T-cell-rich B-cell lymphoma: a clinicopathologic study of eight cases.

T-cell-rich B-cell lymphoma (TCRBCL) is a recently recognized and ill-defined form of non-Hodgkin's lymphoma (NHL), with no generally accepted diagnostic criteria and with limited information regarding its incidence, cellular origin, morphologic spectrum and biologic behavior. The recent findings suggest that TCRBCL could be a biologically distinct disease characterized by male preponderance, advanced-stage disease initially and high incidence of extranodal localization, especially in the bone marrow. For the time being, proper diagnosis rests on the immunohistochemical identification of the scattered large malignant B-cells amid a sea of small reactive T-lymphocytes. In this study, the clinicopathologic features of 8 patients (pts) with TCRBCL are presented. The male to female ratio was 5/3, and the median age was 52 years (32-67). The disease was advanced in most patients: 5 pts with stage IV and 2 pts with stage III. The patients presented with generalized lymphadenopathy (5), splenomegaly and/or hepatomegaly (4) and bone marrow involvement (4). The diagnosis of TCRBCL was initially established in 6 pts, while the remaining 2 pts were initially diagnosed as having Hodgkin's disease (of mixed cellularity in 1 pt and lymphocytic predominance in another). Revision of the 2 samples comprising immunohistochemistry enabled diagnosis of TCRBCL. Immunohistomorphologically the present series can be differentiated from other types of lymphoma such as lymphocyte-predominant Hodgkin's disease and peripheral T-cell lymphoma.

Posttraumatic stress disorder--effects of psychosocial treatment in children.

In this paper, the authors were researching on expression of posttraumatic stress disorder symptoms and on effect of psychosocial help to children. Existence, time of appearing, way of going through experience and expression of PTSD symptoms were established. There were methodical used structured interviews, Questionnaire for children (Saigh, 1991) and Estimation scale for engagement of children in group work. Refugee and domicile children of age from 7 to 11 years and from 12 to 16 years old were involved in program of psychosocial care. In this research there were more significant the girls than the boys (p < 0.005). Applied therapy methods have given significant effect in increasing of willing to take part in the dialogue (p < 0.005), in increasing of interest to work in group (p < 0.005) and bigger contribute each one within group (p < 0.005). By that, it is being confirmed that the effect of methodical procedure and therapy treatment at children is successful and applicable in future activities.

[Protective colostomy or ileostomy?] ].

A group of patients is presented, where colorectal or coloanal anastomosis after resection was performed together with concomitant covering colostomy (102) or ileostomy (25), during the period 1985-2000. Covering colostomy was most often performed upon hepatic flexure (82.1%) and upon sigmoid colon (8.9%). The incidence of complications during performing or closure of colostomy was 6.8% with one case (0.8%) of death due to unsuccessful protection of primary anastomosis. The incidence of complications in covering ileostomy was 16%, with 3 (12%) deaths. Authors are analysing the data from literature about indications and technique of covering enterostomies, comparing them with their experience and concerning the choice of appropriate protective procedure. They came to conclusion that the reason for covering procedure has been in many instances reasonable, and that in the question of protection both methods are comparable, but covering ileostomy has potentially more dangerous complications. Therefore as a method of anastomotic protection authors are generally recommending covering colostomy except in technically specific cases.

[Evaluation of enterogastric reflux in relation to functional status of the gallbladder].

The aim of the study was estimation of the relation between the gallbladder (GB) motility function and the presence and quantity of enterogastric reflux (EGR). We investigated 172 patients with: physiological GB function (filling and emptying)(FGB), impaired GB function (prolonged filling and ejection fraction < 45%) and afunctional gallbladder (AGB)(without visualization). The study was performed during 90 min (1 f/min) after i.v. application of 185 MB 99mTc-dietil IDA. After 30 min. test meal was given while at the end stomach was marked. According to the parameters from time activity curves over stomach and hepatobiliary system, the index of ERG was calculated, while GB filling and ejection fraction were estimated from the GB time/activity curve. We can conclude that EGR occurs more frequently in the patients with afunctional GB in comparison to those with functional and decreased motor function. Also, EGR quantity is in correlation with the impairment of the GB function.

