[Periodontal therapy. II. The possible interpretations of the therapeutic results from surgical and nonsurgical treatment].

The authors put forward four different explanations to justify the similarity of published results on the efficacy of different techniques of periodontal therapy. It is stressed that over and above the purely doctrinal evaluation of results obtained using different forms of periodontal therapy, it is indispensable to draw up a treatment protocol for each case taking into account the specific conditions and requirements of the patient to be treated.

[A longitudinal clinical study of the efficacy of tetracyclines combined with root planing in patients with periodontal disease].

A retrospective longitudinal study was performed to assess the effectiveness of tetracycline therapy associated with root planing compared with root planing alone in patients affected by medium to severe periodontal disease. From an analysis of results it appeared that the combined therapy was generally more successful, even though statistical significance was only achieved in cases of healing angular bone lesions. In conclusion, larger studies should be carried out to contribute to broaden the spectrum of indications for the use of antibiotic therapy in periodontal disease in adults.

[A comparative assessment of the capacity of 2 bleeding indices to represent the concentration of thromboxane B2 in the periodontal tissues].

Comparative assessment on the capability of two bleeding indexes to measure the concentration of thromboxane B2 in periodontal tissues. The aim of the study was to compare two bleeding indexes, the Gingival Bleeding Index (GBI) and the Gingival Bleeding Time Index (BTI) in evaluating the intensity of inflammation in deep periodontal tissues measured by the level of thromboxane B2 in the tissues themselves. Results showed that both bleeding indexes were closely correlated to tissue inflammation but that of the two, BTI is more significantly correlated. On the basis of these findings, the authors hope to see the application of bleeding indexes in clinical and research studies which can offer a graduated assessment of bleeding, thus increasing the specificity of this important diagnostic sign.

[A comparative evaluation of oral and parenteral cephalosporins in inhibiting the growth of bacteria pathogenic to the periodontium].

A study was carried out in vitro to assess the inhibitory activity on the growth of Bacteroides gingivalis and Bacteroides intermedius using two oral cephalosporins (Cephalexin and Cephradine) in comparison to two parenteral cephalosporins (Cefotaxime and Ceftriaxon) and the classic Tetracycline HCl. The results of the study show that the inhibitory activity on the growth of the two periodontopathogenic bacteria produced by the oral cephalosporins is comparable to that of other antibiotics studied, and that in comparison to parenteral cephalosporins the former have the advantage of being better accepted and more economic.

[Oral ulcerative lesions in young adults].

Following a general discussion of the pathological and clinical aspects of recurring oral ulcerative lesions most frequently observed in dentistry, the paper illustrates a few typical and atypical clinical cases. In conclusion, the authors make a few comments on the differential diagnosis and therapy of these diseases.

[Modifications in the clinical and microbiological parameters of the periodontal tissues after the removal of overhanging class-II amalgam fillings].

A short-term longitudinal study was carried out to assess how microbiological and clinical characteristics, measured surrounding periodontal sites with amalgam class II fillings intentionally made to overhang, vary following their replacement with a correct form of filling. The results of the study showed a significant reduction of the total bacterial count and the percentages of Gram-negative and anaerobic organisms spreading from overhanging fillings to non-overhanging fillings. The paper concludes that, as has been already demonstrated in the case of onlays, there are both qualitative and quantitative variations in the subgingival plaque in the presence of overhanging fillings which can lead to periodontal damage.

[The reliability of different groups of sample teeth in assessing the status of periodontal disease in epidemiological studies].

The possibility of evaluating periodontal disease status in a given population by examining a limited number of sample teeth is of undoubted value in epidemiological studies. To this end, a cross-section study was performed to assess whether the dental sample proposed by Ramfjord effectively reflects the entire dentition status in terms of the main Clinical Indices of Periodontal Disease, in particular pocket depth. In addition, two randomly selected samples of six teeth were also evaluated using the same criteria. On the basis of their findings, the Authors confirm the epidemiological validity of Ramfjord's dental sample and stress that different groups of sample teeth may be efficaciously used to assess the periodontal disease status of the entire dentition in epidemiological studies.

