[Changes in platelet activity in invasive cardiologic procedures].

The authors revealed some changes in the platelet activity in patients with invasive cardiological procedures. Changes of the platelet function were manifested by an enhanced aggregation of platelets in vivo, an increased secretion from alpha granules and increased release of prostaglandin metabolites from platelets and from the vascular wall. Acetylsalicylic acid (ASA) suppressed the formation of circulating platelet aggregates in vivo, but the platelet activity was manifested by another mechanism, independent on ASA. The authors recorded therefore an increase of prostaglandin metabolites and PF4 even in patients who were treated with ASA before the invasive examination.

[Changes in platelet function in patients with arterial hypertension].

The authors examined some indicators of platelet activity in patients with hypertension stage II (according to WHO). They revealed an increased platelet activity which was manifested by an increased concentration of platelet factor 4 (50.2 +/- 28.2 ng/ml) and thromboxane B2 (49.9 +/- 20.1 pg/ml) in plasma. The increase of indicators of platelet activation associated with hypertension is a molecular marker of activation of haemostasis and according to data in the literature it is an indicator of vascular complications in hypertension. It is not clear whether the increased platelet functions in hypertension is primary or an induced change.

[Bleeding time in incisions in patients with thrombocyte transfusion].

The authors used the Simplate test (Organon Teknika) to examine the bleeding time from an incision in 15 healthy blood donors and found normal values of 3.45 +/- 0.90 min. The authors proved a close correlation between the improvement of the number of platelets in 10 patients with thrombocytopenia, treated with thrombocyte infusions and the improvement of the bleeding time when using the Simplate R test after one hour and after 24 hours following thrombocyte administration. Examination of the bleeding time by means of the Simplate R test (Organon Teknika) was well tolerated by the authors' patients and the test does not produce any side-effects.

Migraine--a haematological disorder?

We analysed 22 patients suffering from migraine to evaluate the possible activation of platelet function utilizing the following methods: platelet factor 4, thromboxane B2 and 6 - keto prostaglandin F 1 alpha levels in platelet poor plasma. Laboratory tests were performed on all patients during headache-free period and the results obtained were compared with those of a normal healthy individuals. The mean results obtained in the patients group during pain-free period indicated a significant activation of platelet function when compared with the normal group.

Protein metabolism in specific tissues of endotoxin-treated rats: effect of nutritional status.

Rats received an injection of [14C]leucine and were then divided into four groups. Groups I and II consisted of ad libitum fed rats were administered saline or endotoxin of Salmonella enteritidis eight and twenty-two h after the [14C]leucine treatment. Animals of Group III (saline) and Group IV (endotoxin) fasted after [14C]leucine injection. Twenty three hours after [14C]leucine treatment rats were injected with [3H] leucine and sacrificed 20 min afterwards. Endotoxin administration decreased body weight in fed rats only. After endotoxin treatment, higher [3H]leucine specific activity in the blood plasma, decreased leucine incorporation into proteins and lowered plasma amino acid levels were observed. [14C]leucine radioactivity was significantly higher in the spleen and lower in skeletal muscles of endotoxin-treated rats. All changes were less expressed in fasted than in ad libitum fed animals. Our results indicate that endotoxin treatment results in (a) changes in host metabolism that are not mediated solely by anorexia; (b) a decrease of protein synthesis in the viscera and skeletal muscles; (c) an increase of protein degradation in skeletal muscles; (d) reutilization of leucine released from skeletal muscles in viscera, and (e) a slower disappearance rate of leucine from the blood.

[Diagnosis and therapy of disseminated intravascular coagulation].

The syndrome of disseminated intravascular coagulation (DIC) is an acquired coagulation disorder which is characterized by a significant activation of haemostasis and the formation of intravascular microthrombi, the consumption of coagulation factors and activation of fibrinolysis. DIC has similar clinical stages and a similar laboratory picture, regardless of the causal factor. The diagnosis of DIC should be based on anamnestic, clinical and laboratory data. The laboratory diagnosis of DIC is possible by using relatively simple tests, which have the character of statim examinations. DIC treatment is based on the principle of treatment of the cause, discontinued consumption of haemostatic material, substitution of lacking factors and restoration of the microcirculation. The possible development of DIC must be taken into account in a number of acute conditions, in particular the development of shock syndrome. The presented article summarizes some views on causes, diagnosis and possible treatment of DIC.