Determination of anionic and nonionic surfactants, their degradation products, and endocrine-disrupting compounds in sewage sludge by liquid chromatography/mass spectrometry.

A comprehensive analytical method based on reversed-phase liquid chromatography and mass spectrometry using both atmospheric pressure chemical ionization and electrospray ionization has been developed for the simultaneous determination of anionic and nonionic surfactants, their polar degradation products, and endocrine-disrupting compounds (EDCs) in sewage sludge. Extraction of target compounds, with recovery rates from 86% to nearly 100% for polyethoxylates and from 84 to 94% for polar degradation products, was achieved applying ultrasonic solvent extraction with a mixture of methanol/ dichloromethane (7:3, v/v). Cleanup of sample extracts was performed on octadecyl solid-phase extraction cartridges. Determination of less polar compounds: alcohol ethoxylates (AEOs), nonylphenol ethoxylates (NPEOs), coconut diethanol amides, poly(ethylene glycol)s, and phthalate esters was accomplished by reversed-phase LC-APCI-MS in positive ionization mode, while more polar compounds: nonylphenolcarboxylates, nonylphenol (NP), octylphenol, and bisphenol Awere analyzed by ion-pair LC-ESI-MS under negative ionization conditions. This protocol was successfully applied to the trace determination of anionic and nonionic surfactants, polar degradation products, and EDCs in sewage sludge collected from different sewage treatment plants. The analysis revealed the presence of NP at high concentration levels ranging from 25 to 600 mg/kg. Polyethoxylates (AEOs and NPEOs) were also found in all samples at parts-per-million levels (10-190 mg/kg AEOs and 2-135 mg/kg NPEOs, respectively).

Disseminated Burkitt's-like lymphoma during pregnancy.

The occurrence of Burkitt's-like lymphoma (BL) during pregnancy is rarely diagnosed and its outcome is poor. A case of BL localized in the uterus, ovaries and breast during the course of pregnancy is presented. The patient was treated with a combination of surgery and chemotherapy and was disease-free for 6 months after the diagnosis.

The stability of non-ionic surfactants and linear alkylbenzene sulfonates in a water matrix and on solid-phase extraction cartridges.

The stability of nonylphenol ethoxylates (NPEO), alcohol ethoxylates (AEO), coconut diethanol amides (CDEA) and linear alkylbenzene sulfonates (LAS) in a water matrix and preconcentrated on SPE cartridges was studied. A stability study was carried out in a water matrix (spiked ground water and real-world waste water) comparing different pretreatment procedures (addition of sulfuric acid to pH = 3, preservation with 1% and 3% of formaldehyde). When stored in a water matrix serious qualitative and quantitative changes occurred in waste water during the period of time studied (30 days). The losses of C12-C14 alcohol ethoxylates ranged from 72% to 88% when the sample was preserved with acid and from 17% to 86% when the sample was preserved with formaldehyde (3%). Simultaneously, an enrichment of the shorter alkyl chain homologues (C7EO and C10EO) was observed. The losses of NPEO were from 45% (sample preserved by acidification or by addition of 3% of formaldehyde) to 85% (sample preserved with 1% of formaldehyde). Additionally, an increase in concentration of polyethylene glycols (PEGs) and formation of different acidic forms, such as monocarboxylated (MCPEGs) and dicarboxylated polyethylene glycols (DCPEGs) were observed. The stability of surfactants preconcentrated on SPE cartridges was studied as a function of storage time and storage conditions (room temperature, 4 degrees C and -20 degrees C). The results indicate that disposable SPE cartridges can be recommended for the stabilization of non-ionic surfactants and LAS. Storage at -20 degrees C is feasible for long periods (up to 3 months for ground water and up to 2 months for waste water), while storage at 4 C can be recommended for a maximum of 1 month. When cartridges were kept at -20 degrees C the losses of AEOs (n = 12, 13 and 14), preconcentrated from waste water, ranged from 17 to 29% (after 60 days) and other compounds suffered small losses (maximum of 14% for C13LAS). At room temperature, after 7 days, the losses were less than 11%, indicating that shipping of samples by mail can be done without any special requirements.

