Correction of high serum aluminium in a patient on maintenance haemodialysis.

Withdrawal of oral aluminium hydroxide resulted in a sustained fall of serum aluminium from potentially toxic levels in a patient on maintenance haemodialysis. This result confirms the experience of others, and suggests that this manoeuvre is more appropriate than expensive water treatment in the prevention of aluminium toxicity in patients at risk.

Antistreptococcal antibody titres in the serum of school children in Auckland.

Geometric mean titres for serum antistreptococcal antibodies in a group of 12-year-old Auckland children are presented. The mean titres did not change over a period of 12 months in paired samples. The values obtained are similar to published figures from other parts of the world, suggesting the techniques used provide comparable data.

Prolonged curarisation in the presence of renal impairment.

Prolonged curarisation in the presence of renal failure occurred in six cases where pancuronium was used, and one case where alcuronium was used. The cases are presented with a brief review of the literature. Pancuronium must be used with great caution if postoperative reversal problems are to be avoided. Greater use of adjuvants will reduce requirements and may eliminate the problems encountered in renal failure.

The transient nephrotic syndrome of pregnancy.

The transient nephrotic syndrome was observed in 24 pregnancies in 23 patients, over a five-year period, in a base hospital obstetric unit. The clinical and biochemical features of this syndrome were similar to the nephrotic syndrome observed in non-pregnant patients. In only half the episodes was the patient a primigravida. In three multigravida patients, previous pregnancies were normal. In 10 of the 24 pregnancies, a perinatal death occurred. A bad fetal prognosis was associated with an early onset of the nephrotic syndrome. Of 11 pregnancies in nine patients occurring after the pregnancy complicated by the transient nephrotic syndrome, seven were normal. One pregnancy was complicated by heavy proteinuria, one by hypertension and intra-uterine death, one by an hydatidiform mole, and one was terminated.

A new hereditary cylindromatosis family associated with CYLD1 on chromosome 16.

Hereditary cylindromatosis (HC; MIM 132700) is an autosomal dominant condition characterized by benign skin appendage tumors most commonly on the scalp and face. Previously, the HC gene (CYLD1) was linked to chromosome 16q12-13, and tumors showed loss of heterozygosity (LOH), suggesting that CYLD1 is a tumor suppressor gene. Here we report a new multi-generation cylindromatosis family whose condition maps to that region, with 7/13 tumors showing LOH on 16q.
