Risk prediction in operatively treated fractures of the hip.

There have appeared no objective means by which preoperative risk in patients with fractures of the hip can be quantitatively predicted. The developed risk's core is based on medical history, physical examination, chest radiograph, and screening laboratory data. This system reproducibly assigns patients into one of three groups. Three hundred seventeen patients with surgically treated hip fractures (93 femoral head, 166 neck, 58 intertrochanteric fractures) were analyzed in this study. Surgical treatment included 102 Ender-pinnings, 43 Böhler-nails, 79 dynamic hip screws, and 93 endoprostheses of the hip (according to fracture type). Overall mortality rate during hospitalization was 9.5%. The mortality rate in Group I was 2.5%; in Group II, 11.9%; and in Group III, 44.1%. The medical complications in Group I were 12.6%; in Group II, 28.6%; and in Group III, 67.6%. The difference for each group was significant. This preoperative risk assessment appears effective in more accurately identifying patient risk.

[Judgement of prognosis in multiple trauma of patients with blunt abdominal injuries based on different useful trauma scores].

The use of trauma indices for characterisation of polytraumatized patients concerning prognoses is being discussed for nearly 30 years. In the seventies a couple of trauma indices were developed. Some of them are in use all over the world. In the Department for Trauma surgery in the General Hospital of Salzburg three of the common trauma scores were analysed and compared with one another: Injury Severity Score (Baker), Polytraumascore of the University Clinic of Hannover, Trauma index (Schreinlechner and Eber). As a rule most studies presently published are based on the Injury Severity Score. In a retrospective analysis of 62 polytraumatized patients we could show that the Trauma index of Schreinlechner and Eber is more practicable for classification of polytrauma because it shows a good correlation of score, fatality and prognoses.

[Color-coded duplex sonography and ultrasound contrast medium in the study of peripheral arteries--initial clinical experiences].

Ultrasound contrast agents (US-CA) amplify reflected sound waves. Most substances used as contrast agents are destroyed when passing the lungs. SH U 508 is a new US-CA that can pass the lungs without impairment after peripheral intravenous application. In a clinical trial of this US-CA, we investigated its effect on the visualization of blood movement in peripheral arteries by color-coded Duplex sonography (CCDS). The leg arteries of 20 patients with severe chronic arterial occlusion were examined by CCDS (QAD I and Platinum) after i.v. application of the US-CA. After passage of the pulmonary capillaries, the US-CA amplified blood flow signals in the arterial system in a dose-dependent manner with both systems used. Undesired side-effects were not observed. The amplification produced by appropriate concentrations of the US-CA markedly improved the visualization of blood movement. Further studies are required to determine the optimal dosage and application technique as well as the indication for using this US-CA.

[Primary and secondary tethered cord syndromes. Their nuclear magnetic tomographic diagnosis and comparison with the surgical findings in 40 patients].

The MRT findings in 40 patients with the tethered cord syndrome (TCS) have been compared with the operative findings. In cases of primary TCS (17 patients), MRT showed a low conus medullaris in 15 cases and this was confirmed surgically in 14. Other abnormal findings were lipomas (MRT 12/operatively 13), diastematomyelia (MRT 2/operatively 2) and thickened filum terminale (MRT 6/operatively 3). In secondary TCS (23 patients) postoperatively adhesions were prominent (MRT 16/operatively 23) while low position of the conus was found by MRT in 19 and surgically in 18 patients. Other findings were lipomas (MRT 5/operatively 6), epidermoids (MRT 2/operatively 2) and diastematomyelia (MRT 2/operatively 0). Agreement between MRT and operative findings regarding fixation of the spinal cord is closer in primary TCS (15/17, 88%) than in secondary TCS (16/23, 69%).

Dyskeratosis congenita associated with elevated fetal hemoglobin, X-linked ocular albinism, and juvenile-onset diabetes mellitus.

