The Madigan prostatectomy.

A total of 59 patients with bladder outflow obstruction underwent the Madigan prostatectomy. The method, results and complications are described. For select patients with large benign adenomas without a median lobe or hematuria we believe the technique to be the operation of choice, particularly in younger patients. Advantages include absence of postoperative hematuria and clot retention, a decreased requirement for blood transfusions, a 16F postoperative catheter with early removal, avoidance of post-catheter stricture, a low rate of postoperative urinary infection, a short comfortable period of hospitalization and, in the long term (median followup 6 years), preservation of potency and antegrade ejaculation.

The continent ileostomy and the isolated ileal bladder.

The histories of 19 patients operated on by Kock's technique are reported. Follow-up extended from 3 years 4 months for the first case to 2 weeks for the last. There was one fatality. Some details of technique, contraindications, and complications and their treatment are discussed. Seventeen patients had a total proctocolectomy for ulcerative colitis and 2 had urinary diversion into an isolated ileal reservoir for inoperable carcinoma of the urinary bladder.

Solitary ulcer of the rectum.

Solitary ulcer of the rectum is usually a disease of young adults of either sex which has a characteristic appearance on sigmoidoscopy. Distinctive changes may also be seen in biopsies taken from mucosa adjacent to the ulcer. The name ;solitary ulcer' is misleading because more than one ulcer may be present. Moreover, there is a preulcerative phase which is clinically and histologically recognizable. The condition is essentially benign and may persist for many years unchanged. It has not responded satisfactorily to medical or surgical methods of treatment. The cause of solitary ulcer is unknown. Different views on the pathogenesis are discussed.
