Brainstem auditory evoked potentials in a case of 'Manto syndrome', or spasmodic torticollis with thoracic outlet syndrome.

A case of spasmodic torticollis with thoracic outlet syndrome observed for over 18 months is presented and discussed. Maximal head rotation (determining backward gaze) was associated with compression of the brachial plexus between the scaleni muscles and motor, sensory and trophic troubles in the hand. This new syndrome is called after the diviner Manto, quoted by Dante Alighieri in his 'Divina Commedia' (Inferno, XX, 52-56). The etiology was ascribed to subacute toxic effects of methylparathion. Brainstem Auditory Evoked Potentials (BAEPs) demonstrated severe brainstem involvement, maximal in the mesencephalic structures. Clinical and neurophysiological data improved on treatment with L-5-hydroxytryptophan. Finally, BAEPs returned to normal.

[Aspecificity of the immunodiffusion reaction for the diagnosis of non-A non-B viral hepatitis].

A precipitating antigen-antibody system was detected by immunodiffusion using acute and convalescent sera from patients with non-A, non-B (NANB) viral hepatitis in which the diagnosis was made by exclusion. The system showed identity with reference sera. Positivity for antigen was found in 5 out 13 patients with acute NANB hepatitis, but also in 3 out 8 patients with acute A hepatitis, in 6 out of 15 with acute B hepatitis and in 95 out of 221 voluntary blood donors. Antibody was detected in all the above mentioned categories of patients and also in normal subjects in percentages ranging from 9.1 to 25.0 percent. The lack of specificity observed is in contrast with results of other reported serological NANB related systems.

[Treatment of postoperative pain with non-narcotic drugs; evaluation of lysine acetylsalicylate in high doses. Double-blind controlled study].

Further work on the treatment of postoperative pain with non-narcotic analgesics involving an investigation of the effect of twice the customary dose of lysine acetylsalicylate in a double-blind trial versus meperidine is reported. In addition to subjective and objective pain, the parameters included blood pressure, pulse rate, breathing rate, sweating, pupil diameter, nausea, vomiting, somnolence, hiccuping feeling of cold, erythema, dizziness, and nasograstric sound intolerance. Application of Wilcoxon's non-parametric tests and the chi-square test showed that the drug was as good as or better than meperidine with regard to pain, and performed very well overall with respect to the other parameters. The conclusion is drawn that its administration in high doses is justified in many patients subjected to general surgery.

[Greater concentration of anti-hepatitis B antibodies (anti-HBs) in specific hyperimmune human gamma globulins (non-anti-hepatitis B) as compared with normal human gamma globulins].

Twenty-seven lots of standard immunoglobulin from 5 different producers and 18 lots of specific immunoglobulin (non anti-Hepatitis B) were titrated by RIA test for anti-HBs content. The highest titre in lots of standard IG (1:40) was lower than the lowest titre of the lots of specific IG (non anti-Hepatitis B) (1:44). The difference between means (1:17 and 1:42) was statistically significant (p less then 0,01). The reasons for the difference are discussed.

Histoprognostic evaluation of papillary bladder tumors.

A total of 799 cases of papillary bladder tumors were reviewed using both Franksson and Koss classifications. We report the results of a comparative analysis of the two classifications. No correlation between the histologic gradings in the two classifications emerges, especially regarding papilloma (Franksson, 39.3 per cent; Koss, 2.3 per cent). In both classifications recurrences are more frequent as the degree of malignancy increases, although recurrence cannot be excluded even in the lowest grades. As a result of our study, it appears that cellular anaplasia rather than invasion is a better criterion for reliable histoprognostic evaluation of papillary bladder tumors.
