[Rehabilitation of cervico-thoraco-brachial outlet syndromes].

The author analyses the results of the reeducation of 26 patients suffering from a syndrome of vasculo-nervous compression in cervico-thoraco-brachial leakage. The treatment involves sessions of kinesitherapy, the technology of which is described in detail; daily exercises which the patient does on his own; and a passive elevation device of the scapulatory band, using adhesive elastic bandage. This treatment does produce a most significant improvement in the arterio-venous and nervous factors as well as in the rachidian pain syndrome.

Influence of head position on dorsal neck muscle efficiency.

The aim of this study was to assess the influence of head position on dorsal neck muscle efficiency in the sagittal plane. Fifteen subjects participated. The EMG versus isometric extension moment of dorsal neck muscles was studied in neutral (with subject gazing on a horizontal plane), cervical flexed, and cervical extended positions. A vectorial construction was created by means of photographs to calculate the extension moment which balances measured pulling force and gravitational force in isometric conditions. The maximum extension was highest in neutral position. The EMG/moment relationship was non-linear. The ratio between the EMG and the generated moment differed significantly in the three positions (p < 0.01) and was lower in neutral position. These results demonstrate the influence of head position on dorsal neck muscle efficiency; muscles appeared most efficient in neutral position. Muscle length, depending on head position, is probably the main influencing factor.

Epitope peptides from interleukin-6 receptor which inhibit the growth of human myeloma cells.

A panel of monoclonal antibodies against the soluble IL-6 receptor was used to search for linear epitopes by a Pepscan analysis. Two such epitopes were found and the corresponding peptides were synthesized chemically. The peptides were active to inhibit the IL-6 dependent growth of human multiple myeloma cell line and the effect of IL-6 on growth of murine hybridoma cells. The epitope-defined, antagonist peptides reduced the transduction of the IL-6 signal which activates binding of Stat transcription factors to specific enhancers, but did not affect IL-6 binding. These effects were not seen with several other peptides from the IL-6 receptor sequence. A computer three-dimensional model of the IL-6 receptor complex was built and indicates that the antagonist peptides define one of the two possible sites of interaction between the domain-II of the IL-6 receptor molecule and that of the gp130 molecule within the hexameric receptor assembly.

Education for outpatients with rheumatic diseases. Evaluation of short-term and medium-term results.

This prospective study was conducted to evaluate the short- and medium-term impact of the outpatient education sessions that have been available at the Cochin Teaching Hospital since 1992. Each patient was asked to complete a ten-item anonymous questionnaire at the beginning of the education session (evaluation 0), immediately after the session (evaluation 1), after six months (evaluation 2) and after 12 months (evaluation 3). Mean numbers of correct answers per patient were calculated. Eight education sessions were evaluated (osteoarthritis, osteoporosis, rheumatoid arthritis, back pain, conservative treatments, surgical treatments, low-calorie diets and high-calcium diets). One hundred twenty-four patients completed the first two questionnaires, 94 (75.80%) completed the six-month questionnaire and 75 (60.5%) completed the 12-month questionnaire. Mean numbers of correct answers were as follows: 5.7 before the session, 7.3 after the session (p = 0.0001), 7.2 after six months (p = 0.0001), and 7.9 after 12 months (p = 0.0001). These results demonstrate that the education sessions significantly improved patient knowledge in the short and medium term. Their impact on quality of life is being evaluated.
