[The neodymium:YAG laser in infiltrating bladder tumors].

Over the last 11 years we have treated 47 patients with infiltrating bladder carcinomas with the neodymium:YAG laser alone or in combination with TUR. After a mean observation period of 3 1/2 years (19-133 months) 39 patients (83%) were still alive. Only two of them showed tumor dissemination. Thirteen patients (27.7%) had no recurrent disease after laser therapy. Mostly heterotopic recurrences with no tumor progression were found in 23 patients (48.9%). Six patients (12.8%) developed recurrences with progression, including two cases with pelvic lymph node metastases. Five patients (10.6%) died of their tumor disease and three of myocardial infarction, having shown no signs of tumor recurrence at the last follow-up examination.

Pep5, a new lantibiotic: structural gene isolation and prepeptide sequence.

A wobbled 14-mer oligonucleotide was derived from the amino acid sequence of the 34-residue propeptide of the lantibiotic Pep5 (Kellner et al. 1989). Using this hybridization probe, the structural gene of Pep5, pepA, was located on the 18.6 kbp plasmid pED503. The nucleotide sequence of pepA codes for a prepeptide with 60 residues and proves that Pep5 is ribosomally synthesized. The N-terminus of the prepeptide has a high alpha-helix probability and a characteristic proteolytic cleavage site precedes the C-terminal 34-residue propeptide. Our present theory is that maturation of Pep5 involves (a) enzymic conversion of Thr, Ser and Cys into dehydrated amino acids and sulfide bridges, (b) membrane translocation and cleavage of the modified prepeptide.

Polymorphism of the human T cell receptor alpha chain variable genes: identification of a highly polymorphic V gene probe.

In this study, we report the RFLP of the human T cell receptor (TCR) alpha chain variable gene segments. Using DNA samples from 20 individuals and three restriction endonucleases (BamHI, EcoRI and HindIII), the degree of RFLP of a number of different V gene segments was defined. Half of the V alpha subfamilies (6/12) were characterized by a predominant hybridization pattern, with only a few individuals displaying a second pattern. However, one particular V gene family, V alpha 6, has at least five allelic forms that segregated consistently in familial studies. The V alpha polymorphisms revealed in this study, together with those exhibited by V beta gene subfamilies, should prove useful in studying possible associations between TCR gene usage and disorders of the immune system.

[Penis plication-plasty--an alternative to the treatment of penile deviation].

Congenital and acquired penile curvature are observed increasingly often. A new surgical technique (plicating procedure) was applied in 27 patients with penile deviation. All patients retained potency, and 96% were completely satisfied with the functional result of surgical correction. No severe complications were observed. Nesbit's technique of excising ellipses from the tunica albuginea to correct penile deviation should be discontinued as it involves greater surgical trauma. Since the functional and cosmetic results are equally good with the plicating procedure, we believe this is the operation of choice for the correction of penile curvature.

[Adjuvant 5-fluorouracil lacquer administration for reducing the rate of recurrence of condylomata acuminata following laser surgery treatment].

In the recent past we have observed HPV-induced condylomata acuminata with increasing frequency. Providing 60-80% complete remission the laser coagulation is primary in the treatment of condylomata acuminata. However, the generalized mucosal infection with subclinical condylomata acuminata leads to a recurrence in 30-70% of cases. Therefore, we combined laser treatment with adjuvant 5-fluorouracil painting. In a prospective study, 137 patients with a total of 225 treatments were randomly allocated to three groups (laser/laser + 5-FU ointment 5%/Laser + 5-FU 2% painting). After laser treatment only, relapse occurred in 29% of cases, after laser treatment plus 5-fluorouracil 5% ointment in 32%, and after laser treatment and adjuvant 5-fluorouracil 2% painting in only 4%. A topical prophylactic treatment is beneficial because of the multifocal action of HPV. The adjuvant 5-FU painting after laser surgery has been shown to be superior to other treatment modalities with a recurrence rate of only 4%.

Renal carcinoma. Reevaluation of prognostic factors.

The purpose of the study documented in this article was to reevaluate the prognostic value of tumor size, cytologic grading, and histologic pattern in patients with renal carcinoma. One hundred and eighty-six renal carcinomas were histologically graded. In 175 cases clinical follow-up information was obtained. It was found that for 5 years of survival there are essentially two prognostic groups. The first group includes tumors with solid patterns and clear cells, tumors with granular cells without field, and tumors with a papillary pattern or oncocytic cells. This group has a much better than the second group of tumors, which have either a large number of granular cells with field or a sarcomatoid or stratified pattern. It was also observed that for prognostic evaluation the histologic pattern was a better parameter than size and cytologic grading.

The small cystic and noncystic noninflammatory renal nodules: a postmortem study.

We report the incidence and features of small cystic and noncystic noninflammatory renal lesions found in a series of 500 unselected necropsies performed for forensic reasons in a Portuguese population. A total of 997 kidneys was sectioned serially in 0.2 cm. sections. There were 1,256 lesions (the large majority being less than 1 cm.) found in 380 examined organs belonging to 265 cases. A total of 1,105 cystic lesions was observed in 195 (39 per cent) specimens, 108 medullary fibrous nodules in 89 (17.8 per cent), 29 cortical adenomas in 20 (4 per cent), 5 leiomyomas in 5 (1 per cent), 4 lipomas in 4 (0.8 per cent), 2 fibromyolipomas in 2 (0.4 per cent) and 3 fibromas of the capsule in 3 (0.6 per cent). Cystic lesions showed an increase in number with patient age, which does not occur with medullary fibrous nodules. Cortical adenomas were most frequent at the fifth and sixth decades of life. The risk of malignancy, with the possible exception of some of the cortical adenomas, is believed to be small.

