Prevention of irrigant absorption during TURP: continuous low-pressure irrigation.

Comparative studies have been carried out on bladder pressure. The pressure gradient between bladder and abdominal cavity during TURP proved to be significantly lower with continuous low-pressure irrigation than with conventional high-pressure irrigation. Absorption is reduced to a clinically insignificant rate, as shown by blood sugar determinations during TURP in 100 cases.

Contrast-enhanced, K-space-centered, breath-hold MR angiography of the renal arteries and the abdominal aorta.

The purpose of this study was to evaluate the capability of contrast-enhanced breath-hold fast imaging with steady-state precession (FISP) three-dimensional MR angiography (MRA) to detect stenotic lesions of the abdominal aorta, the renal arteries, and the iliac arteries by using a K-space-centered 20-ml gadolinium-diethylene pentaacetic acid (Gd-DTPA) bolus. Fifty patients were studied before conventional x-ray angiography. Contrast-enhanced breath-hold FISP three-dimensional MRA was applied in the coronal view, centered at the renal arteries. Twenty ml of Gd-DTPA was used in all subjects. A test bolus was applied to determine the injection time for the K-space-centered bolus injection. Of 300 segments, 284 segments were classified correctly, 11 were overestimated, and five were underestimated. Sensitivity was 98%, specificity was 96%, positive predictive value was 96%, negative predictive value was 98%, and accuracy was 97%. Of the 50 patients studied, 43 were staged correctly. No venous overlay was seen in 31 patients; partial overlay was seen in 16 patients, and venous structure overlay obscuring arterial anatomy was found in two patients. Six of nine accessory renal arteries could be identified by MRA. Intraobserver variability was .94. This study has shown the ability of contrast-enhanced breath-hold FISP three-dimensional MRA to detect and grade vascular lesions in the abdominal aorta and the renal arteries. The method may serve as a screening tool in the future.

Avoidance of DNA methylation. A virus-encoded methylase inhibitor and evidence for counterselection of methylase recognition sites in viral genomes.

The ocr+ gene of bacterial virus T7 codes for the first protein recognized to inhibit a specific group of DNA methylases. The recognition sequences of several other DNA methylases, not susceptible to Ocr inhibition, are significantly suppressed in the virus genome. The bacterial virus T3 encodes an Ado-Met hydrolase, destroying the methyl donor and causing T3 DNA to be totally unmethylated. These observations could stimulate analogous investigations into the regulation of DNA methylation patterns of eukaryotic viruses and cells. For instance, an underrepresentation of methylation sites (5'-CG) is also true for animal DNA viruses. Moreover, we were able to disclose some novel properties of DNA restriction-modification enzymes concerning the protection of DNA recognition sequences in which only one strand can be methylated (e.g., type III enzyme EcoP15) and the primary resistance of (unmethylated) DNA recognition sites towards type II restriction endonuclease EcoRII.

An open-label, noncomparative, multicenter study of ramipril in the treatment of patients with mild-to-moderate hypertension.

Ramipril, a once-a-day angiotensin-converting enzyme inhibitor, was studied in the treatment of mild-to-moderate hypertension in 240 patients. A total of 194 patients (111 women and 83 men; mean +/- SD age, 46.0 +/- 11.5 years) were considered assessable for study. After a 2-week placebo washout phase, all patients received ramipril 2.5 mg once daily for 4 weeks. At 4 weeks, blood pressure was evaluated for response to therapy. Responders to 2.5 mg were continued on the same dose; nonresponders received 5 mg once daily for another 4 weeks. Results showed that 91% of patients responded after 8 weeks of therapy, 60% to 2.5 mg and 31% to 5 mg. Ramipril was well tolerated and, because of its long half-life, can be considered a true once-a-day angiotensin-converting enzyme inhibitor for the treatment of patients with mild-to-moderate hypertension.

Roxatidine versus ranitidine in the treatment of patients with duodenal ulcer: a randomized, double-masked, multicenter study.

The comparative efficacy of roxatidine and ranitidine in the treatment of patients with acute duodenal ulcer was assessed at 4 and 6 weeks in this multicenter study. Ninety-four of 192 patients were given roxatidine in a single nightly dose of 150 mg, and 98 patients were given ranitidine in a single nightly dose of 300 mg. All the patients had endoscopically proven duodenal ulcer. Of the 171 assessable patients, ulcers were healed in 88% of the roxatidine group (73 of 83) and in 84% of the ranitidine group (74 of 88). No serious adverse events were reported in either group. We conclude that roxatidine 150 mg once daily is as effective and safe for the treatment of acute duodenal ulcer as ranitidine 300 mg once daily.

[Trisomy 4p as result of a maternal translocation t(4;8)(q11;p23)].

We report on a 21 month old boy with congenital anomalies and a trisomy of the short arm of chromosome 4 as result of a maternal translocation 4/8 with meiotic 3:1 segregation. Karyotype/phenotype correlation is compared with other reported cases and discussed in a context of nearly pure trisomy 4p.

An experimental selection system to identify bacterial cells exhibiting a new DNA host specificity.

Restriction-modification enzymes interact with DNA sequences in a highly specific manner. Mutations within the DNA binding region of the enzymes could be expected to produce enzyme variants with changed DNA sequence specificities. We developed an efficient in vivo selection system that enabled us to detect one cell coding for a restriction-modification system with a new DNA sequence specificity in a background of more than 10(6) cells with the original DNA sequence specificity.
