Dental arches, where almost all the teeth are missing, have been rehabilitated by prostheses leaving one or at most three contiguous teeth. Cases like this have been resolved with the use of a removable prosthesis with stress distribution by wires so as to obtain axial load on the abutment tooth and uniform distribution on the remaining tooth or teeth and on the alveolar mucosa. Examination 7 cases followed up for a period varying from 2 tp 13 years has shown that such prostheses are extremely functional.
The results obtained with 79 odontoprosthetic rehabilitations by means of removable force distribution prosthesis are reported. Using half-round section wires, this prosthesis distributed masticatory forces axially on the abutment teeth and also distributes them evenly over the edentulous crest, thus achieving a mixed dental and mucous support. Clinical and statistical assessment takes in 79 patients of both sexes aged between 22 and 69 treated over the fifteen year period since 1964. All types of edentulism according to the Kennedy classification were treated. The clinical results were considered, particularly the periodontal situation and the mechanical results, above all friction between retention and function elements of the connecting wires. Clinically, 82,2% of results were positive and 74.6% of the results were mechanically positive also.
Prosthetic rehabilitation of unilateral distal edentulism (Kennedy class 2) and its combination with intercalated absence on the other side (class 2, modification 1) is discussed. Success obtained in a series of 29 cases followed for 9 yr using a removable prosthesis is described. This solved the difficult problems involved by employing metal wires to distribute the forces axially on the abutment teeth and over the unilateral edentulous ridge, thus creating a mixed dental and mucosal support.
Prosthetic rehabilitation in cases of Kennedy class 4th, particularly when the from gap is extensive, presents problems of both aesthetic (lack of front teeth) and functional nature that are not easy to solve. On one side a mixed dental and mucosa support must be obtained while on the other over-loading of the abutment teeth must be avoided by distributing force axially and unformly. This is done by using a special type of prosthesis described here where force is distributed by means of wires. Cases treated like this and followed up have shown positive results as regards both clinical and mechanical results.
