Information technologies, health, and "globalization": anyone excluded?

Modern information technologies and worldwide communication through the Internet promise both universal access to information and the globalization of the medico-social network s modes of communication between doctors, laboratories, patients, and other players. The authors, specialists in public health and members of an association that aims to create opportunities for access to training in public health in developing countries, warn that the use of the term "globalization" ignores the reality of the "digital divide," that is, the fact that social inequalities may preclude the realization of this promise on a truly global scale.

[Panmedullary ependymoma. A case report].

Panmedullary ependymoma is an unusual disease last reported by Fuentes in 1986. The present case is the first to have been studied by magnetic resonance, using gadolinium. The findings indicated a panmedullary lesion rather than a tumour. Histopathological investigation confirmed the hypothesis.

Current trends of mental health care in Belgium.

Till 1970, most psychiatric patients were "treated" in legally closeed wards. The nursing and medical personnel were scarce. A fundamental change is now slowly taking place. From a legal point of view, many patients forcedly admitted are no longer under this legal status. From a hospital point of view, new standards have been enforced concerning medical and paramedical personnel, architecture and function. Another fact: in 1972 the closed psychiatric institutes were at last regarded as hospitals. Paralleling these changes, an effort is being made to create mental health services outside of the hospital. All of these modifications should allow the setting up of a curative and preventive psychiatry. The therapeutic function of the hospital should replace the residential function of the psychiatric hospitals.

[Intentions and realizations of hospital planning in psychiatry].

Hospital planning : why? how? from when? in which frame? These are the questions raised in this article. A historical review is made for the criteria used in hospital planning today. The most important steps are related to changes in hospital psychiatry policy: creation of open services in 1960, integration of psychiatric services in the law on hospitals and development of outpatient services in 1970. The authors emphasize the difficulties in planning because of services overlapping and economic difficulties. The outpatients services and the services for "handicapped" should also be part of a general planning. The problems of coordination between these sectors both for planning and functioning are also raised in this article.

[Carbon monoxide exposure in automobile garages: evaluation of mechanics].

Carbon monoxyde (CO) exposure is an occupational hazard for car mechanics. Following two cases of CO poisoning and a preliminary environmental assessment of four car dealerships, a descriptive study was initiated in 76 car dealer repair shops in the Quebec City area to evaluate CO exposure. Alveolar CO of 496 mechanics and ambient CO in 47 car dealer repair shops were measured: 44.6% of non smoker workers had an alveolar CO level greater than 27 ppm which is equivalent to a 5% level of carboxyhemoglobin (HbCO). Ambient CO was greater than 35 ppm in 42.6% of car repair shops, a level considered a risk for chronic health effects. The following measures were proposed to car dealership owners to reduce mechanics' exposure to CO: 1) Install or improve local or general ventilation systems, 2) improve the layout of the repair shops, and 3) prohibit running of car engines without proper connection to the ventilation system.

[Long-acting neuroleptics. IV. Preliminary study of clopimozide (R 29764)].

Clopimozide is, in our opinion, a noteworthy neuroleptic drug. Its action is mainly ataraxic but also antidelusional, long-acting and accompanied with minimal side-effects. It is therefore particularly indicated in the maintenance therapy of psychotics, at a weekly dosage of 7.5 to 25 mg. The present conclusions are preliminary because of the limited number of patients (10) but also because of the rigidity of the trial protocol suggested to us.