Effects of essential phospholipids on the amino acid metabolism in whole body irradiated rats.

Ionizing radiation induces injury to cell membranes resulting in an increase of plasma amino acids concentration and catabolic response of the body. Essential phospholipids (EPL) administered before radiation protect the cell structures and can be used for their renewal. For this reason EPL are expected to have some effect on amino acid metabolism in radiation sickness. The adult male Wistar rats were injected with EPL in a dose of 125 mg EPL/kg b.w. (Essentiale) or saline solution. Three h later whole-body gamma irradiation with a single dose of 10 Gy was performed. Untreated pair-fed rats served as control. The rats were sacrificed 2 days after irradiation. In irradiated rats a decrease in body weight, increased concentrations of plasma amino acids, higher oxidation of branched chain keto acids in liver mitochondria, higher incorporation of leucine into liver proteins and decreased leucine incorporation into proteins of small intestine were observed. In EPL pretreated rats a longer survival time, a higher body weight, lower levels of plasma amino acids and higher protein synthesis in small intestine and in liver when compared with irradiated, saline treated rats were found. No significant changes were observed in branched chain amino acid oxidation measured by means of labeled ketoisocaproate in liver mitochondria.

[Thrombocytes as an indicator of activity in Crohn's disease treated with total parenteral and/or enteral nutrition].

UNLABELLED: Inflammatory bowel disease can be associated with increased platelet count. Aim of our study was focused on assessing thrombocyte count in evaluation of the activity in Crohn's disease (CD). The study included a total of 19 patients [6 men, 13 women (aged 18-57 years, mean 33, median 28]. All patients were malnourished in active stage of the disease on total parenteral and/or enteral nutrition (TP/EN). Thrombocytes were estimated at the beginning and after 3 weeks of TP/EN in venous blood sample using automatic analyser Coulter Counter JT3 (normal range: 130-380.10(9)/l). Serum prealbumin, transferrin, haptoglobin, alpha 1-orosomucoid and C-reactive protein were measured simultaneously. Thrombocytes count below 400.10(9)/l was found only in 1/19 at the beginning and in 7/19 patients in the end of the study. After 3 weeks of TP/EN a significant platelet count fall was found (mean +/- sd: 472 +/- 98 vs. 354 +/- 110, p < 0.01). A correlation was found between thrombocytes and serum prealbumin (r = -0.3871, p < 0.05), haptoglobin (r = 0.5247, p < 0.001), alpha 1-orosomucoid (r = 0.4857, p < 0.01) a C-reactive protein (r = 0.4354, p < 0.01). CONCLUSION: Thrombocyte count helped in the assessing of disease activity in CD on 3-week-TP/EN. Improvement of clinical status, increase of nutritional parameters and decrease of positive acute phase reactants were associated with a fall of platelet count in 15/19 patients.

The platelet functions in stroke.

The share of platelets in the process of atherosclerosis and its thrombotic complications depends on the rate of their activation, which can be assessed by investigating the plasmatic levels of platelet factor 4 (PF4). Authors made use of this method in 68 patients with ischemic stroke and searched for the correlation between the found results, clinical picture and supposed etiopathogenesis of the disease. On the grounds of given findings they reached the conclusion that the increased PF4 levels represent no sign of a cerebral complication but only characterize the group of risky patients threatened by their platelets function status.

[Computer selection of thrombocyte donors based on the HLA system].

Thrombocyte concentrates are vitally important in thrombocytopenic types of haemorrhage. If repeated transfusions are necessary, they are associated with immunological reactions, which cause side-effects in the recipient and the transfusion is ineffective. Donors should be selected according to the HLA system. The authors prepared a programme in the Redap language for selection of donors according to HLA antigens (loci A and B) which respects cross reactions and split antigens. The donor is selected by the grade of agreement with the recipient. The authors differentiate between nine possibilities from complete agreement to complete disagreement between donor and recipient. The initial experience revealed that the programme is feasible for the user and is sufficiently quick.

[The significance of changes in fibrinogen levels during thrombolytic therapy in myocardial infarct].