An 11-year-old boy had dyskeratosis congenita, elevated fetal hemoglobin level, X-linked ocular albinism, and juvenile-onset diabetes mellitus. A review of the international literature revealed that elevated fetal hemoglobin has been noted in 15 reported cases of dyskeratosis congenita. It is a previously unrecognized, commonly associated finding in dyskeratosis congenita that may provide insight into the location and function of the gene for dyskeratosis congenita.

Polypeptides synthesized in rabbit cells infected with murine herpesvirus (MHV): a comparison of proteins specified by various MHV strains.

Polypeptides synthesized in rabbit embryo fibroblasts (REF) infected with six isolates of murine herpesvirus (MHV) coming from different localities in CSFR were compared by electrophoresis (SDS-PAGE). Altogether 28 virus-coded polypeptides with apparent molecular weights ranging from 240 kD to 16 kD were identified. While isolates MHV-60, MHV-72, and MHV-76 had identical polypeptide profiles, the isolates MHV-68 and MHV-78 lacked a polypeptide with apparent molecular weight of 46 kD, the rest of polypeptides being identical as well. The polypeptide profile of MHV Sumava A.f. was most different from that of other MHV isolates. This result is in agreement with some different biological properties of this isolate. At a high multiplicity of infection (40 TCID50 per cell) first polypeptides with apparent molecular weights 125 kD and 84 kD were detected by 5-9 hr post-infection (p.i.). All other proteins appeared 14-18 p.i.

[The choice of the method of anesthesia as a risk factor in the femoral fracture near the hip joint in elderly patients].

Between January 1983 and December 1988, 317 patients with fractures near the hip joint were operated upon. In a retrospective study we compared 180 patients with general anesthesia (AN) and 137 patients with spinal anesthesia (RA). The average age of patients with RA was 80.5 and with AN 79.8 years. The preoperative management was done by an internist. The operation followed at an average of 2.8 days (Tables 1, 2). All patients were placed into 3 risk groups, based on after preoperative cardiac and pulmonary risk scores, as modified by Goldman (Table 3). Spinal anesthesia was performed with 2 ml 4% hyperbaric mepivacaine (80 mg mepivacaine hydrochloride, 190 mg glucose monohydrate). The AN was induced with thiopental (3-5 mg/kg) and maintained with N2O and enflurane. Fentanyl (0.05-0.15 mg) was given for analgesia. The statistical examination was done using the chi-square test (Fisher-exact or Pearson). The groups were compared for age, time of operation, and risk score (Table 4). The mortality in operations with protection of the femoral head (FP) (Ender pinnings, Böhler pinnings and dynamic hip screws) was significantly higher after RA (13.8%), than with AN (6.5%). In operations with hip prostheses (HP), the mortality for RA (19.0%) was also significantly higher then for AN (4.2%) (Fig. 1). The mortality in group I (4-20 points) was 1.7% in patients with AN (FP 1.4%, HP 2.1%) and 3.7% for RA (FP 4.1%, HP 0%).(ABSTRACT TRUNCATED AT 250 WORDS)

Characterization of Mycobacterium leprae cell wall-associated proteins with the use of T lymphocyte clones.

Development of a vaccine against leprosy depends on the identification of Ag that stimulate cell-mediated immune responses. We have previously demonstrated that cell wall proteins of Mycobacterium leprae are highly immunogenic. By using human cell wall-specific T cell clones we have begun to characterize soluble proteins that integrate into the cell wall skeleton. T cells from leprosy lesions were expanded with IL-2 in vitro yet retained specificity to Ag of the insoluble cell wall core (CWC) in vitro, indicating that T cells had been activated by CWC Ag in vivo. A cell wall protein-peptidoglycan complex and cell wall protein preparations lacking carbohydrates and lipids from CWC retained T cell reactivity. To identify immunogenic protein component(s) of cell wall protein, T cell lines were established to cell walls and tested against M. leprae proteins separated by SDS-PAGE and transferred to nitrocellulose. Greatest T cell reactivity was observed to proteins of Mr 7 kDa, 16 kDa, and 28 kDa. T cell clones reactive with 7-kDa and 16-kDa Ag from gels failed to respond to proteins of other Mr separated under either reducing or nonreducing conditions, indicating that these molecules are not subunits of larger proteins and may represent monomeric units polymerized into cell walls. The approaches described herein for characterization of immunodominant T cell Ag of M. leprae may be useful for study of T cell Ag in cell walls of bacterial pathogens of man.

