Antenatal diagnosis of sickle-cell anaemia by means of DNA restriction analysis.

Prenatal diagnosis by restriction enzyme analysis is now available in South Africa for globin-related disorders. An Indian couple at risk for sickle-cell anaemia requested prenatal diagnosis which was carried out by restriction enzyme analysis of DNA from cultured amniotic cells. In the case of the sickle-cell mutation a restriction enzyme, Mst II, is available for direct detection of the mutant gene. Mst II together with linked restriction fragment polymorphisms were used to make the diagnosis in this family. The fetus was found to be homozygous for the normal allele and this was confirmed postnatally by placental DNA analysis.

A comparison of two developmental scales in evaluating infants with Down syndrome.

35 sets of developmental scores were obtained on Down syndrome children by administering the Griffiths Mental Developmental Scale and the Bayley Scales of Infant Development in one testing session. The mean age at testing was 14.3 months, with a range of 3-30 months. The developmental scores of the two tests were similar for the 3-7 month-olds, but became significantly different for the 8-30-month-olds, the Griffiths scores being consistently higher. In addition, the Griffiths scores provided more detailed profiles of the developmental status of children with Down syndrome.

A comparative study of two developmental scales: the Bayley and the Griffiths.

50 high-risk infants consecutively selected from the population of a Neonatal Follow-up Clinic were tested using both the Bayley and the Griffiths Scales on the same day by the same psychologist without prior knowledge of the medical history. The mean age at testing was 13.1 mth with a range of 12--15 mth. Scores obtained from the Griffiths Scale were consistently higher than those obtained from either of the Bayley Scales (P less than 0.001). Intercorrelations between the Griffiths and the Bayley test were high at pH 0.01. These results suggest that, although the two tests may be used interchangeably, their numerical scores are not equivalent when administered at this age.

Serogroup W135 meningococcal disease in Hajj pilgrims.

An outbreak of W135 meningococcal disease occurred in the spring of 2000 among pilgrims returning from Saudi Arabia and their contacts. Clinical isolates from England and France were examined and compared with reference strains from other countries. Characterisation of isolates by a range of typing methods showed them to be of clonal origin (ET-37) and closely related to other meningococci with an established propensity to cause disease clusters. A reappraisal of vaccination strategies for travellers is required.

Tranexamic acid in subarachnoid hemorrhage. A double-blind study.

The effects of intravenous tranexamic acid were compared with placebo in 64 patients with subarachnoid hemorrhage. A double-blind procedure was used. One gram of tranexamic acid was given intravenously every 4 hours up to the time of operation on an intracranial arterial aneurysm or for up to 21 days after the first bleeding if operative treatment was not feasible. There were no differences in re-bleeds, morbidity or mortality between the tranexamic and placebo-treated groups. No thromboembolic complications were noted in either group. Our results do not support the use of tranexamic acid in subarachnoid hemorrhage in daily doses of 6 g.
