[Special aspects of right colon cancer].

752 patients with colon carcinoma of the years 1966-1985 were studied retrospectively. 189 tumors were located in the right and 563 in the left colon. There was no difference in the 5-year survival rate, Dukes classification and operative mortality in both groups. Considering the higher median of tumor diameter, the three months longer diagnostic delay and the twice higher rate of undifferentiated carcinomas the prognosis of the right sided tumors is relatively better. In our opinion the cause of this more efficient local tumor defence is the better blood and lymph supply of the right colon and mucosal mechanisms. Expression of this better tumor defence is the distinct tumor colitis in the right colon. Improvement of the right colon carcinoma prognosis is to be expected of earlier tumor diagnosis. Routine endoscopic evaluation of hemoccult positive subjects is most helpful.

[Indication and risk of internal intestinal splinting. Comments and supplement to the published article of W. Kapral: Splinting of the small intestine with the Miller-Abbot tube. Chirurg (1984) 55:394].

The usefulness of splinting of the bowel to prevent recurrent bowel obstruction caused by adhesions is analysed. For that is differentiated between three possible indication groups: 1. late bowel obstruction, 2. early postoperative bowel obstruction, 3. mixed peritonitic and mechanical ileus. The risk of splinting of the bowel can only be seen in relation to the number of recurrences, complications and mortality rates as seen in the three indication groups, treated without splinting. From that it shows that in case of peritonitis splinting can only be an adjuvant measure of therapy, and that in case of both recurrent late bowel obstruction and early postoperative bowel obstruction splinting of the bowel has a real preventive effect. This differentiation in indication is of no importance in childhood.

[Indication for preventive measures in benign colonic diseases].

For non-malignant colon diseases the indications for preventive measures can be of tumorous and non-tumorous nature. In case of ulcerative colitis and Crohn's colitis local and systemic complications may demand a colectomy. The operative procedures in this case are proctomucosectomy with ileumpouch or ileorectostomy , if continence can be preserved. Mucosal proctectomy with ileumpouch will prevent the obligatory malignant change in familial adenomatosis. The preventive resection in diverticulitis bewares of the complication of the progressive phlegmonous infection of the colonic wall.

[Preoperative treatment of severe coronary stenosis with the balloon catheter in patients with operable carcinomas (author's transl)].

Two patients with severe angina pectoris during minimal physical exercise and angiologically proven severe coronary stenosis also had a surgically operable carcinoma of the bronchus and stomach, respectively. Extensive operation for carcinoma would have been a serious cardiological risk. A preceding aortocoronary bypass operation would have been an additional stress as well as a delay of surgery by several weeks. Using percutaneous transluminal coronary dilatation it was possible to remove severe and isolated coronary stenoses rapidly and with little effort. Both survived carcinoma operations a few days later without trouble.

[Early resection in diverticulitis (author's transl)].

The danger with diverticulitis is that the disease may progress to life threatening complications. The development of diverticulitis leads from local infiltration and fibrosclerosis to final perforation. Nonresected diverticulitis favors the development of diverticulosis proximal to the affected bowel. Conservative therapy seems to be of little use in avoiding the progression of recurring diverticulitis. Signs of irreversible diverticulitis are repeated attacks, signs of local peritonitis, fixed deformation of the wall and stenosis, revelaed by barium enema. One-stage resection was performed in 101 patients for uncomplicated diverticulitis. There was one death (myocardial infarction).

[Rectal neoplasms: the current state of surgical treatment].

The number of patients with rectal cancer ist increasing steadily in the BRD. Even if diagnosis of early stages of rectal cancer is not too difficult, the fatal diagnostic delay is 7-8 months on the average. After operative methods have been subtilized and radicalized only preventive medical examinations and early public instruction may improve the prognosis of the patients concerned. Additional treatment with cytostatical agents seems to be promising. If preventive medical care makes it possible to detect rectal cancer in its early stages, the same operative results can be obtained by the meanwhile developed continence-preserving methods as by the hitherto carried out widespread and continence-sacrificing rectal resection as our own experiences prove.

[Problems in the surgical treatment of sliding hiatal hernia (author's transl)].

The aim of surgical treatment of hiatus hernia is well defined: restoration of the esophago-gastric closing mechanism to prevent further reflux of gastric contents and gastric juice into the esophagus. The main problem to realize this demand lies in the variety of discussed closing mechanisms. Interaction of different mechanisms influencing the "anti-reflux barrier" as well as hormonal and neural regulation of the lower esophagus are not yet examined sufficiently enough. Benefits and hazards of several types of operations for repair of hiatal hernia are discussed before the background of the small but slowly growing understanding of the esophago-gastric closing mechanism.

[Local cryosurgical management of colonic and rectal cancer].

Complex experimental investigations created the basis for cryosurgical treatment of colon and rectum cancer. 27 patients were chosen for cryotherapy according to strict indication. Especially developed instruments and technique are presented. A three year-follow up-study shows 19 patients alive, most of them without recurrence or troubles. Immediately after starting with cryosurgical treatment patients recover remarkably. In collaboration with the Dept. of Microbiology (Rhein.-Westfäl. Techn. Hochschule Aachen) the possibility of a special cryoimmunological response is investigated.
