This 56-year-old male patient came to our emergency ward because of a painless pulsation on his left neck. The clinical examination revealed an irregular pulse and a systolic murmur praecordially. In further diagnostic procedures the diagnosis of a severe tricuspidal insufficiency could be confirmed. In the history of the patient there was a motorcycle accident with blunt chest trauma which had to be considered as cause of the lesion.
A precondition for satisfactory (long-lasting) prosthetic results is a good occlusion. This can sometimes only be achieved with the aid of orthodontics. This article concludes that orthodontics can be carried out in patients with reduced periodontal support provided thorough periodontal treatment is carried out initially and treatment goals and biomechanics are carefully considered.
The demand by patients considered for better esthetics during treatment should be seriously considered. With the introduction of fiberglass brackets one has tried to solve that problem. The purpose of this investigation was: 1. a microscopic examination of the slots of these fiberglass brackets after various periods of wearing; 2. an in vitro investigation of the adhesion of the fiberglass brackets in comparison to metal brackets; The following results were obtained: 1. There are remarkable differences in the slot dimensions between new fiberglass brackets and those which have been in the mouth for varying periods of time. 2. Ligating wires into fiberglass brackets may be difficult because of wing fractures. 3. There are significant differences regarding the adhesion of fiberglass brackets compared with metal brackets. Results of tests carried out under defined conditions (bonding, storage, time-factor) concluded that metal brackets behaved much more favourably; the adhesion of metal brackets was nearly ten times greater than that for fiberglass brackets.
A critical period exists (approximately 72 h) following administration of sheep red blood cells (SRBC) during which inescapable footshock suppressed the peak splenic plaque forming cell (PFC) response and serum antibody titers in CD-1 mice. The immunosuppression was likewise provoked in mice that were exposed to the stressor 2 weeks prior to immunization and reexposed to stressor-related cues 72 h after inoculation. In contrast, when the stressor was initially applied immediately after immunization, reexposure to the stressor-related cues was found to provoke an immunoenhancement. Moreover, stressor exposure immediately following inoculation had the effect of counteracting the immunosuppression otherwise induced by a stressor applied at the 72-h interval. Evidently, environmental cues associated with a stressor provoke profound alterations of immunoresponsiveness, but the direction of the effect is dependent upon the time at which the stressor was initially applied.
To determine in adults and children the dose-response relationship and the duration of action of pipecuronium bromide during fentanyl-nitrous oxide (N2O), isoflurane, and halothane anesthesia, the authors studied 30 ASA Physical Status 1-2 adults (age: 16-55 yr) and 30 ASA Physical Status 1-2 children (age: 1.7-11.5 yr) during minor elective surgery. Patients were anesthetized with N2O/O2 (60:40) supplemented with either fentanyl (4 micrograms/kg), or isoflurane (adults, 0.9%; children, 1.2%), or halothane (adults, 0.6%; children, 0.7%). Neuromuscular (NM) blockade was measured by electromyography. Incremental iv doses of pipecuronium were administered to determine the cumulative dose-response relationship of pipecuronium until a 95% twitch depression (ED95) had been obtained. In adults, ED50 was 31.7 +/- 2.9 micrograms/kg (mean +/- SE) during fentanyl-N2O/O2, reduced by isoflurane (18.0 +/- 4.8 micrograms/kg, P less than 0.05) but not by halothane (25.0 +/- 2.6 micrograms/kg, NS). ED95 was 59.4 +/- 5.4 micrograms/kg during fentanyl-N2O/O2, reduced by isoflurane (42.3 +/- 2.5 micrograms/kg, P less than 0.05), but not by halothane (49.7 +/- 3.1 micrograms/kg, NS). In children, ED50 was 43.9 +/- 4.7 micrograms/kg during fentanyl-N2O/O2, reduced by isoflurane (23.1 +/- 1.6 micrograms/kg, P less than 0.05), and halothane (33.2 +/- 3.2 micrograms/kg, P less than 0.05). ED95 was 79.3 +/- 9.8 micrograms/kg during fentanyl-N2O/O2, and reduced by isoflurane (49.1 +/- 3.1 micrograms/kg, P less than 0.05), but not by halothane (62.5 +/- 7.3 micrograms/kg, NS). Comparison between adults and children reveals no statistically significant differences, except for ED50 during fentanyl-N2O/O2 anesthesia which was increased in children.(ABSTRACT TRUNCATED AT 250 WORDS)