STAT3 involvement in the acute phase-related expression of the rat haptoglobin gene.

Turpentine-induced acute-phase (AP) response in rats is followed by transcriptional activation of the haptoglobin (Hp) gene in liver. Analysing the promoter sequence of the rat Hp gene we postulated an involvement of Signal Transducer and Activator of Transcription 3 (STAT3) in the regulation of this process. Results obtained by using a combination of Western immunoblot and DNA-binding assays revealed AP-induced binding of constitutive 86kD-and inducible 91kD-STAT3 isoforms to the rat Hp gene inducible promoter element. On the basis of these data we assumed that AP-related interactions of these two STAT3 isoforms correlates with an activated transcriptional status of the rat Hp gene.

[Premalignant and malignant changes in idiopathic inflammatory bowel disease].

It is well known that patients with long-standing inflammatory bowel disease (IBD), ulcerative colitis (UC) or Crohn's disease(CD) are at increased risk for developing colorectal cancer (CC). Before adenocarcinoma develops, the intestinal epithelium progress through a premalignant phase of dysplasia, which can be identified via mucosal biopsy and routine tissue histology. Surveillance colonoscopy and prophylactic colectomy for dysplasia or asymptomatic cancer is advised as a method of reducing cancer-related mortality. Many physicians suggests that surveillance for extensive colitis should begin after 8 to 10 years of disease, and surveillance for left-sided colitis should begin after 15-20 years. Colonoscopy is recommended with frequent biopsies, at least every 10 cm in all four quadrants, and with biopsy of any suspicious lesion. The emerging field of colon cancer genetics has identified several important tumor markers that have potential to improve sensitivity for detection of early neoplasia.

[Surgical treatment of Crohn disease of the small intestine].

The evolution of the surgical treatment of Crohn's disease has three attainable aims: to be minimal, safe and therefore timely. Conservative surgery is increasingly the trend in surgical treatment of small-bowel Crohn's disease. Patients with Crohn's disease are managed non-surgically until complications of the disease or failure to thrive or significant side effects of medication occur, and then--at this point--surgery usually occurs. Once a complication has developed, it is not recommended to wait for this to become further complicated. Resectional surgery and strictureplasty for Crohn's disease of the small bowel are today methods of choice. Laparoscopic intestinal surgery is going to be of a great significance. Abdominal surgery for Crohn's disease is not curative and forms only part of a lifetime management of the patient.

[Metastasis of malignant melanoma in the small intestine--case report].

Small bowel metastases of the malignant melanoma are usually discovered in 2-5% of the patients with malignant melanoma of the skin during the life. They are usually being diagnosed with a complication on the small bowel, such as occlusion, bleeding, anemia and perforation of the intestine. Authors show three patients with metastases of the malignant melanoma in the small bowel, which lead to invagination, subocclusion and anemia due to bleeding. In all the patients, resection of the small bowel was performed with termino-terminal anastomosis. One of them lived 7, the other 4 months after the surgery and is in pretty well condition, while third patient was not controlled at all.

[Surgical treatment of rare anatomic variations and anomalies of the thyroid gland].

Choristoma and thyroid gland malpositions are embryonal disorders. The aim of this article is ten years analysis of their diagnostic and operative treatment. In 1425 thyroidectomies we have found: one lingual, intratracheal, intracardial thyroid and ovarial struma; six separated intrathoracic goitre and 10 mostly intrathoracaly (80%) situated goitre; 1.4% out of all. Correct preoperative diagnosis we have in lingual thyroid, two intrathoracal thyroid and in all mostly intrathoracal thyroids, and in others after histopathological examination. Operative approach was: collar incision with medial sternotomy in nine, collar incision with lateral thoracotomy in three, right thoracotomy in two, and only collar incision in one. There were two operative complications, one recurrent nerve lesion and one mediastinal haematoma. There was no hypocalcaemia and mortality. Thyroid gland anomalies are very rare and associated with lot of problems in diagnostic and treatment. In specialised institutions frequency of operative complications is the same like in routine thyroid gland surgery.

