Acute lower respiratory tract infection due to virus among hospitalized children in Dhaka, Bangladesh.

The nature of acute lower respiratory tract infection (ALRI) in hospitalized children and of the associated viral agents was assessed in a study of 601 children less than 5 years old over a 24-month period. Of these children, 80% were less than 24 months of age and the ratio of boys to girls was 1.7:1. Pneumonia (86.5% of cases) was the most frequently observed clinical manifestation. Shedding of virus was detected in 21.1% of the children; the highest rate occurred in infants 0-5 months old (27%) as compared with a rate of only 12.5% in children 25-60 months old. Virus was detected in 33.3%, 32.8%, 21.2%, and 20% of the cases of tracheobronchitis, bronchiolitis, pneumonia, and croup, respectively. Among the viruses detected, 78% were respiratory syncytial virus (RSV) (91% of infections with this virus occurred in children less than 2 years old) and 14.4% were influenza virus types A and B. Of the RSV infections, 61% occurred in infants less than 1 year old. The case-fatality rate was 6.8% overall and was 4.8% in virus-associated cases. No consistent pattern of seasonal occurrence of viral infections was discerned. RSV was detected throughout the year, with increased prevalence from January to April.

Pregnenolone sulphate and Zn2+ inhibit recombinant rat GABA(A) receptor through different channel property.

AIMS: We compared the antagonistic effects of state-dependent gamma-aminobutyric acid A (GABA(A)) receptor blockers picrotoxin, Zn(2+) and pregnenolone sulphate (PS) on GABA- and pentobarbital-activated currents in recombinant rat GABA(A) receptors in Xenopus oocytes. METHODS: Experiments were performed with wild type rat alpha1 beta2 gamma2L and alpha1beta2 receptors, mutants alpha1V256S beta2 gamma2L and alpha1beta2A252Sgamma2L receptors by the two-electrode voltage-clamp technique. RESULTS: In contrast to respective 3840- and 56-fold increases in Zn(2+) potencies to inhibit GABA- and pentobarbital-activated currents in the alpha1beta2 receptor, the corresponding potencies of PS remained unchanged in comparison with the alpha1 beta2 gamma2L receptor. A homologous mutation of the residue at 2' position closest to the cytoplasmic end of the M(2) helix to serine on both alpha1 and beta2 subunit, alpha1V256S and beta2A252S, abolished the inhibition of GABA(A) receptor by PS. In comparison with the wild type alpha1beta2gamma2L receptor, mutants alpha1V256S beta2 gamma2L and alpha1beta2 A252S gamma2L receptors did not affect the Zn(2+) inhibition. Furthermore, a significant increase in GABA potency was observed in the mutant alpha1V256S beta2 gamma2L receptor (P < 0.05), but not the mutant alpha1beta2 A252S gamma2L receptor compared with the wild type receptor. CONCLUSIONS: Pregnenolone sulphate was a gamma2-subunit independent inhibitor in the GABA(A) receptor, whereas the Zn(2+) antagonism was profoundly influenced by the gamma2-subunit. It is likely that the 2' residue closest to the N-terminus of the protein at M(2) helix on both alpha1 and beta2 subunit are critical to the inhibitory actions of PS and the function of Cl(-) channels. These results are consistent with the hypothesis that PS behaves as a Cl(-) channel blocker that does not share with Zn(2+), the coincident channel property in the GABA(A) receptors.

Dose distributions in regions containing beta sources: uniform spherical source regions in homogeneous media.

The energy-averaged transport model for the calculation of dose rate distributions is applied to uniform, spherical source distributions in homogeneous media for radii smaller than the electron range. The model agrees well with Monte Carlo based calculations for source distributions with radii greater than half the continuous slowing down approximation range. The dose rate distributions can be written in the medical internal radiation dose (MIRD) formalism.

Influence of superstition on the date of hospital discharge and medical cost in Japan: retrospective and descriptive study.