The submitted work was concerned with two problems: a) investigation of the dynamics of changes of the fibrinogen and fibrin breakdown products after a single dose of streptokinase, b) evaluation of the effectiveness of thrombolysis from changes in the fibrinogen concentration. Two groups of patients were examined with the Q form of acute myocardial infarction who were treated at an intensive care unit. The first group comprised 62 patients (mean age 56.3 years) who were given 1 million units of streptokinase by the i.v. route. The second group comprised 52 patients who were given 1.5 million units of streptokinase. The authors investigated the fibrinogen plasma concentration and the fibrin breakdown product concentration in serum after 3-hour intervals. Concurrently they examined the creatine kinase curve. They proved a significant increase of the fibrinogen and fibrin degradation products before thrombolysis (p less than 0.001). The fibrinogen concentration declined in both groups highly significantly with a maximum after 12 hours. The fibrinogen level reached normal levels in both groups after cca 24 hours. Patients with a high initial fibrinogen level (above 5 g/l) did not attain hypofibrinogenic values during thrombolysis. This phenomenon did not depend on the size of the streptokinase dose. The decline of fibrin breakdown products in both groups declined to baseline values within 24 hours. The authors confirmed that the effectiveness of thrombolysis is influenced among others also by the residual fibrinogen value.

[Preparation of thrombocyte concentrates in the Travenol CS-3000 separator (factors affecting the thrombocyte yield and presence of leukocytes)].

The authors made 184 thrombocytaphereses for the preparation of thrombocyte concentrates on a Travenol CS-3000 separator. They evaluate indicators on which the thrombocyte yield depends: it was found that the yield is higher in donors with a higher number of platelets and that it increases with the period of separation. The mean thrombocyte yield was 2.91 +/- 0.84 x 10(11). It corresponded to the average lower platelet number of donors--182 x 10(9)/L. In those donors where the number of platelets was higher than 180 x 10(9)/L, the yield was 3.21 x 10(9)/L. The undesirable presence of leucocytes (in 98% lymphocytes) increases in relation to the number of leucocytes in the donor and their decline after separation. It was lower in longer separations. The authors discuss the possibility to increase the thrombocyte yield without contamination with leucocytes.

[Selection of a compatible donor using the blood platelet aggregation technic].

The authors assessed the test of platelet aggregation for the selection of a compatible donor. The test was compared with the hitherto used microlymphocytotoxic test. Consistent with data published abroad, the authors confirmed that the aggregation test improves the accuracy of selection of a suitable donor. The test can be performed in a shorter time than the microlymphocytotoxic test, this being important in particular in case of acute haemorrhage.

Changes in the activity of thrombocytes by smoking.

Platelet function was investigated in 11 healthy non-smokers, 11 medium smokers (less than 10 cigarette/day) and 11 heavy smokers (greater than 20 cigarette daily). Cigarette consumption was in medium smokers 6.2 cigarette/day, and in heavy smokers 30.7 cigarette/day. Erythrocyte count, platelet count, plasma levels of cholesterol and triglycerides, and circulating platelet aggregates showed no significant difference between smokers and non-smokers. Thromboxane B2 level in plasma was higher in heavy smokers (20.1 +/- 17.3 pg/0.1 ml) than in non-smokers (58.8 +/- 37.1 pg/0.1 ml) and medium smokers (65.4 +/- 32.1 pg/0.1 ml) (P less than 0.01). Platelet factor 4 level was high in non smokers prior to smoking 1 cigarette and normal in medium and heavy smokers. Plasma concentration of beta thromboglobulin and 6-ketoprostaglandin F1 alpha showed no significant changes between non-smokers and heavy smokers. No dependence on smoking 1 cigarette was found in any group. The data suggest that long term cigarette smoking increases platelet activity manifested by an increased activity of thromboxane A2. Cigarette smoking contributes to the development of coronary artery disease also by means of activation of platelet functions.

[Screening for venous thrombosis and pulmonary embolisms in patients with stroke using an adaptive classifier].

The use of adaptive linear neuron is referred to in recognizing veinous thromboses and lung embolism in a total of 88 acute palsies. As the input signs, the accessible clinical and laboratory data were used. Both training and adaptation of neuron have been performed in a part of total [35 patients], and 53 patients were submitted to the classification of patients with unknown distribution. In classified group with present complications, the screened diseases have been recognized in 16/20 patients. In 29 from 33 patients without thromboembolic states, the distribution by classificator appeared as correct one. The level of correct resolution was as high as 88% in all.