Odontogenic myxoma and particularly juvenile fibromatosis are rare benign tumors in the maxillo-facial region. The aggressive local and sometimes voluminous development of these tumors may necessitate partial mandibulectomy. Autologous bone graft from the iliac crest is the best material for mandibular reconstruction because of its functional and esthetic results. If osteosynthesis of the graft is achieved by means of internal wiring, several weeks of intermaxillary fixation are required, whereas internal rigid fixation with plate and screws allows the patient to open the mouth more rapidly.

Poly-3-hydroxybutyric acid belongs to the biological polymers, which are produced by bacterials. The determination of the grain size, moisture content, flowability and the parameters for the direct compression was performed in regard of their use as auxillary substance for the preparation of solid sustained release dosage forms. The production of the matrix tablets was performed on the basis of a factorial design. The content of substance, an addition of Heweten 12 and the compression power served as factors. Caffeine was used as model drug. The in vitro release values show, that all three factors have an influence of the drug release. Optimized matrix tablets were produced on the basis of this result.
In connection with the preparation of solid sustained-release forms extrusion pellets were prepared. The conditions for the processing of the thermoplastics were determined. Lithium sulfate and caffeine served as model drugs. The drug release from the thermoplastics is insufficient. There was no evidence for a possible relation between the properties of the thermoplastics and the drug release. By means of selected examples a correlation between the thermoplastic capacity to water and the drug release could be found.
Five binders characterized by particle size and dissolution rate have been studied in order to evaluate their qualification for fluidized-bed granulation. For this purpose they were applied as powder and solution, respectively. Quality parameters of the granules and tablets obtained were compared. Compared with solutions of binders the dry incorporation of the binder led to a slower growth of the granules mainly depending on the dissolution rate of the binders. Due to this fact the formulations showed a decreased strength except PVP (high dissolution rate) which causes an increased growth of the granules at the same time. A size reduction of binders with a low dissolution rate led to tablets with characteristics of strength similar to the findings when solutions of binders were used, whereas the growth of granules remained uninfluenced.

For the first time, the fibrinogen preparation according to Blombäck was used to purify rat fibrinogen. Intermediate as well as final products were compared with those of human fibrinogen. Yields of final products were about 25% of plasma fibrinogen. Thrombin clottability amounted to 96.5 +/- 1.3% for rat fibrinogen and 95.0 +/- 1.1% for human fibrinogen, respectively. The integrity and the characteristic differences in molecular mass of rat and human fibrinogen chains were demonstrated by means of SDS-PAGE.
Biodegradable polymers get more interest for the preparation of drug formulations. Their main advantage is their physiological safety. Polylactic acid is one of the biodegradable polymers. It was prepared by the direct polyesterification of lactic acid and by polymerisation of the cyclic oligomers. Both the polymers were identified by elementary analysis, by IR spectroscopy, by X-ray-test and by the molecular mass. They differ in their molecular mass and in the degree of the crystalline state. The determination of the grain size, moisture content, flowability and of the parameters for the direct compression were carried out with regard to the use of polylactic acid as auxiliary substance for the direct compression. The results show, that polylactic acid which was prepared on the basis of polyesterification, is a very suitable auxiliary substance for the direct compression.
It is reported on the preparation and investigation of matrix tablets on the basis of the polylactic acid. Caffeine and phenazone served as model drugs. The release values obtained were analyzed by the equations of Noyes-Whitney and of Higuchi. It was demonstrated, that the solubility and the grain size of the drugs, their content in the tablets and an addition of Heweten 12 have an influence of the release of the drugs. In in vitro-investigations can be shown a hydrolytic degradation of the polymer, but not an enzymatic degradation.
