Temporal patterns of alcohol use by alcoholics.

Examination of data gathered from 21 hospitalized alcoholics regarding rank order to time intervals for use (most common to least common) of alcohol showed a significant correspondence to the temporal pattern for drug use in young polydrug users (rho = .943). The most common time for use was 6 PM to 10 PM.

Analysis of morphine and its 3- and 6-glucuronides by high performance liquid chromatography with fluorimetric detection following solid phase extraction from neonatal plasma.

A rapid, sensitive and simple to operate high performance liquid chromatographic method for the simultaneous determination of morphine, morphine-3-glucuronide and morphine-6-glucuronide in plasma is described. The drug and its metabolites were extracted from plasma using commercially available reversed phase octylsilane bonded silica columns (1 mL Bond Elut C8, 50 mg). Chromatographic separation of morphine and its metabolites was achieved using a mobile phase, consisting of 2 mM sodium dodecyl sulphate in 0.05% phosphoric acid:acetonitrile (71.5:28.5 by volume), at a flow-rate of 1.2 mL/min, in conjunction with a Waters Nova-Pak C18 column (300 x 3.9 mm). The analytical column was used in combination with a Guard-Pak module containing a Nova-Pak C18 Guard-Pak insert. Using fluorescence detection (excitation 245 nm, emission 335 nm), plasma levels in the region of 5-10 micrograms/L for the drug and its metabolites can be detected with only 200 microL of plasma. The method has been applied to studies of the disposition of morphine and its metabolites in premature neonates requiring mechanical ventilation who were receiving the drug intravenously; preliminary findings in patients at steady state are presented.

A case of early-stage epithelial ovarian cancer in pregnancy.

There are few reports in the literature of platinum-based chemotherapy administered in pregnancy. We present a case of serous adenocarcinoma of the ovary complicating pregnancy. Following laparotomy at 16 weeks of gestation, four cycles of cisplatin were administered prior to confinement at 32 weeks. There were no neonatal sequelae. We believe there is increasing evidence for the safe use of cisplatin in pregnancy.

Selective block of nerves in the axillary approach to the brachial plexus.

BACKGROUND AND OBJECTIVE: Optimal surgical conditions for flexor tendon tenolysis in the hand were achieved by ulnar, radial, and musculocutaneous nerve block at the axilla combined with median nerve block at the wrist. RESULT: Because the median nerve function proximal to the wrist was preserved, the patient was able to flex the finger intraoperatively. CONCLUSION: Palpation of nerves together with a nerve stimulator were used to localize the nerves accurately.

Keller arthroplasty with autogenous bone graft in the treatment of hallux limitus.

This manuscript presents an alternative surgical procedure in the treatment of hallux limitus. A description of a modified Keller arthroplasty in which the removed base of the proximal phalanx is remodeled and replaced in the first metatarsophalangeal joint is presented. An in-depth review of the procedure has been performed in 220 cases over the last 4 years at the Podiatry Hospital of Pittsburgh, and a study of 25 patients with over 15 months of follow-up is included. Early range of motion, preservation of articular surfaces, no implanted materials, and the option of multiple revisional procedures are the advantages of this procedure.