[The in vitro sensitivity of Bacteroides gingivalis and Bacteroides intermedius to different classes of antibiotic chemical molecules].

A study was carried out to assess the in vitro sensitivity of Bacteroides gingivalis and Bacteroides intermedius to various classes of chemio-antibiotic molecules. The activities of two cephalosporins (cefuroxime and cefotaxime), which have never previously been used for periodontal therapy, were also evaluated. Results showed that the cephalosporins possessed an equal or greater inhibitory activity against the bacteria studied than traditional tetracyclines, and a greater inhibitory activity than other antibiotics studied (metronidazole, ampicillin, piperacillin). Further studies are required to assess in vivo efficacy of cephalosporin therapy in severe periodontal diseases.

[Concentration of thromboxane B2 in gingival tissues in relation to main clinical parameters of periodontal health].

Considering the role played by some metabolites of Arachidonic Acid in the pathogenesis of periodontal disease, the Authors have investigated on the existence of a relation between the Thromboxane B2 concentration in gingival tissues and the principal clinical Indices of periodontal health: Gingival Index, Plaque Index, pocket depth, and Bleeding Time Index. The results showed that just the bleeding Index resulted to be well related to the Thromboxane B2 tissue concentration and then to the level of inflammation of deep periodontal tissues. On the ground of these data the Authors underline the importance held by the bleeding index in the showing to the Clinician the therapy necessity of a periodontal site in order to prevent a possible future attachment loss.

[Prostaglandins in periodontology].

On the basis of critical review of the literature, the Author describes the role played by prostaglandins and other arachidonic acid metabolites in the pathogenesis of the periodontal lesion, emphasizing the possible future applications of such theoretical knowledges in the therapy of periodontal diseases.

[The age factor in periodontics].

In the past, the age of an individual was considered an important factor in the development of periodontal disease and its treatment. Elderly patients were considered to be at higher risk and have a worse prognosis than younger patients who constituted a low risk category for serious periodontal destruction. Through a critical review of the literature it is pointed out that at the moment age as such can no longer be considered a conditioning factor in the evolution of periodontitis and in its treatment. Rather, it is the proneness of individuals to periodontal disease that is of primary importance in determining periodontal damage, in its prognostic evaluation and in its treatment.

[The sign of induced bleeding in the evaluation of individual susceptibility to periodontal disease. Experimental study in diabetic and non-diabetic patients].

The most recent hypotheses on the pathogenesis of periodontal disease have underlined the great importance exerted by individual susceptibility to the disease. The ability of a high ratio between bleeding index and plaque index as a way of identifying subjects at risk has been investigated, the values of this ratio being compared in a group of diabetic patients and in a group of non-diabetics with periodontal lesions of similar extent. The results obtained confirmed those of other Authors, testifying to the good capability of the bleeding/plaque ratio to identify patients with a high risk of developing advanced forms of periodontal disease.

Rapidly progressive periodontitis. Neutrophil chemotaxis inhibitory factors associated with the presence of Bacteroides gingivalis in crevicular fluid.

In the last few years several bacteriological and immunological studies have investigated the role of bacteria and immune defects in order to establish the etiopathogenesis of periodontal disease. With regard to the immune system, a defect in polymorphonuclear neutrophil (PMN) chemotaxis has been frequently reported in patients with rapidly progressive or juvenile periodontitis. The purpose of this study was to investigate in five patients with rapidly progressive periodontitis and normal chemotaxis of peripheral blood PMNs the presence of chemotaxis inhibitory activity in gingival fluid and to relate such activity to three types of bacteria, often involved in rapidly evolving periodontal lesions, that are able to inhibit in vitro PMN chemotaxis: Bacteroides gingivalis, Capnocytophaga sp., and Actinobacillus actinomycetemcomitans. We found strong inhibitory activity in three of these patients. This activity was consistently associated with the finding of B. gingivalis in gingival pockets. We cannot rule out, however, that other substances not of bacterial origin could be responsible for such inhibitory activity. The strict association with B. gingivalis, known to secrete blocking factors, is highly suggestive, although this data must be considered preliminary.