OBJECTIVES: To determine the influence of superstition about Taian (a lucky day)-Butsumetsu (an unlucky day) on decision to leave hospital. To estimate the costs of the effect of this superstition. DESIGN: Retrospective and descriptive study. SETTING: University hospital in Kyoto, Japan. SUBJECTS: Patients who were discharged alive from Kyoto University Hospital from 1 April 1992 to 31 March 1995. MAIN OUTCOME MEASURES: Mean number, age, and hospital stay of patients discharged on each day of six day cycle. RESULTS: The mean number, age, and hospital stay of discharged patients were highest on Taian and lowest on Butsumetsu (25.8 v 19.3 patients/day, P=0.0001; 43.9 v 41.4 years, P=0.0001; and 43.1 v 33.3 days, P=0.0001 respectively). The effect of this difference on the hospital's costs was estimated to be 7.4 million yen (¿31 000). CONCLUSION: The superstition influenced the decision to leave hospital, contributing to higher medical care costs in Japan. Although hospital stays need to be kept as short as possible to minimise costs, doctors should not ignore the possible psychological effects on patients' health caused by dismissing the superstition.

Effects of tropisetron, a 5-hydroxytryptamine type 3 receptor blocker, on intestinal secretion induced by cholera toxin or deoxycholic acid in rabbits in vivo.

It has been suggested that 5-hydroxytryptamine is involved in the pathogenesis of various intestinal hypersecretory states including cholera. In this study, the effect of tropisetron (ICS 205-930), a specific 5-hydroxytryptamine type-3 receptor blocker, on jejunal and colonic fluid secretion induced respectively by cholera toxin and deoxycholic acid was investigated in rabbits using isolated loops of intestine in vivo. Marked fluid accumulation in both the jejunal and colonic loops was observed after exposure to cholera toxin and deoxycholic acid respectively. Elevation of jejunal and colonic mucosal cyclic adenosine monophosphate concentrations was also noted. Intraperitoneal administration of tropisetron dose-dependent inhibited jejunal secretion induced by cholera toxin. In contrast, no significant anti-secretory effect of tropisetron was observed against colonic secretion induced by deoxycholic acid. Tropisetron did not affect elevated mucosal cyclic adenosine monophosphate concentrations. The inhibitory effect of tropisetron on intestinal secretion induced by cholera toxin, which was independent of cyclic adenosine monophosphate formation, suggests that 5-hydroxytryptamine plays an important role in this type of secretion.

[Severe diarrhea due to rotavirus infection in a Belgian hospital 1981-2002].

Rotavirus infections are a major cause of severe diarrhea in children younger than 2 years. In Belgium they cause many hospitalizations because of dehydration. A study of the laboratory diagnosis of rotavirus infections in 28.251 stool samples at a university teaching hospital in Belgium during a twenty-year period (1981-2002) showed a marked seasonality. The virus was most often diagnosed during the winter months: 54% of the rotavirus isolates were found in the first three months of the year, with 21% of the positive samples occurring in February. Recently, rotaviruses can be genotyped based on differences in the viral outer capsid protein VP7. Vaccines are currently being developed against the four most prevalent genotypes G1, G2, G3 and G4. During the last three epidemic seasons (1999-2002) in Belgium, G1 was the most prevalent genotype and accounted for 62% of the rotavirus isolates recovered. G2, G3 and G4 were also isolated, and other emerging types need to be carefully monitored too, since G9 (45%) was co-dominant with G1 (42%) in the 2000-2001 rotavirus season in Belgium. The future development of an efficient rotavirus vaccine will need to take the diversity of the rotavirus genotypes into account.

Contraceptive use in Matlab, Bangladesh: the role of gender preference.

Research in several Asian societies has suggested that sons are generally preferred over daughters. The implications of gender preferences for actual fertility behavior have not been adequately investigated, however. This analysis examines the effect of the sex composition of surviving children on the acceptance and discontinuation of contraception in a sample of 3,145 women in Matlab, Bangladesh, who were observed for 60 months. Hazards regression analyses are employed in the analysis. Strong and highly significant effects of gender preference on contraceptive use are observed. The preference is not monotonically son-biased but is moderated toward a balanced composition, because parents desire to have several sons and at least one daughter. These findings suggest that gender preferences, particularly a preference for sons, represent a significant barrier to fertility regulation in rural Bangladesh.

