Effect of 41-CRF antiserum on the secretion of ACTH, B-endorphin and alpha-MSH in the rat.

In order to elucidate the physiological role of the 41 amino-acid residue corticotropin-releasing factor (41-CRF) on the secretion of ACTH, B-Endorphin and alpha-MSH, plasma levels of these peptides were measured by radioimmunoassay in intact and adrenalectomized rats, two hours after the injection of either 41-CRF antiserum (CRF-AS) or normal rabbit serum for controls. The administration of CRF-AS strikingly lowered the plasma ACTH levels in both intact and adrenalectomized rats. A statistically significant reduction of plasma levels of B-Endorphin was also observed in the same rats. However, the effect of CRF-AS on B-Endorphin release was less pronounced than the effect on ACTH release. No changes in plasma alpha-MSH levels were observed after passive immunization with CRF-AS. We conclude that, in the rat, 41-CRF plays a physiological role in the regulation of ACTH and B-Endorphin secretion, but is not involved in the regulation of alpha-MSH release from the pituitary gland.

[Toxic shock syndrome due to osteomyelitis].

An 11 3/4 year old girl contracted Toxic-Shock-Syndrome following osteomyelitis. The symptoms comprised a severe fall in blood-pressure with centralisation, petechiae and haematoma particularly in the lower extremities, hyperaemia of the oral mucosa and of the throat with profuse and unstaunchable bleeding in this area within in a short period of time. In order to stabilise the blood-pressure the patient had be given approx. 15 l colloidal solution (350 ml/kg) intravenously, mainly frozen-plasma. The risk of aggravation of a pulmonary failure along the lines of ARDS particularly due to the backward flow of the interstitial deposited fluid was avoided by the use of continuous veno-venous haemofiltration (CVVH).

[Cholelithiasis in the neonatal period].

We report about a four weeks old male infant who developed jaundice with cholangitis and fever caused by cholelithiasis which could be confirmed by sonographic examination. Familiar history revealed several relatives with gallstones.

[Diagnosis of bilateral renal vein thrombosis in infancy by pulsed Doppler sonography of the renal vessels].

In a premature infant bilateral renal venous thrombosis and associated thrombosis of the inferior vena cava were diagnosed at the age of 6 weeks, using a 2-dimensional ultrasound sector scanner with integrated pulsed doppler ultrasound of the renal vessels. In contrast to 30 healthy children, who showed continuous forward flow in the renal arteries throughout systole and diastole, diastolic flow was decreased or even retrograde in renal venous thrombosis (RVT) according to the severity of thrombosis. Doppler recordings in renal veins normally showed continuous retrograde flow patterns, whereas in RVT venous flow was decreased or absent. Pulsed doppler ultrasound is a useful and noninvasive method for recording renal flow patterns, especially in renovascular disease.

Impact of measles in France.

Although measles vaccine has been licensed since 1968, immunization against measles has not met with much success in France, partly because the disease is no longer feared. The level of vaccination coverage appears to be less than 20%. Indeed, the present epidemiologic situation is similar to the natural situation in a developed country. A multicenter, retrospective hospital survey revealed that 1,157 patients with measles were hospitalized during a 30-month period in five areas (total population, 4.2 million). Analysis of the preliminary results of this survey and of available national data showed an incidence of 5.6-7.5 cases per 1,000 population; a hospitalization rate of one per 10,000; a death rate of 0.56 per 1 million; and a disability rate of 0.48 per 1 million. Encephalitis (the definition of which should be reappraised) occurred once in 2,850 cases, and subacute sclerosing panencephalitis was not rare (incidence, one case per 2.6 million people). The problem posed by measles is sufficiently grave to justify a national campaign urging the widespread vaccination of children.

Bacterial removal from inexpensive portable water treatment systems for travelers.

BACKGROUND: There are many personal portable water treatment systems for travelers on the market, including chemical agents, iodine resin purifiers and filters. However, information on the real efficacy of these systems in the field is often lacking. We have therefore estimated the capabilities of several inexpensive personal portable water treatment systems for travelers to remove bacteria in various situations of water quality, using stressed indigenous strains of bacteria. METHODS: Four chemical agents (Drinkwell chlorine, Hydroclonazone, Aquatabs, 2% iodine in ethanol), two iodine resin purifiers (the straw PentaPure Outdoor M1-E, the PentaPure Traveler purifying and filtration system) and four filters (the flexible bottle Pres2Pure, the hand-pump filters Mini Ceramic, First Need Deluxe and WalkAbout) were evaluated in triplicate using both turbid and clear water at 25 degrees C. Bacteria were counted by conventional culturing techniques, colorimetric and fluorescent assays of coliforms and Escherichia coli enzyme activities (Colilert)/Quantitray method), and viable but not culturable bacteria were assessed quantitatively by 5-cyano-2,3-dilotyl-tetrazolium staining. RESULTS: The best systems were the three hand-pump filters, Mini Ceramic, First Need Deluxe, and WalkAbout. All had a submicron filtration element that completely removed 3 log (99.9%) or more of viable bacteria, and no coliforms or E. coli were detected in the effluent. The PentaPure Traveler removed more than 99.3% of the viable bacteria. The only chemical agents that gave a bacterial inactivation of over 2 log in clear water were the Drinkwell chlorine, the Aquatabs, and the 2% iodine in ethanol. The three other devices, Hydroclonazone, Outdoor M1-E, and Pres2Pure, performed poorly, as coliforms and E. coli were detected in the treated water by the Colilert method. The chemical agents and the iodine resin straw performed poorly on raw river water; coliforms and E. coli were detected in the treated water. CONCLUSIONS: These data demonstrate the differences between the systems tested. The effectiveness of other devices on the market should also be tested, so as to help travelers and hikers select the most appropriate portable water treatment system.

[Stuporous state in an epileptic treated with sodium valproate. Diagnostic problems].

The authors report a case of stupor secondary to valproic acid, in a patient treated with this drug and phenobarbital for a complex partial form of epilepsy. The pathogeny of this rare complication is discussed. In this patient, a paradoxical epileptic reactivation is excluded, and the authors favor a metabolic mechanism: either hyperammoniemia or a perturbation in cerebral neuro-transmitter systems are proposed.
