[Changes in the endometrium in treatment of breast carcinoma with tamoxifen].

A group of twenty postmenopausal breast cancer patients treated with tamoxifen was used for examination of the endometrial morphology, role of transvaginal sonography, and consequences of insufficient collaboration between gynaecologist and oncologist. One case of endometrial carcinoma, one case of abnormal hyperplasia, and ten endometrial polyps in asymptomatic patients support the need for routine transvaginal sonography as a screening method. Any uterine bleeding should be evaluated by hysteroscopy.

S-100 protein positive (sustentacular) cells in pulmonary carcinoid tumorlets: a quantitative study of 24 cases.

Stellate-shaped S-100 protein positive sustentacular cells (SCs) appear to correlate inversely with the degree of tumor malignancy in some neuroendocrine tumors. Therefore, the SCs have been investigated in carcinoid pulmonary tumorlets and subsequently examined quantitatively in order to provide a basis for estimation of this phenomenon in tumorlet related lesions, especially carcinoids and neuroendocrine carcinomas. Pulmonary tissue from twenty-four patients with carcinoid tumorlets was studied immunohistochemically for S-100 protein positive SCs together with glial fibrillary acidic protein, actin, desmin, vimentin and cytokeratins. Tumorlet SCs were calculated per 10,000 tumorlet elements. The presence of SCs was proven in 18 subjects (75%) of all examined cases with quantitative frequency between 13 and 196 SCs per 10,000 tumorlet elements. The histogram showed three separate clusters of cases. Cluster 2 and cluster 3 with strikingly high frequency of SCs, i.e. from 66 up to 196 SCs per 10,000 tumorlet cells, may represent the biphasic differentiation potential of tumorlet elements. The mentioned lesions could be regarded as the possible precursors of pulmonary paragangliomas or paraganglioid carcinoids.

Retroperitoneal bronchogenic cyst in a young adult.

Abdominal sonography performed in a 21-year-old man for suspected hepatopathy revealed a pathological lesion within the retroperitoneal space. Morphological examination showed an unusual mode of presentation of an asymptomatic bronchogenic cyst.

[Satellite cells in pulmonary carcinoid tumorlets and carcinoids].

Pulmonary tissue of twenty-four patients with carcinoid tumorlets and twenty-three typical carcinoid tumors was studied immunohistochemically for stellate-shaped S-100 protein positive (sustentacular) cells. Sustentacular cells (SCs) have been calculated per 10,000 tumorlet or carcinoid elements. The presence of SCs was proved in 18 subjects (75%) of all examined tumorlet cases with quantitative frequency between 13 and 196 SCs per 10,000 tumorlet cells. These elements were also found in 18 carcinoid tumors (79%) of all 23 investigated cases with quantitative frequency between 5 and 927 SCs per 10,000 carcinoid cells. The cluster analysis showed two separate clusters in both groups of lesions with strikingly high frequency of SCs, i.e., from 66 up to 196 SCs/10,000 tumorlet elements and from 138 up to 923 CSs/10.000 carcinoid cells, respectively. SCs may not be used as an indicator of biological behaviour of pulmonary neuroendocrine tumors for their a broad frequency spectrum in examined benign lesions, i.e. tumorlets and typical carcinoid tumors.

Pulmonary endocrine cells in plexogenic pulmonary arteriopathy associated with cirrhosis.

A clear association has been described between numbers of pulmonary endocrine cells and the migration and/or proliferation of myofibroblasts which is thought to underlie the vascular changes seen in plexogenic pulmonary arteriopathy due to cardiac shunts and primary pulmonary hypertension. In contrast, the pulmonary endocrine system in a subject with florid pulmonary plexogenic arteriopathy associated with cirrhosis was entirely normal, suggesting possible differences in its pathogenesis.

Unusual clinical presentation of endotracheal mesenchymoma.

This report describes an unusual case of 35 year old man with endotracheal mesenchymoma. He was treated for two years as asthmoid bronchitis or asthma without any favourable result. The diagnosis was established by x-ray tomography and the patient was referred to the surgical treatment. The tumour was removed and the trachea was reconstructed. Postoperative course was very good. The patient is doing well without recurrence many years following the operation.

[Carcinoid tumorlets and pulmonary hypoxia].