Biomedical publications in south Asian countries.

By and large, biomedical research is not a priority sector in south Asian countries, land of world's one-fifth population. The total number of studies published during 1990-98 from each of the south Asian countries were elicited based on MEDLINE database. The number of studies were normalised by population and physician size, and gross domestic products. The results showed that the following countries are more productive: India and Sri Lanka, when publications were normalised to population; India, Sri Lanka and Nepal when normalised to GDP; Nepal and Sri Lanka when normalised to physician number.

Epidemiology of typhoid and paratyphoid fever in Kathmandu: two years study and trends of antimicrobial resistance.

Enteric fever is prevalent in developing countries including Nepal, where it still remains as a major health problem. Appropriate antibiotics are essential for the treatment of typhoid and paratyphoid fever. A prospective study was carried out to characterize the epidemiological features of enteric fever in Kathmandu, Nepal and to analyse the recent trend of antimicrobial resistance pattern of the Salmonella isolated from the cases of enteric fever from different hospitals in Kathmandu during June, 2002 to June, 2004. A total of 1469 Salmonella typhi and Salmonella paratyphi 'A' isolates collected during this period from five different hospital laboratories situated in Kathmandu were studied. The antimicrobial susceptibilities of the isolates towards Ampicillin (10 mcg), Chloramphenicol (30 mcg), Cotrimoxazole (25 mcg), Ciprofloxacin (5 mcg) and Ceftriaxone (5 mcg) were determined by standard disc diffusion technique and Agar dilution technique were used to determine the minimum inhibitory concentration (MIC) for Ampicillin, Ciprofloxacin, Chloramphenicol and Ceftriaxone. All the isolates tested were found to be sensitive to Ceftriaxone and Ciprofloxacin, the most commonly used antibiotic for treatment of enteric fever in Nepal. Of the total isolates studied, 15.5% from 2002, 8% from 2003 and 3.45% from 2004 were found to be multidrug resistant (exhibiting resistance towards Ampicillin, Chloramphenicol and Cotrimoxazole). Of the total multi drug resistant Salmonella isolates, 92% were Salmonella typhi. All the multidrug resistant isolates were also further tested for susceptibilities towards Tetracycline (30 mcg), Nalidixic acid (30 mcg), Streptomycin (10 units), Gentamycin (25 mcg), Azithromycin (15 mcg), Kanamycin (30 mcg), Neomycin (30 mcg). 50% of the multi drug resistant Salmonella typhi were also resistant to Tetracycline. Plasmid analysis revealed that all of the multidrug resistant Salmonella typhi isolates with Tetracycline resistance harbored a large molecular weight (147 Kb) plasmid.

Improved serum HDL cholesterol profile among Bangladeshi male students during Ramadan fasting.

Effects of Ramadan fasting on serum lipids of 20 healthy males in Bangladesh were assessed. Anthropometric parameters and blood lipids were measured 1 day before Ramadan, day 26 of Ramadan and 1 month after Ramadan. Body weight and body mass index decreased significantly during Ramadan compared with before and after Ramadan. Fat intake was significantly higher during Ramadan than after. High-density lipoprotein (HDL) cholesterol increased significantly during Ramadan. Other lipids were not significantly different. Regression analysis of Ramadan HDL cholesterol levels indicated positive association with pulse rate and fat intake and negative association with systolic blood pressure and weight loss. The findings indicate improved HDL cholesterol profiles during Ramadan.

Strengthening government health and family planning programs: findings from an action research project in rural Bangladesh.

An ongoing study at the International Centre for Diarrhoeal Disease Research, Bangladesh (ICDDR,B) is based on the premise that public sector health and family planning programs can be improved through an assessment of the dysfunctional aspects of their operations, the development of problem-solving capabilities, and the transfer of strategies successfully tested in a small-scale pilot project. This paper reports findings from a field trial implemented in a subunit of the project area at an early stage of the project. Operational barriers to public sector program implementation are discussed with regard to the quantity of work, the quality of work, supplies and facilities, integration of health and family planning, and leadership, supervision, and decision making. Initial results of the ICDDR,B intervention on these managerial processes are also indicated.