[2d testicular cancer detected during thorough after care].

Bilateral testicular occur in about 5% of patients. The time interval between the diagnosis of the first and second tumor may be more than 10 years. Because the prognosis depends on early detection of the second tumor, longterm follow-up to exclude a contralateral malignancy is mandatory. This article discusses the principle of follow-up of 208 patients with testicular tumors and the treatment of patients with bilateral testicular cancer. The role of ultrasound in the diagnosis of a possible second tumor is emphasized.

Different mechanisms of insensitivity to the staphylococcin-like peptide Pep 5.

Three different mechanisms of insensitivity to the bactericidal membrane disrupting action of the staphylococcin-like peptide Pep 5 were found to occur among certain Gram-positive bacteria. i) The immunity of the producer of Pep 5 Staphylococcus epidermidis 5 was shown to be due to a Pep 5 antagonist, which was excreted along with Pep 5 during growth. The cells were killed by Pep 5 when incubated with high doses exceeding the concentration of the antagonist. ii) Bacillus subtilis W23 produced a protease which cleaved Pep 5 into fragments with greatly reduced bactericidal activity. Like with S. epidermidis 5, cells of B. subtilis were susceptible to the membrane damaging action of the peptide and growth in culture was only possible after inactivation of Pep 5. iii) By prolonged incubation in the presence of Pep 5 resistant mutants of S. cohnii 22 and S. epidermidis 5 Pep 5- could be selected. Their cytoplasmic membrane was not sufficiently disrupted by the peptide to promote active killing, resulting in unhindered growth of these mutants in the presence of high doses (200 AU/ml) of Pep 5.

Familial occurrence of testicular cancer.

A case of testicular cancer in 2 brothers is reported and a review of the literature about testicular malignancies and etiologic factors in closely related family members is given. The familial incidence of testicular tumor is found to be 3% in twins and 0.6-2.1% in less closely related men. Tumors were of the same histology in 70-77% of the twins, whereas in brothers and in other degrees of relationship tumors of different histology mostly occur. Following the diagnosis of the tumor in the first man, the average interval to presentation of the tumor in the relative was 3.7 years in twins, 7.7 years in nontwin brothers and 13.5 years for less closely related men. The need for a thorough checkup of other family members is advised.

Effects of phencyclidine and analog drugs on acetylcholine receptor of cultured muscle cells.

Myotubes grown in culture provided a convenient experimental system for the study of the effects of phencyclidine (PCP) and analog drugs on both acetylcholine receptor (AChR) function and on its binding properties. The extent of PCP retention by these cells was studied on the same preparations. PCP, N-ethyl-l-phenylcyclohexylamine (PCE), PCP methiodide (PCPMeI), 1-[1-(3-aminophenyl)-cyclohexyl] piperidine (NH2PCP) and 1-[1-(2-thienyl)cyclohexyl] piperidine (TCP) were found to inhibit carbamylcholine (CbCh)-induced 22Na and 45Ca ion fluxes with 50% inhibition (I50) at 2-6 microM drug concentration. The I50 for CbCh-induced 42K+ efflux was 8-20 microM. Ketamine was less efficient with an I50 of 100 microM. Binding of [125I] alpha-bungarotoxin [( 125I]alpha-BGT) was not affected at drug concentrations that cause 100% inhibition of ion fluxes. Retention of [3H]PCP by the myotubes was a saturable process with half-maximal saturation at approximately 20 microM PCP. It was inhibited by PCP and several tertiary analogs, with and I50 of approximately 20 microM. PCPMeI was much less effective, with an I50 of 1 mM. PCPMeI was, however, as potent as PCP in its inhibition of the AChR function although the amount retained by the cells was 50-fold lower than that of PCP. These results are consistent with the theory that PCP and analog drugs affect AChR at a site other than the alpha-BGT binding site, possibly at the ionic channel of the nicotinic receptor.

[Renal-cell carcinoma. The basic concepts].

This article includes brief discussions of renal parenchymal neoplasms. The authors summarise epidemiological data and natural history of these lesions, the most relevant histopathologic characteristics, the classification of renal tumors, as well as outcome and the basic principles of treatment and follow-up. The staging systems, survival and prognostic factors in renal cancer are also reviewed. Special attention is given to the detection and earlier diagnosis of renal tumours because of the wider use of sonography and CT scanning. Although this has improved the cure rate of renal cell carcinoma, it has also increased the detection of all types of renal masses, benign and malignant, including those that require surgery and those that do not.

Supracondylar fractures of the humerus associated with ipsilateral forearm fractures in children: a report of forty-seven cases.

Supracondylar fractures of the humerus associated with ipsilateral forearm fractures are uncommon and treatment recommendations are controversial. The purpose of this study was to determine whether pin fixation of both fracture components, humerus and forearm, would improve the outcome. In a two-center trial, 884 children sustaining supracondylar fractures of the humerus were retrospectively reviewed, and 47 (5.3%) showed associated ipsilateral forearm fractures. Of those, 29 underwent Kirschner-wire fixation of the forearm fracture, and 18 of the forearm fractures were treated with casting alone. Three of the 18 forearm fractures with casting alone reangulated. There were no reangulations in the patients who had pin fixation of their fractures. There were no complications due to pin fixation in the humerus or the forearm. In unstable supracondylar humerus and forearm fractures, stabilization with pin fixation to prevent reangulation should be considered.
