Carcinoma of the cardia and proximal third of the stomach. Results of surgical treatment in 91 consecutive patients.

In the present retrospective study, 79 percent of the patients were men with a median age of 65.5 years. Of 91 patients, 84 were surgically explored. Excision of the tumor was carried out in 57 patients (63 percent) and was curative in 28 cases and palliative in 29 cases. Curative resection included proximal gastrectomy in 5 patients and total gastrectomy in 23 patients, which was extended to the spleen and distal pancreas in 18 patients. Lymph node metastases were found in 67 percent of the patients treated by resection. According to the TNM classification, 18 percent of the patients undergoing resection had stage I tumor, 28 percent had stage II tumor, 40 percent had stage III tumor, and 14 percent had stage IV tumor. The postoperative mortality rate was 16 percent for the entire series, 21 percent for patients undergoing palliative resection, and only 4 percent for patients undergoing curative resection. Three patients had an obvious anastomotic leak, one of whom died. Median survival time was 5 months for patients who had exploration only, 6 months for patients who had palliative resection, and 36 months for patients who had potentially curative resection. The 5 year actuarial survival rate was 20 percent for patients having resection and 40 percent for patients having curative resection. Survival was closely related to tumor stage. These results indicate that total gastrectomy with complete abdominal lymph node dissection can give comparatively good long-term results when performed for cure. Nevertheless, only one of three patients in this series could have a potentially curative resection. The only way to increase the curative resection rate and to improve survival is to detect the cancer at an early stage.

Peripheral neuropathy in patients treated with almitrine dimesylate.

Sensory peripheral neuropathy developed in 5 patients treated with almitrine dimesylate, 60-100 mg/day. Onset was insidious, beginning symmetrically in the legs with stocking sensory loss and loss of ankle-jerks. Cerebrospinal fluid protein levels were slightly increased and there was electrophysiological and histological evidence of distal axonopathy. The interval between the first dose of almitrine dimesylate and onset of symptoms ranged from 2 to 4 months in 4 patients. All patients had noted a recent weight loss of 4-15 kg which may have resulted in release of previously bound drug.

[Pancreatico-peritoneal fistula in the newborn infant revealed by a hemorrhagic ascites].

Chronic haemorrhagic ascites in an infant is reported. After much hesitation without a definite diagnosis the first laparotomy was carried out and the estimation of amylase, lipase and proteolytic enzyme activities in the ascitic fluid suggested a pancreatic origin for the ascites. This was conformed at a second laparotomy when the condition was correted. This emphasises the importance of the estimation of pancreatic enzymes in unexplained ascites in children.

[Disturbances of wakefulness, sleep and respiratory function in Steinert's disease].

36 night sleep recordings were carried out on 15 patients suffering from myotonia dystrophica. 9 of these patients complained of diurnal hypersomnia. 10 patients had a disturbance of night sleep with a reduction of REM sleep sometimes associated with interruption of the recording with an increase in the light stages of sleep or alternatively with an increase of REM sleep with a reduction in the latency period of the first paradoxical sleep or with narcoleptic elements. 13 patients had abnormally early abolition of chin EMG activity, almost on falling asleep. 11 cases had pathological apnoeic episodes during sleep and in 9 of the 10 patients who underwent respiratory function studies there was a restrictive airways defect. In addition 9 had frank hypoxia without hypercapnia and 4 a right to left shunt. 3 clinically unaffected patients but with affected relatives were also investigated, 2 were found to have sleep disturbances 1 of which was associated with early abolition of tone.

[The Sjögren-Gougerot syndrome].

The Sjögren-Gougerot syndrome represents a rare disease of the collagenous tissue characterized by the three symptoms Xerophtalmy, Xerostomy, polyarthritis. Women after the menopause are the most affected. Oral symptoms addup to diverse dermatological manifestations. The bilateral parotidic swelling and systemic complications accompany the disease, and the immunological disorder is coupled with hypergammaglobulinemia. Prognosis is very reserved and treatment delicate.

Application of multi-wavelength fluorometry for on-line monitoring of an anaerobic digestion process.