Correlation between acute phase-related expression of C/EBPbeta and transcriptional regulation of the haptoglobin gene during rat liver development.

The hepatic 35 kD nucleoprotein participates in transcriptional regulation of the haptoglobin gene during the development of rat liver. The same molecular mass and trans-acting role of the above protein as the active isoform of C/EBPbeta transcription factor, made homology between them possible. We could detect C/EBPbeta in control hepatic nuclear extracts not earlier than in the second week after the birth. However, the acute-phase reaction induced the expression of 35 kD-C/EBPbeta at day 7 of postnatal development suggesting that the above trans-active nucleoprotein displays the structural and functional characteristics of C/EBPbeta isoform from that day on.

[The presence and significance of Helicobacter pylori in patients with enterogastric reflux].

The aim of the study is assessment of the relationship between enterogastric reflux and the presence of Helicobacter pylori infection as a factors that cause gastritis, peptic ulcer and adenocarcinoma ventriculi. The study was performed in 52 patients with different digestive disorders, using gamma camera, during 90 minutes (1 frame/min) after intravenous injection of 185 MBq 99mTc-dietil IDA in cubital vein. According to time activity curves from the region of hepatobiliary system and gaster, index of enterogastric reflux was assessed (EGR). There is no correlation between the presence of Helicobacter pylori and EGR (r = 0.181, DF = 52, P < 0.05). However, Helicobacter is present more frequently in the patients with positive EGR (p 0.01). However, there is no significant difference (p < 0.05) in reflux value in patients with both positive and negative finding of Helicobacter.

Acute-phase induced phosphorylation of rat liver nucleoprotein p70 modulates its binding affinity for the haptoglobin gene.

An increase in the binding affinity of rat liver trans-acting nucleoprotein p70 for the hormone responsive element of the rat haptoglobin gene in acute-phase reactions has implicated a posttranslational modification. This investigation examines the proposed acute-phase related structural alterations of p70 using an in vitro phosphorylation/dephosphorylation assay and selective digestion of p70 with Staphylococcal aureus V8 protease. The results show that p70 requires phosphorylation to express its DNA-binding ability. Selective proteolysis of p70 provided evidence that acute-phase induced phosphorylation of this protein alters its conformation in such a way that its DNA-binding ability is increased.

Participation of two isoforms of C/EBPbeta transcription factor in the acute-phase regulation of the rat haptoglobin gene.

Previous analyses of the mechanism of the transcriptional induction of the rat haptoglobin (Hp) gene during acute-phase (AP)-reaction have revealed the involvement of several trans-acting nucleoproteins (NPs) in controlling this process. In this study, by using antibodies against C/EBPbeta factor in Western immunoblot assay, we found that rat liver trans-acting NPs p35 and p20 are two characteristic C/EBPbeta isoforms whose expression is induced under AP-conditions. DNA-binding assays identified the binding sites for these two C/EBPbeta proteins in the functionally defined elements A and C of the rat Hp gene and also revealed that they have specific binding affinity towards these elements. Under non-induced conditions, p35 was the only C/EBPbeta binding factor; however, upon AP-conditions both, 35 kDa- and 20 kDa-C/EBPbeta binding activities were significantly induced suggesting that these interactions are necessary for the activation of the Hp gene. By in vitro phosphorylation assay and selective proteolysis, we also present evidence that p35 requires phosphorylation for its DNA binding ability. Thus, we conclude that increase in binding of C/EBPbeta isoforms during AP-reaction occurs through their upregulation and structural modification.

[Surgical treatment of liver metastases of colorectal carcinoma].

Colorectal carcinoma metastasizes into the liver, but liver-only metastases are infrequent. Liver-only metastases are seen mainly from colorectal carcinoma. This is the only metastatic disease where treatment aimed only or mainly at the liver metastases is employed with curative intent. If liver resection for colorectal metastases is done by an experienced team, adhering to predefined indications, five year survival ranges from 30-40%, operative mortality is 3-5% and the postoperative morbidity is acceptable. New diagnostic techniques have been introduced and indications for liver resection extended. This paper presents the current limitations and possibilities for the surgical management of colorectal metastases in the liver.
