Clinical evaluation of Tc-99m N,N'-bis(mercaptoacetyl)-2,3-diaminopropanoate as a replacement for I-131 hippurate: concise communication.

A clinical comparison of Tc-99m N,N'-bis( mercaptoacetyl )-2,3- diaminopropanoate (Component A) (Tc-99m CO2-DADS-A) and I-131 hippurate was conducted in a series of five normal volunteers and 18 patients. Each subject was studied in one session with Tc-99m CO2-DADS-A and I-131-hippurate; digital and analog images were recorded for 30 min and after voiding. In the normal volunteers, digital images with Tc-99m CO2-DADS-A gave a kidney-to-background ratio at 3 min that was greater relative to I-131 hippurate, a leading-edge parenchymal transit time that was similar to I-131 hippurate, and a percent injected dose in the urine at 30 min that was slightly less than I-131 hippurate (p less than 0.05). In patients (serum creatinine 1.0 to 14.3 mg/dl), decreasing renal function impaired excretion of Tc-99m CO2-DADS-A more than that of I-131 hippurate (p less than 0.01). In analog images, Tc-99m CO2-DADS-A always gave superior spatial resolution. No evidence of hepatobiliary excretion was detected with either radiopharmaceutical. We conclude that Tc-99m CO2-DADS-A and similar compounds should be pursued as possible replacements for I-131 hippurate.

Work in progress: clinical evaluation of Tc-99m-trimethylbromo-IDA and Tc-99m-diisopropyl-IDA for hepatobiliary imaging.

Six healthy individuals and six patients with a wide range of hepatobiliary function abnormalities were studied with Tc-99m-trimethylbromo-IDA; all normal subjects and four of the six patients were also studied with Tc-99m-diisopropyl-IDA. Visual evaluation of analog images demonstrated a greater liver-to-kidney ratio for Tc-99m-trimethylbromo-IDA (p less than 0.01). Sampling for radiopharmaceutical in urine at three hours following injection demonstrated that Tc-99m-trimethylbromo-IDA had a lower renal excretion rate than Tc-99m-diisopropyl-IDA regardless of whether hepatocyte function was normal or abnormal (p less than 0.01). There were no significant differences between the two radiopharmaceuticals in hepatocyte extraction efficiency or hepatic parenchymal transit time. It is concluded that the lower rate of renal excretion and, therefore, greater hepatocyte specificity of Tc-99m-trimethylbromo-IDA justifies expanded clinical trials and may make it the radiopharmaceutical of choice for hepatobiliary imaging.

Anomalous foramen transversarium of C2 simulating erosion of bone.

A case is reported of a patient with nocturnal myoclonus and weakness of the left arm in whom a round defect was seen in the arch of C2 on the left side. Vertebral angiogram revealed that this was caused by an anomalous course of the vertebral artery and not by an aneurysm. A similar location of this foramen was found in nine of 100 cases reviewed.

Prediction of gestational age of infants from the abdominal radiograph.

Measurements of lumbar spine elements on abdominal radiographs of 183 white infants were compared with the estimated gestational ages of these infants as determined by physical examination. Using the technique of multiple regression, an equation was derived to predict infant age from the measurements of the spine.

CT of emphysematous gastritis.

Emphysematous gastritis is a serious, often fatal condition. It is the result of destruction of the mucosal membrane with subsequent bacterial invasion of the stomach. We have encountered two cases at CT. Recognition is important since early intervention may be lifesaving.

CT demonstration of a giant adrenal endothelial cyst of the angiomatous subtype.

Adrenal cysts are uncommon lesions with only approximately 300 having been reported to date. Histologically, they are heterogeneous in nature with angiomatous endothelial cysts being quite rare. The computed tomography of a woman with a very large cyst of this variety displayed thin walls with a few septations and foci of soft tissue within. The possibility of a large cystic mass originating from the adrenal gland must be considered in the diagnosis when a large abdominal mass is encountered.

CT findings in abdominal actinomycosis.

The diagnosis of abdominal actinomycosis is difficult because of its relative infrequency and imitation of other diseases. We present four cases in which these difficulties in diagnosis were experienced and show the utility of CT in suggesting this disease. Aspiration, biopsy, and proper culture technique, in conjunction with typical CT findings of transfascial and multiple organ involvement, led to the proper diagnosis in all cases.

Hemorrhagic adrenal metastases: CT findings.

Adrenal hemorrhage secondary to metastases is uncommon. We have encountered four such cases that presented as large adrenal masses. In all cases the CT findings were of an inhomogeneous mixed-density adrenal mass with extensive perirenal changes suggestive of perirenal hemorrhage or mass. When such a lesion is seen, hemorrhagic adrenal metastases should be considered. Fluid may be of high density, suggestive of hemorrhage. However, as the adrenal is in the perinephric space, hemorrhage from any cause (trauma, metastases, or anticoagulant) in the adrenal gland will gravitate into the perinephric space.

Case report. MR and CT appearance of cardiac hemangioma.

We present a case of cardiac hemangioma in a symptomatic patient. MR and CT each have specific characteristics that should make one consider including or excluding this in the differential diagnosis of a cardiac tumor.