This work examined the use of multi-wavelength fluorometry for on-line monitoring of an anaerobic digestion process. Experiments were carried out in a laboratory-scale anaerobic digestor fed with either synthetic or agricultural (cheese factory) wastewater. An in-line fiber optic probe installed in the external recirculation loop of the reactor was used to acquire fluorescence spectra with an interval of 5-10 min. The spectra were compared with analytical measurements taken at the same time to develop regression models, which were then used to predict concentrations of chemical oxygen demand, volatile fatty acids, and other key process parameters. A comparison of partial least squares (PLS), nonlinear principal components regression, and step-wise regression models on an independent set of data showed that the PLS model gave the best prediction accuracy.

293SF metabolic flux analysis during cell growth and infection with an adenoviral vector.

Metabolic flux quantification of cell culture is becoming a crucial means to improve cell growth as well as protein and vector productions. The technique allows rapid determination of cell culture status, thus providing a tool for further feeding improvements. Herein, we report on key results of a metabolic investigation using 293 cells adapted to suspension and serum-free medium (293SF) during growth and infection with an adenoviral vector encoding the green fluorescence protein (GFP). The model developed contains 35 fluxes, which include the main fluxes of glycolysis, glutaminolysis, and amino acids pathways. It requires specific consumption and production rate measurements of amino acids, glucose, lactate, NH(3), and O(2), as well as DNA and total proteins biosynthesis rate measurements. Also, it was found that extracellular protein concentration measurement is important for flux calculation accuracy. With this model, we are able to describe the 293SF cell metabolism, grown under different culture conditions in a 3-L controlled bioreactor for batch and fed-batch with low glucose. The metabolism is also investigated during infection under two different feeding strategies: a fed-batch starting at the end of the growth phase and extending during infection without medium change and a fed-batch after infection following medium renewal. Differences in metabolism are observed between growth and infection, as well as between the different feeding strategies, thus providing a better understanding of the general metabolism.

Adaptive control at low glucose concentration of HEK-293 cell serum-free cultures.

Fed-batch cultures were implemented to study the metabolism of HEK-293 cells. Glucose, measured every 30 min by a FIA biosensor system, was maintained at 1 mM throughout the culture using an adaptive nonlinear controller based on minimal process modeling. The controller performed satisfactorily at both low and high cell concentrations without the need for retuning between different culture phases. Overall, lactate production was significantly reduced by maintaining a low glucose concentration, thus decreasing the rate of glycolysis. The rates of glucose and glutamine uptake as well as the lactate and ammonia production were compared to those obtained in batch mode with an initial glucose concentration of 21 mM. Basically, three phases were observed in both culture modes. The metabolic shift from the first to the second phase was characterized by a significant reduction in glucose consumption and lactate production while maximum growth rate was maintained. The specific respiration rate appeared unchanged during the first two phases, suggesting that no change occurred in the oxidative pathway capacity. In the third phase, cell growth became slower very likely due to glutamine limitation.

Somatosensory evoked potentials from posterior tibial nerve; normative data.

The somatosensory evoked potentials from posterior tibial nerve were studied in a group of normal adults. The latencies of popliteal fossa potential, L3, N33 and P40 waves as well as central conduction time were analyzed as a function of height and age. The correlation of the peripheral conduction velocity with the subject's age was also studied. The corresponding regression formulae were calculated and can be used to predict normal values of these parameters over a wide range of height and age. Standard error estimates are given for all parameters.

[Basedow's disease: recurrence after surgical treatment. Study of a series of 88 patients].

A homogeneous series of 88 patients with a history of surgery for Basedow's disease was studied retrospectively to evaluate long term recurrences. A total of 88 cases were reviewed after a follow up of a median of 50 months (range 1 to 13 years), and hormone assays were carried out in 74 cases. Surgical complications were minor: no mortality, no definitive hypoparathyroidism, 45% unilateral recurrent nerve palsies and 14.3% definitive hypothyroidism occurring during the first year. Fifteen patients had one or several recurrences of hyperthyroidism. The actuarial recurrence rate at 5 years was 17%, the risk increasing progressively up to the sixth year. The type of operation was the only predictive factor of recurrence surveillance of these patients should be prolonged and involve regular hormone assays, since 6 of the 15 recurrences were poorly symptomatic.
