Peripheral chondrosarcoma of the hyoid bone. Report of a case.

The case of a 31-year-old man with a neck mass of 7 months' duration is described. An X-ray examination showed a tumor originating from the right greater horn of the hyoid bone that pushed the larynx and pharynx to the left. Upon visual inspection the tumor seemed to be cartilaginous in nature. Histologically it was a well-differentiated chondrosarcoma that originated from the periphery of the hyoid bone. Ultrastructural study demonstrated an abundant rough endoplasmic reticulum, a particularly well-developed Golgi apparatus, and pleomorphic microvilli. 10 months after excision, a local recurrence invaded the primary marrow spaces of the hyoid bone. A review of the literature indicates that the prognosis of this tumor is unfavorable and that the recurrence of hyoid bone chondrosarcoma generally is less differentiated than the primary tumor.

[Spasmodic torticollis, substantiating Manto syndrome, of possible toxic aethiology, with alterations of brainstem acoustic evoked potentials (BAEPs). Treatment with L-5-hydroxytryptophan. Follow up of 18 months, during which high degree resolution of symptoms and normalization of BAEPs took place].

A case of very severe spasmodic torticollis observed for 18 months is presented and discussed. Head was so rotated that permitted only backward seeing and compressed brachial plexus between scaleni muscles with sensory, motor and trophic troubles in the hand. A toxic aethiology from parathion is likely. Brainstem Acoustic Evoked Potentials (BAEPs) showed in the beginning abnormal responses, especially as refers to waves originating in the mesencephalon. Therapy with L-5-hydroxytryptophan subdued neurological symptoms; a parallel normalization of BAEPs recording was observed. The Authors propose to call this syndrome (spasmodic torticollis with thoracic outlet syndrome) after the mythical diviner Manto, which Dante Alighieri refers to in his "Divina Commedia" (Inferno, XX, 55 e segg.).

[Felice Fontana precursor of neurosciences (author's transl)].

The A.A. insert the life and work of the naturalist and chemist Felice Fontana, born in Pomarolo (Trentino), in the frame of 18th century sciences, beside other great names of that century like Carolus Linnaeus, Réaumur, von Haller, Spallanzani, Morgagni, Priestley and Lavoisier. In the field of general biology, the discovery of nucleus and nucleolus and consequently the discovery of the eukaryotic cell, as we say in our days, in his, as well as the one of anabiosis. The A.A. enucleate and analyse the contributions of Fontana to the neurosciences; he has discovered the axon and the myelinic sheath half century before Remak and Purknije; he found out that the white matter of the brain is made of fibres alike those of nerves and the grey matter is made of globules (i.e. cells) mixed up with fibres; he discovered in the retina a part of coming out from the brain; he described the transversal bands of fibres of the skeletal muscles; he was the first to introduce into physiology the law of "all and nothing"; he attributed the irritability to the whole animal life; he identified the pupillar reflexes to the light, the reflex of accommodation, the consensual reflex, the psycho-emotive mydriasis and at last the myosis of sleep. He made experimental searches about nerves and recognised their regeneration, he enumerated various pathological intracranial masses, he made an important anatomopathological research about hydatid cyst in the brain of the sheep affected by "capostorno" and madness, he demonstrated their parasitical nature (he said that the hydated cysts were covered inside by small animals), he come out to formulate the hypothesis that some neuropsychiatric diseases of man can depend from similar aetiology. He declared that passions may have pathological effects (psyco-somatic aetiology), but he has also drawned the attention against the danager of aprioristical generalisation of neurogenical causes in all diseases. The A.A. give to Fontana the palm of precursor of modern neurosciences and of neurology intended as synthesis and unified corpus of these sciences.

[Further studies of the theory of nervous decussations. I. The phylogenic organization of the optic chiasm].

We introduce in our theory on the biological significance of nervous crossings an hypothesis about phylogenetic origin of optic chiasma. This hypothesis admit a migration of 180 degrees of primitive eyes in ancestors of Chordates. This migration would have been the cause of the crossing of the optic nerves. We analyse the hypothesis by the mean of schemes and discuss it in relation with the old and the new hypothesis about the origin of Chordates from Annelids and respectively from Echinoderms. We present probationary signs from comparative embryology and comparative morphology.

HCV-RNA survival as detected by PCR in the environment.

We have shown that HCV-RNA is resistant to drying at room temperature for at least 48 hours. This is a factor which could influence the diffusion of the virus in the general population, which epidemiological studies have shown to be surprisingly high. It should be considered in evaluating the importance of the inapparent parenteral routes of transmission.

Elevated serum levels of a 90,000 daltons tumor-associated antigen in cancer and in infection by human immunodeficiency virus (HIV).

Levels of a 90,000 daltons monoclonal antibody-defined tumor-associated antigen, termed 90K, were measured in the serum from 649 patients with various types of cancer and 1215 patients infected by the human immunodeficiency virus (HIV). Significantly increased 90K serum levels (12.1 +/- 0.5 U/ml) were found in cancer patients with respect to healthy controls (5.7 +/- 0.3 U/ml), with the highest levels in neoplasms of the breast, lung and gastrointestinal tract. In 355 patients with breast cancer, the elevation of serum 90K levels was more pronounced at advanced stages of disease. Mean levels of 90K for 1215 HIV-infected subjects (21.2 +/- 0.8 U/ml) were significantly higher than controls and cancer patients, and the levels progressively increased with disease worsening from asymptomatic infection to full blown AIDS. These data suggest that 90K is not merely a tumor-associated antigen and lead us to postulate it to be a signalling molecule whose production might be related to the immune deficit caused by pathogenetic events such as neoplastic progression and virus infection.
