Switzerland's participation in MONICA.

Switzerland is currently participating in the multicenter study "Monitoring of Trends and Determinants in Cardiovascular Disease - MONICA". Incidence of acute myocardial infarction will be studied over a period of ten years in a target population. The results will be compared with changes in medical care and the spread of known risk factors for cardiovascular disease in the population. This paper describes how the different data will be registered in Switzerland and how far the work has progressed up to now.

Hepatitis B vaccination campaign in a low endemicity area.

Between January 1982 and December 1983 14,666 high-risk individuals in the Canton of Zurich, which has one million inhabitants, received 36,234 hepatitis B vaccine injections. The annual number of acute hepatitis B cases dropped from an estimated 220 to 280 in 1981 to 177 in 1982 and 133 in 1983. This drop of 40-50% resulted mainly from reduced numbers of cases among health care workers, drug addicts and homosexuals. Vaccination was a factor responsible for the reduced incidence of hepatitis B among health care workers and drug addicts, high-risk categories substantial proportions of which had been vaccinated. There was no evidence of a secondary protective effect of vaccination extending to other high-risk groups.

Hepatitis B vaccination of high-risk individuals in the canton of Zurich.

In an effort to vaccinate as many high-risk individuals as possible against hepatitis B, 10,484 persons received a total of 27,818 injections of H-B-Vax (Merck Sharp & Dohme) or HEVAC B (Sanofi/Pasteur) between January 1982 and January 1983. They represented 1.1% of the total population of the canton of Zurich including approximately 70% of all medical and dental personnel, a great proportion of eligible high-risk patients and contacts of HBs Ag carriers, but not more than 10% of drug addicts and promiscuous homosexuals. Twelve to 17% of the vaccine injections were accompanied by side effects which were benign in nature and transitory. Six vaccinated individuals developed hepatitis B, all within 60 days after the first vaccine dose. One hundred and seventy-seven cases of hepatitis B were recorded in 1982, 109 in the first half of the year and 68 in the second half. It was concluded that the vaccination campaign provided protection for medical and dental personnel. There was still no conclusive evidence of a vaccine-related drop in the incidence of hepatitis B in other high-risk groups vaccinated or - as a secondary effect - in other non-vaccinated populations.

Trends in cardiovascular risk factors (1984-1993) in a Swiss region: results of three population surveys.

BACKGROUND: This study attempted to assess the time trends in lifestyle and cardiovascular risk factors in the Swiss region of Vaud-Fribourg (population 784,000). METHODS: Three surveys (1984/1985, 1988/1989, and 1992/1993), based on independent representative samples (n = 3,300) of the population ages 25 to 74, were conducted within the framework of the international WHO-MONICA Project. RESULTS: The most favorable changes were observed in reported behaviors: increased physical activity in leisure time, healthier dietary habits (switch from unskimmed milk, butter, and meat to skimmed milk, margarine, and fish, with no change for fruits and vegetables), and lower prevalence of regular smoking among men (from 32 to 28%). Body mass index did not vary significantly, apart from an increase in the prevalence of obesity among men (from 11 to 15%). Total cholesterol varied only slightly, while the HDL cholesterol levels decreased steadily (from 1.37 to 1.19 mmol/L among men; from 1.59 to 1.51 among women). Average systolic blood pressure regressed among women (from 127.2 to 124.4 mm Hg), while the prevalence of untreated hypertension increased among older men. CONCLUSION: The self-reported changes in lifestyle were only partially reflected by favorable trends in objective measurements. Physical activity, even at moderate intensity, and consumption of fruits, vegetables, and fiber in general should be promoted.

[First tobacco consumption in children].

From 1436 pupils in North-Switzerland 9% of the boys and 3% of the girls smoked their first cigarette within or before the age of six years. 32% of these first cigarettes were smoked in woods or parks. 37% on streets, 31% on other places. 25% of the first cigarettes were stolen, not bought. 50% of all these children remembered to have a headache, vomitying or a stomach-ache after smoking the first cigarette. The boys began to smoke daily at the age of 13, the girls at the age of 14.
