/kg/day at every exposure level. There were no compound related deaths; however, both males and females in the high exposure groups had significant weight loss during the 90-day exposure period. Also, in these same groups females had mild but significant decreases in the following relative organ weights; adrenals, thymus and spleen, while the relative brain weight was increased. In males, the heart, kidneys and liver were mildly decreased while the brain and testes were mildly increased. Red blood cell counts and percent hematocrit were decreased in both sexes in the high dose group. Pituitary gland (pars distalis) vacuolization and thyroid gland colloid depletion were prominent in both sexes in mid and/or high dose animals. A NOAEL of 0.36 mM chlorate/kg b.w./day in males and 0.50 mM chlorate/kg b.w./day in females were established.

Toxicity studies of 1,3-dichlorobenzene in Sprague-Dawley rats.

Male and female Sprague-Dawley rats received 1,3-dichlorobenzene daily by corn oil gavage for 10 or 90 consecutive days. The 10-day study doses were 0, 37, 147, 368 and 735 mg/kg; the 90-day study doses were 0, 9, 37, 147 and 588 mg/kg. In the 10-day study, there was a significant depression of body weight in both sexes at 735 mg/kg. Liver weights were significantly increased in both sexes at 368 and 735 mg/kg. Serum cholesterol levels were significantly elevated in both sexes at 368 and 735 mg/kg. Histopathological evaluation revealed centrolobular hepatocellular degeneration at 368 mg/kg in males and 735 mg/kg in females. In the 90-day study, body weights were significantly depressed in both sexes at 588 mg/kg. Normalization of food and water consumption by final body weight indicated that at 588 mg/kg both sexes had increased food and water consumption relative to controls. Absolute and relative liver weights were significantly increased in both sexes at 147 and 588 mg/kg. Relative kidney weights were significantly higher in both sexes at 588 mg/kg and in males at 147 mg/kg. Serum cholesterol and calcium levels were significantly elevated over controls in females at 37, 147, and 588 mg/kg, and in males at all dose levels. Histopathological evaluation at 147 and/or 588 mg/kg demonstrated liver and thyroid lesions in both sexes, and pituitary and kidney lesions in males. A NOAEL was not firmly established.

Iss from a virulent avian Escherichia coli.

No single characteristic of virulent avian Escherichia coli has been identified that can be exploited in colibacillosis detection protocols. Research in our lab suggests a strong association between the presence of an iss DNA sequence with an isolate's disease-causing ability. The study presented here focuses on the techniques used in the expression, purification, and characterization of avian E. coli Iss protein. In brief, iss was cloned into an expression vector, the construct was transformed into a protease-deficient E. coli, and expression was induced. The protein was expressed as a glutathione-S-transferase (GST) fusion and purified by affinity chromatography. The GST portion was cleaved from Iss, Iss was harvested by affinity chromatography, and the identity of Iss was confirmed by N-terminal sequencing. Currently, purified Iss is being used to prepare hybridomas for production of monoclonal antibodies with the goal of evaluating anti-Iss as a reagent for the detection of virulent avian E. coli.