[Nuclear magnetic resonance tomographic appearance of disseminated encephalomyelitis in relation to clinical diagnosis].

The MRI findings in 149 patients with a clinical diagnosis of disseminated encephalomyelitis (D.E.) were related to the diagnostic criteria of McAlpine (definite, probable, possible). The most common pattern in all three groups was a mixed peri- and para-ventricular distribution of foci. The findings in 'possible' D.E., compared with 'definite' D.E., showed fewer confluent lesions, but were characterised by peri- and para-ventricular as well as solitary foci. The results of the present study permit better classification and evaluation of the MRI findings if the clinical diagnosis of D.E. is only suspected (probable or possible).

A simple novel procedure for preparation of herpes simplex virus subunit vaccine.

A simple procedure for preparation of herpes simplex virus type 1 (HSV-1) subunit vaccine is described. The method is based on treatment of virus-infected cells with a nonionic detergent, removal of cell nuclei by low speed centrifugation, separation of viral nucleocapsids by high speed centrifugation through a sucrose cushion and, finally, precipitation of viral and cellular glycoproteins and proteins with ammonium sulphate (AS). Unlike to acetone precipitation, affinity chromatography on lectins or hydroxylapatite chromatography, AS precipitation repeatedly yielded the best and most reliable results as judged by relative protein content, antigenicity and immunogenicity of the final vaccine product.

Varicella-zoster virus IgM antibody fraction separated by minicolumn gel filtration in serodiagnosis of acute infection.

Gel filtration through Sephadex G-200 or Ultrogel AcA 34 minicolumns was applied to isolate IgM antibodies from sera of patients with varicella and herpes zoster (HZ). Serum samples were stained with Sudan III and Evan's Blue in order to monitor the separation process visually when running several columns at the same time. The presence of specific IgM antibodies was demonstrated by enzyme linked immunosorbent assay (ELISA). Rapid separation of stained sera by diffusion chromatography on minicolumns with successive determination of specific IgM antibodies in ELISA proved suitable for serological diagnosis of varicella, whereas the significance of IgM-specific antibody response in HZ seems uncertain.

[Diagnostic procedure in uveitis patients to determine general concomitant disorders].

A thorough examination, planned jointly by an internist and an ophthalmologist, is advocated for patients with uveitis. Close cooperation with all medical subspecialties is mandatory in order to save the time and money of both the physician and the patient. One or more relevant concomitant diseases were diagnosed in 79 patients out of a total of 206 who came to the Deutsche Klinik für Diagnostik, Wiesbaden, for an investigation of their uveitis.

Motility mutants of Dictyostelium discoideum.

We describe six motility mutants of Dictyostelium discoideum in this report. They were identified among a group of temperature-sensitive growth (Tsg) mutants that had been previously isolated using an enrichment for phagocytosis-defective cells. The Tsg mutants were screened for their ability to produce tracks on gold-coated cover slips, and several strains were found that were temperature-sensitive for migration in this assay. Analysis of spontaneous Tsg+ revertants of 10 migration-defective strains identified six strains that co-reverted the Tsg and track formation phenotypes. Characterization of these six strains indicated that they were defective at restrictive temperature in track formation, phagocytosis of bacteria, and pseudopodial and filopodial activity, while retaining normal rates of oxygen consumption and viability. Because they had lost this group of motile capabilities, these strains were designated motility mutants. The Tsg+ revertants of these mutants, which coordinately recovered all of the motile activities, were found at frequencies consistent with single genetic events. Analysis of the motility mutants and their revertants suggests a relationship between the motility mutations in some of these strains and genes affecting axenic growth.
