Role of percutaneous fine needle aspiration in the diagnosis of lung cancer: our experience with 140 patients.

The authors have performed percutaneous fine needle aspiration (FNA) biopsy in 140 patients with pulmonary lesions of suspected neoplastic nature. In 63% of the cases histologic diagnosis could be made on the bioptic sample. Comparison of these findings with those obtained with conventional exfoliative cytology suggest that the latter technique gives higher percentage of histological characterization, it is free of complications and easier to perform. Thus, FNA appears to be an additional, irreplaceable tool when conventional methods fail to provide clear-cut information regarding the histologic nature of pulmonary lesions.

[Molecular structure with potential inhibitory action on the biosynthesis of FAH4].

Molecules formed by two pharmacophoric synthons--4-H2N--C6H4--SO2NH--and 2,4-diaminopyrimidine--were prepared. These structural units, with sulphamidic and antifolic action respectively, are bound without mutual electronic interaction, as demonstrated by their experimental electronic indices. The compounds were tested for activity on several bacterial strains and their biological activity was compared with that of molecular combinations formed by 4-aminobenzensulphonamide and substituted 2,4-diaminopyrimidines, to discover whether these two pharmacophoric synthons, so bound, are synergic.

[Structure-activity relationships in N1-acetylderivatives of sulpha drugs].

The N1-acetylderivatives (ASA) of sulphanylamides (SA) were prepared and their stability to hydrolysis was evaluated together with the spectroscopic and antibacteric activity parameters, with the aim both of obtaining electronic structure-activity relationships and of comparing these results with those previously found for SA. From our results it appears that: a) the stability to hydrolysis is dramatically reduced on passing from the aqueous medium (pH 7) to the culture broth, and that b) in general, the ASA examined do not show any greater antibacterial activity with respect to the parent compounds, as suggested by the values of the spectroscopic indices-taken as experimental electronic indices. This result is in full agreement with the structure-activity relationships previously proposed fo the sulpha drugs.

Choroidal lacunae and Aicardi's syndrome.

Three children with flexion spasms and an ophthalmologic picture characterised by choroidal lacunae are described. Aicardi's syndrome was diagnosed in two cases. A diagnosis of Aicardi's syndrome should include CT scan and/or pneumoencephalographic findings of partial or total agenesis of the corpus callosum and/or other anomalies of the interhemispheric structures. Choroidal lacunae are not entirely pathognomonic.

[Breast metastasis as the initial clinical symptom of renal adenocarcinoma].

Renal adenocarcinoma shows an unpredictable clinical course. At autopsy, metastases are present in as many as 95 percent of patients with renal adenocarcinoma. The percentage of metastases from renal adenocarcinoma is greater than would be expected from the frequency of this carcinoma among all carcinomas. The breast is an exceptional site of metastasis, that is not described in literature. In the present report the metastasis in the breast represented the early symptom of renal adenocarcinoma.

Immunologic monitoring during combined radiochemoimmunotherapy in inoperable lung cancer.

49 patients with advanced lung cancer underwent a combined treatment including radiochemoimmunotherapy. Before and at the end of the combined treatment, the immune status of the patients has been evaluated by testing the immunoreactivity to PPD in vivo and the phytohemagglutinin lymphocyte response and the rosette-forming cells in vitro. While the in vitro tests were not significantly modified by immunostimulation, as regards the immunoreactivity to PPD, we observed a positive conversion at the end of the treatment in 11 out of 25 patients who showed a negative reaction before therapy. All the patients who converted from negative to positive showed also a good response to therapy with regression or no progression of the tumor. Survival curves confirmed the prognostic value of PPD reactivity.

[Iodo-derivatives of 2-aminobenzensulfonamide and 1,2,4-benzothiadiazine-1,1-dioxide].

A series of iodo derivatives of 2-aminobenzensulfonamide and its derivatives has been prepared by reaction with iodine chloride in N,N-dimethylformamide or N,N-dimethylacetamide. The structure of each compound was confirmed by transformation with formic acid into the corresponding 1,2,4-benzothiadiazine and comparison with the same compounds prepared via the Sandmeyer reaction starting form amino derivatives of known structure. It was found that the iodination reaction in N,N-dimethylformamide, can lead, depending on the nature of the starting material, to the formation of benzothiadiazine derivatives apparently generated from sulfonamide via a formylation reaction due to N,N-dimethylformamide.

[Geographical differences in the incidence of Australia antigen in chronic liver diseases].

In order to evaluate the role of the Australia Antigen and of the many other factors commonly invoked in the etiology of chronic liver diseases a series of study have been performed by radioimmunoassay on: a group of blood donors who showed persistent antigenemia and two groups of patients with chronic hepatitis who were studied respectively at Brescia General Hospital and at the Departement of Internal Medicine of the University of Naples. The results were as it follows: 1) Liver damage, from mild to severe (from transient increase of GOT and GPT levels to cirrhosis) was present in 69 out of 145 blood donors with persistent antigenemia. 2) Antigenemia was more frequent in the neapolitan group of patients not only when considering the entire study population (39%) but also when the cirrhotic group was considered (40.7%). In the Brescia study group the figures were 11.7% and 8.6% respectively. 3) Comparable high incidence of antigenemia was present in both groups when only patients with chronic aggressive hepatitis and liver carcinoma were considered. 4) When only patients with chronic persistent hepatitis and chronic aggressive hepatitis were considered the incidence of antigenemia was remarkably different.

[Influence of cholestasis on enzymatic induction with phenobarbital and other enzymatic aspects: experimental studies on guinea pigs].

Investigations in guinea pigs to evaluate interferences between cholestasis and enzymatic induction by phenobarbital, and influences that both experimental conditions exert on the levels of gamma-glutamyl transpeptidase in microsomes and other subcellular fractions, have shown that: 1) cholestasis inhibits inductions of cytochrome P-450 from phenobarbital; 2) gamma-glutamyl transpeptidase is not an inducible enzyme in guinea pig; 3) rises or ferum gamma-glutanyl transpeptidase produced either by cholestasis or phenobarbital administration, are not related to an increased enzymatic synthesis in the various subcellular fractions of liver cell.
