Good results in detoxification methods have been reached using both together clonidine and opiate receptors antagonists. One hundred fifty-two heroin-abusing patients were studied evaluating withdrawal symptoms after therapy with (a) clonidine only, (b) clonidine and naltrexone, (c) clonidine and naloxone, and (d) placebos. Treatment results, emotional and behavioral changes, and involvement in psychosocial programs were evaluated after a 6-month follow-up. Although opiate antagonists were able to induce slight and transient withdrawal signs and symptoms, there was, in the group of patients treated with clonidine and naltrexone together, a low percentage of catabolites in urine and an improvement in mood and family relationships. Furthermore, the patients that underwent longer naltrexone treatment showed a stronger involvement in psychosocial programs, and even their relatives demonstrated more interest in the recovery program. A decrease in the difficulties of accepting an opiate antagonists treatment and a different evaluation of withdrawal syndrome were the results of an early use of naltrexone.
Fractures of the femoral neck are one of the most important and severe complications of osteoporosis. To obtain up-to-date estimates on the frequency of these fractures in the province of Parma, a city of northern Italy, a retrospective study on the incidence of hip fractures has been carried out examining the operative records of all the orthopedic departments serving the whole province. Between January 1980 and December 1992, 3774 operations for hip fractures were performed, with a marked prevalence in women (2830 women and 944 men) and an exponential increase with age in both sexes. The incidence of extracapsular hip fractures was slightly higher and in women increased more rapidly with age than intracapsular fractures. Approximately 3 fractures out of 4 in women and 2 out of 3 in men occurred after the age of 75. Between 1980 and 1992, the age-adjusted annual incidence rose significantly by 7.1 cases per 100,000 per year in women and by 2.7 cases per 100,000 per year in men. When compared to other populations, these estimates indicate that the incidence of hip fractures in this province of Northern Italy is lower than that reported in other countries, especially in Northern Europe.
Hematin, like many hematoporphyrines and porphyrin derivatives, shows a higher affinity and preferential localization for tumor cells. These properties are particularly interesting in view of their possible applications in tumor treatment. The aim of the present work was to test the capability of porphyrines and resonant low energy gamma rays to produce some kind of selective inhibition of tumor cell proliferation. We investigated the role of hematin to potentiate the killing capability of 14.4 keV resonant gamma rays from a 57Co Mössbauer source. Human osteosarcoma cell cultures "MG-63" were incubated in the presence or absence of hematin (10(-4) to 10(-5) M) for 24 hours. They were successively irradiated for 4 hours with 14.4 keV gamma rays from a 3.7 GBq 57Co source. The combined effects of hematin and resonant gamma radiation were then examined and tested statistically for significance. Different degrees of growth inhibition were observed when hematin alone, radiation alone, and hematin plus gamma rays were administered to the cultures. Hematin, at the concentrations of 10(-4) M is capable of inhibiting tumor cells growth. While no significant effect is attributable to irradiation alone, hematin plus irradiation show a larger inhibition than that expected for purely additive effects.
