[Possibilities and limitations of computerized tomography in the staging of the subdiaphragmatic spread of Hodgkin's disease].

The value of computed tomography in the assessment of subdiaphragmatic spread was studied prospectively in 72 patients with so far untreated, histologically confirmed Hodgkin's disease. In 17 patients (23.6%) computed tomography (CT) diagnosed subdiaphragmatic involvement, with no false-positive results. In 55 (76.4%) CT was within normal limits. In 30 patients exploratory laparotomy gave false-negative results in 7 (23.4%). The causes of false-negative results in CT are that lymph-nodes which were not enlarged but involved were not demonstrated and spleen involvement was not identified. Including those correctly positively diagnosed cases (17), CT had a sensitivity of 70.8% and a false-negative rate of 29.2%. It is concluded that when CT is within normal limits only exploratory laparotomy can with certainty exclude subdiaphragmatic involvement.

[Diagnosis of bone necrosis by magnetic resonance tomography. The potentials for early diagnosis].

Twenty-eight patients with necrosis in the hip joint, the femoral condyles and the talus were examined by MR tomography. This revealed various features whose frequency depended upon the various stages of the necrosis. Three patients who showed no, or only indefinite, changes on radiographs, displayed clear evidence of necrosis on MR. Staging of femoral head necrosis on the basis of MR criteria is suggested, as well as a suitable procedure for early diagnosis. Accurate localisation and knowledge of the extent of the necrosis by means of MR makes it possible to plan an osteotomy accurately.

[Computed tomographic morphometry of the normal pancreas in adults].

The relationship of the morphology of the normal pancreas, as seen on CT, and age was studied in 200 normal people aged between 19 and 87 years. The A.P. diameter, vertebral body/pancreas ratio, cranio-caudal diameter, contour and structure of the pancreas were analysed. The most marked age-related changed occurred in the vertebral body/pancreas ratio, but all other measurements showed a reduction dependent on age. With increasing age, the contour of the pancreas becomes more lobulated and, at the same time, the structure changes from an homogeneous to an inhomogeneous appearance. These findings correspond with pathological-anatomical observations.

[Fresh and older elongation of the anterior cruciate ligament. Experiences in diagnosis and therapy].

The elongation of the anterior cruciate ligament leads to a ligamentary insufficiency which is followed by an instability of the knee joint. After acute trauma isolated second degree sprains may be treated conservatively, while old elongations, with a secondary laceration of the anterior-medial ligamentary structures, have to be treated operatively. We use the transpositions of the anterior cruciate ligament also in combination with the semitendinous tendon. Since 1980 at our department 36 old elongations of the anterior cruciate ligament were treated by operative reefing of the ligamentary structures. The best results as documented in a clinical and CT-Scan follow-up, were recorded when using a distalisation of the insertion of the anterior cruciate ligament together with a semitendinous transplant.

[CT findings and CT-guided puncture of a perineal cyst].

CT findings and treatment results after CT-monitored surgery of a large perineal cyst are described. This was identified as a Müllerian duct cyst which is considered--according to world literature--to be a great rarity. The high rate of relapses even on instillation of sclerosing solutions, as well as differential diagnosis in relation to other perineal cystic space-occupying growths, are discussed.

MR in the diagnosis of bone tumours.

Forty-two patients with benign as well as malignant bone tumours or with tumour-like lesions of bone were evaluated using MR tomography. The appropriate choice of examination parameters is an essential prerequisite for the adequate visualisation and the definitive verification of the presence of a tumour. By means of its multi-planar display, MR provides definite advantages in the determination of tumour localisation and extension as well as in differentiating intra-tumoural haemorrhage from surrounding, non-involved tissue. In addition, the clear delineation of neighbouring structures such as muscular compartments and blood vessels achieved in MR can be useful in the planning of an eventual surgical approach. This new technique has proved itself superior to other available imaging modalities in the differentiation of residual or recurrent tumour from post-operative changes. A characterisation of the nature or histology of a suspicious lesion on the basis of relaxation-times or signal intensities, however, is not yet possible.

[Normal findings and pathological changes of the patellar ligament in the computer tomogram].

In the computer tomogram, the normal patellar ligament appears as a flat band with an average density of 110 HE. After trauma and also after chronic overloading, the cross-section increases, whereas the density decreases. CT is superior to conventional x-ray diagnosis and to sonography in respect of detecting and differentiating of circumscript changes in the patellar ligament.

The effects of fluid-mechanical factors on MR imaging in the arterial model.

Blood flow is a significant factor in MR imaging. The purpose of this investigation was to study fluid-mechanical effects on MR imaging. MR images generated by the pulsatile flow of bovine blood in an elastic model of the aorta with both renal arteries were studied. The high-frequency pulse sequence was synchronized to an ECG trigger signal. It was demonstrated that different flow velocities, laminar and turbulent flow, and backward flow can be visualized with MR imaging.

[Computed tomographic findings in amebiasis].

With an increase of travel to foreign countries with endemic amoebiasis we will find this disease also in western countries. The typical pathologic changes can be identified in CT, as we report in a case with proved amoebiasis which showed an abscess in the bowel-wall and another in the neighbouring liver.

Cerebral insulin-like immunoreactivity in rats and mice. Drastic decline during postnatal ontogenesis.

We studied the behavior of the insulin-like immunoreactivity in brains of rats and mice during the first 20 d post natum by immunohistochemistry and radioimmunoassay. It was found that a dramatic decline in the concentration of the peptide, accompanied by a strong reduction of immunoreactive cells, takes place during this period. A possible role of cerebral insulin as a promoter of nerve cell growth and development is briefly discussed.

[The joints in the posterior part of the foot in the computed tomogram. Studies on amputation preparations and clinical observations].

CT of the foot often helps clarification of difficult problems where conventional radiology has proved inadequate. Sections in the coronary and transverse plane may demonstrate fractures, joint involvement and displacement of bone fragments. Ill-defined degenerative changes in the talo-calcaneal joint can be demonstrated. The extent and localisation of rare tumours of the foot can be shown and additional information may be obtained by densitometric studies.

[Typical computed tomographic findings in neurofibromatosis].

Three patients with neurofibromatosis (Recklinghausen's disease) were examined by computer tomography. In this way, the exact extent and localisation of the neurofibromatous tumours could be demonstrated. In one patient, CT provided evidence of malignant change; this was verified histologically. The appearances of neurofibromatosis associated with elephantiasis is described. In each patient, treatment was significantly influenced by computer tomography.