Relationship between carcinoid tumourlets and chronic pulmonary hypoxia was studied on a group of 29 patients coming to necropsy (25 cases) in the last 25 years or surgically treated (4 cases) for non-neoplastic pulmonary lesions. All tumourlets expressed the argyrophilia and were immunoreactive for neurone-specific enolase. Pulmonary hypoxia was studied according to the presence of chronic bronchitis, emphysema and microscopical signs of hypoxic arteriopathy. Cigarette smoking was an additional criterion. The results indicated a strong relationship between tumourlets and chronic pulmonary hypoxia. 79% of patients with tumourlets have had the emphysema and in 41% of examined cases with this lesion the chronic bronchitis was diagnosed. The hypoxic arteriopathy was present in 48% of subjects with emphysema and 27% of cases with bronchitis. It was found in the same number of patients with both the examined lung diseases. On the other hand, the possibility of other cause of neuroendocrine cell hyperplasia was to be considered in 17% of investigated subjects in whom no evidence for the above mentioned relationship was found.

[Mycoses of the heart].

There is an increasing frequency of heart mycoses. Authors identified 19 such cases between 1972 and 1991, 63% in the last 6 years (frequency was 0.1% in 17.661 autopsies of patients over 7 days of life). Males prevailed (17:2). Yeasts of genus Candida dominated, unusual etiology represented Candida parapsilosis, Torulopsis candida and Aspergillus restrictus. Sampling from vegetations or abscesses was a prerequisite for positive autoptic cultivation. Probable source of infection was found in 12 patients and 5 of them had an infected thrombosis of venous catheter. Mycosepsis comprised nearly equal number of endocarditides (mostly left sided) and myocarditides. Spread of endocardial infection into myocardium was rare, most myocarditides resulted from disseminated infection in immunocompromised patients. Endocarditides mainly complicated surgical interventions.

[The dynamics of the morphological changes in the tissues after the subcutaneous administration of vaccines against Q fever].

The comparative study of the dynamics of morphological changes in tissues of guinea pigs after the subcutaneous injection of chemical, live and combined vaccines against Q fever during the period from 12 hours to 90 days was made. All vaccines under study were shown to produce a pronounced local damaging effect. Two periods were tentatively discriminated in the dynamics of changes: the early phase (till 48 hours) and the late phase (days 2-90). At the early stage the most pronounced changes were registered after the injection of the combined vaccine. At the late phase the use of the chemical and combined vaccines was accompanied by the appearance of secondary hemorrhages into newly formed connective tissue. Starting from day 30, practically no deviation from the normal state of tissues were registered at the site of injection.

Development of local morphological changes after intradermal inoculation of Q-fever chemovaccine.

Development of morphological changes was studied in the site of intradermal inoculation of Q-fever chemovaccine. The chemovaccine was obtained by trichloroacetic acid extraction from Coxiella burnetii, strain Nine mile, phase I cells. The effect of two vaccine doses (0.2 mg and 1.0 mg) was compared in two groups of guinea pigs (each consisting of 21 animals). After inoculation of 1.0 mg vaccine abscesses formed in the dermis and in subcutaneous tissue along with dystrophic changes of skeletal muscles. Reparation of these lesions progressed sufficiently slowly and was not completed before 60 days after vaccine inoculation. The regeneration of skeletal muscles was of budding type. After inoculation of 0.2 mg of chemovaccine minimal lesions occurred in the subcutaneous tissue only. However, already within 48 hr this vaccine dose evoked a strong reparation response. Interesting was the finding of Kurloff cells in the haemorrhagic foci. The results suggest that the low-reactogenic chemovaccine dose of 0.2 mg might be sufficiently immunogenic after subcutaneous administration.

Megakaryocytes in rabbit pulmonary blood vessels.

Semiserial sections of lung capillaries of seven rabbits were examined for megakaryocytes. Triads of serial sections of each lobe were examined, and megakaryocyte counts per cm2 were determined. Median megakaryocyte count was determined for each lobe. The lobal values were used to calculate the was determined for each lobe. The lobal values were used to calculate the median cell-counts for each experimental animal. Values of 0.16-0.64 megakaryocytes per cm2 were established as "normal".
